    A cognitive taxonomy of medical errors

    AbstractObjective. Propose a cognitive taxonomy of medical errors at the level of individuals and their interactions with technology.Design. Use cognitive theories of human error and human action to develop the theoretical foundations of the taxonomy, develop the structure of the taxonomy, populate the taxonomy with examples of medical error cases, identify cognitive mechanisms for each category of medical error under the taxonomy, and apply the taxonomy to practical problems.Measurements. Four criteria were used to evaluate the cognitive taxonomy. The taxonomy should be able (1) to categorize major types of errors at the individual level along cognitive dimensions, (2) to associate each type of error with a specific underlying cognitive mechanism, (3) to describe how and explain why a specific error occurs, and (4) to generate intervention strategies for each type of error.Results. The proposed cognitive taxonomy largely satisfies the four criteria at a theoretical and conceptual level.Conclusion. Theoretically, the proposed cognitive taxonomy provides a method to systematically categorize medical errors at the individual level along cognitive dimensions, leads to a better understanding of the underlying cognitive mechanisms of medical errors, and provides a framework that can guide future studies on medical errors. Practically, it provides guidelines for the development of cognitive interventions to decrease medical errors and foundation for the development of medical error reporting system that not only categorizes errors but also identifies problems and helps to generate solutions. To validate this model empirically, we will next be performing systematic experimental studies

    INJURY-INDUCED HAND DOMINANCE TRANSFER

    Hand dominance is the preferential use of one hand over the other for motor tasks. 90% of people are right-hand dominant, and the majority of injuries (acute and cumulative trauma) occur to the dominant limb, creating a double-impact injury whereby a person is left in a functional state of single-handedness and must rely on the less-dexterous, non-dominant hand. When loss of dominant hand function is permanent, a forced shift of dominance is termed injury-induced hand dominance transfer (I-IHDT). Military service members injured in combat operation may face I-IHDT following mutilating injuries (crush, avulsion, burn and blast wounds) that result in dominant limb amputation or limb salvage. Military occupational therapy practitioners utilize an intervention called Handwriting For Heroes to facilitate hand dominance transfer. This intervention trains the injured military member how to write again using the previously non-dominant hand. Efficacy and clinical effectiveness studies were needed to validate the use of this intervention. This dissertation contains three studies related to I-IHDT. One study measured handwriting performance in adults who previously (greater than 2 years ago) lost function of their dominant hands. Results verified that handwriting performance, when measured on two separate occasions (six-weeks apart) was similar (stable). A second study examined the efficacy of Handwriting For Heroes in non-impaired participants. Results demonstrated a positive effect on the variables that measured the written product: legibility, writing speed (letters-per-minute); as well as a positive effect on the variables that measured the writing process: kinematic and kinetic parameters. The final study examined the clinical effectiveness of Handwriting For Heroes in an injured military population. Results did not show as positive results as the efficacy study, despite similar compliance with the intervention. Specifically, non-impaired participants started with faster writing speeds in their non-dominant hands (higher letters-per-minute) and made more gains (wider ranges). The non-impaired participants also started with faster dexterity (betters scores on the Grooved Pegboard) but they made fewer gains than the injured service members (smaller ranges). Nevertheless, injured participants clearly made gains in all dependent variables thereby demonstrating clinical effectiveness of the interventio

    A Process Writing Curriculum for Kindergarten Through Third Grade

    The curriculum that has been developed for this project is designed to promote the implementation of process writing in kindergarten through third grade classrooms. Writing activities are planned within the content areas and present writing as a natural way to communicate throughout daily activities. The review of research reflects over fifteen years of work that has predominantly been carried out in classroom settings with elementary school students and teachers. Patterns of writing development have been identified that indicate a positive correlation between the students skills attainment and purposeful reading and writing experiences. The plan incorporates individualized instructional strategies that have evolved as a result of the findings of the researchers and teachers who have studied the process writing approach. Procedures for conferencing and teacher modeling techniques are illustrated in this project. The curriculum establishes the atmosphere of the classroom and outlines the stages of the writing process. Journal writing is planned as a daily activity with additional ongoing activities which encourage purposeful writing. The plan for the content areas is consistent with the whole language perspective. A culminating activity is included to provide an opportunity for students to apply the skills they have learned throughout the year. The evaluation of this project was accomplished through interviews with teachers and the presentation of an inservice program. Samples of writing that the students were currently producing were used to identify developmental stages and to model teaching techniques that were suggested in the plan. This curriculum presents writing as an integral part of daily classroom activities. The goal is for children to learn to write by writing and to develop a sense of authorship
