    Communication: key factor in multidisciplinary system design

    System design research often looks at ways to model the system that is developing. Many modelling techniques and model representations exist. Another aspect these models can be used for is to enable, facilitate and improve communication among the developers during the process. The young System Design Group at the faculty of Engineering Technology of the University of Twente, the Netherlands, aims at focusing on this communication aspect in system design.\ud In the paper, a few finished and running projects undertaken in close cooperation with industry are described concisely. From these projects three research themes are derived. These are: creation of high-level models, combining model representations and condense information. The paper ends with plans for future research

    How competent are healthcare professionals in working according to a bio-psycho-social model in healthcare? : the current status and validation of a scale

    Background : Over the past decades, there has been a paradigm shift from a purely biomedical towards a bio-psycho-social (BPS) conception of disability and illness, which has led to a change in contemporary healthcare. However, there seems to be a gap between the rhetoric and reality of working within a BPS model. It is not clear whether healthcare professionals show the necessary skills and competencies to act according to the BPS model. Objective : The aim of this study was (1) to develop a scale to monitor the BPS competencies of healthcare professionals, (2) to define its factor-structure, (3) to check internal consistency, (4) test-retest reliability and (5) feasibility. Design and Setting : Item derivation for the BPS scale was based on qualitative research with seven multidisciplinary focus groups (n = 58) of both patients and professionals. In a cross-sectional study design, 368 healthcare professionals completed the BPS scale through a digital platform. An exploratory factor analysis was performed to determine underlying dimensions. Statistical coherence was expressed in item-total correlations and in Cronbach's alpha coefficient. An intra-class-correlation coefficient was used to rate the test-retest reliability. Results : The qualitative study revealed 45 items. The exploratory factor analysis showed five underlying dimensions labelled as: (1) networking, (2) using the expertise of the client, (3) assessment and reporting, (4) professional knowledge and skills and (5) using the environment. The results show a good to strong homogeneity (item-total ranged from 0.59 to 0.79) and a strong internal consistency (Cronbach's alpha ranged from 0.75 to 0.82). ICC ranged between 0.82 and 0.93. Conclusion : The BPS scale appeared to be a valid and reliable measure to rate the BPS competencies of the healthcare professionals and offers opportunities for an improvement in the healthcare delivery. Further research is necessary to test the construct validity and to detect whether the scale is responsive and able to detect changes over time

    Mobile phone-based healthcare delivery in a Sami area: Reflections on technology and culture\ud

    This paper analyses the redesign of psychiatric services for children and\ud adolescents in a Sami area in the county of Finnmark in Norway. The project\ud included the introduction of a new technology in support of a decentralized model\ud for healthcare service delivery. We focus specifically on the role of culture in the\ud development and implementation of a mobile phone application during the pilot\ud phase of the project. In our analysis we draw on information infrastructure theory.\ud We are in particular interested in the concept of generativity and critically assess\ud its role of in the analysis of technology in a culturally diverse context

    Trust and Risk Relationship Analysis on a Workflow Basis: A Use Case

    Trust and risk are often seen in proportion to each other; as such, high trust may induce low risk and vice versa. However, recent research argues that trust and risk relationship is implicit rather than proportional. Considering that trust and risk are implicit, this paper proposes for the first time a novel approach to view trust and risk on a basis of a W3C PROV provenance data model applied in a healthcare domain. We argue that high trust in healthcare domain can be placed in data despite of its high risk, and low trust data can have low risk depending on data quality attributes and its provenance. This is demonstrated by our trust and risk models applied to the BII case study data. The proposed theoretical approach first calculates risk values at each workflow step considering PROV concepts and second, aggregates the final risk score for the whole provenance chain. Different from risk model, trust of a workflow is derived by applying DS/AHP method. The results prove our assumption that trust and risk relationship is implicit

    Launched in January 2009, ANCIEN is a research project that runs for a 44-month period and involves 20 partners from EU member states. The project principally concerns the future of long-term care (LTC) for the elderly in Europe and addresses two questions in particular: 1) How will need, demand, supply and use of LTC develop? 2) How do different systems of LTC perform? This case study on Slovakia is part of the first stage in the project aimed at collecting the basic data and necessary information to portray long-term care in each country of the EU. It will be followed by analysis and projections of future scenarios on long-term care needs, use, quality assurance and system performance. State-of-the-art demographic, epidemiologic and econometric modelling will be used to interpret and project needs, supply and use of long-term care over future time periods for different LTC systems

    System Evolution Barriers and How to Overcome Them!

    Creating complex systems from scratch is time consuming and costly, therefore a strategy often chosen by companies is to evolve existing systems. Yet evolving a system is also complicated. Complex systems are usually the result of multidisciplinary teams, therefore it is essential to understand barriers those teams face when evolving a system.\ud From the research carried at Philips Healthcare MRI, we have identified that main evolution barriers employees face are; managing system complexity,communication across disciplines and departments, finding the necessary system information, lack of system overview, and ineffective knowledge sharing. Those barriers were identified as the root cause of many development problems and bad decisions.\ud To overcome those barriers, and therefore enhance the evolution process, effective reuse of knowledge is essential. This knowledge must be presented in a fashion that can be understood by a broad set of stakeholders.     Scaling Up Deliberative Democracy as Dispute Resolution in Healthcare Reform: A Work in Progress

    Simultaneous Localisation and Mapping (SLAM) denotes the problem of jointly localizing a moving platform and mapping the environment. This work studies the SLAM problem using a combination of inertial sensors, measuring the platform's accelerations and angular velocities, and a monocular camera observing the environment. We formulate the SLAM problem on a nonlinear least squares (NLS) batch form, whose solution provides a smoothed estimate of the motion and map. The NLS problem is highly nonconvex in practice, so a good initial estimate is required. We propose a multi-stage iterative procedure, that utilises the fact that the SLAM problem is linear if the platform's rotations are known. The map is initialised with camera feature detections only, by utilising feature tracking and clustering of  feature tracks. In this way, loop closures are automatically detected. The initialization method and subsequent NLS refinement is demonstrated on both simulated and real data
