    Readability Formulas and User Perceptions of Electronic Health Records Difficulty: A Corpus Study

    BACKGROUND: Electronic health records (EHRs) are a rich resource for developing applications to engage patients and foster patient activation, thus holding a strong potential to enhance patient-centered care. Studies have shown that providing patients with access to their own EHR notes may improve the understanding of their own clinical conditions and treatments, leading to improved health care outcomes. However, the highly technical language in EHR notes impedes patients\u27 comprehension. Numerous studies have evaluated the difficulty of health-related text using readability formulas such as Flesch-Kincaid Grade Level (FKGL), Simple Measure of Gobbledygook (SMOG), and Gunning-Fog Index (GFI). They conclude that the materials are often written at a grade level higher than common recommendations. OBJECTIVE: The objective of our study was to explore the relationship between the aforementioned readability formulas and the laypeople\u27s perceived difficulty on 2 genres of text: general health information and EHR notes. We also validated the formulas\u27 appropriateness and generalizability on predicting difficulty levels of highly complex technical documents. METHODS: We collected 140 Wikipedia articles on diabetes and 242 EHR notes with diabetes International Classification of Diseases, Ninth Revision code. We recruited 15 Amazon Mechanical Turk (AMT) users to rate difficulty levels of the documents. Correlations between laypeople\u27s perceived difficulty levels and readability formula scores were measured, and their difference was tested. We also compared word usage and the impact of medical concepts of the 2 genres of text. RESULTS: The distributions of both readability formulas\u27 scores (P \u3c .001) and laypeople\u27s perceptions (P=.002) on the 2 genres were different. Correlations of readability predictions and laypeople\u27s perceptions were weak. Furthermore, despite being graded at similar levels, documents of different genres were still perceived with different difficulty (P \u3c .001). Word usage in the 2 related genres still differed significantly (P \u3c .001). CONCLUSIONS: Our findings suggested that the readability formulas\u27 predictions did not align with perceived difficulty in either text genre. The widely used readability formulas were highly correlated with each other but did not show adequate correlation with readers\u27 perceived difficulty. Therefore, they were not appropriate to assess the readability of EHR notes

    Interprofessional education for first year psychology students: career plans, perceived relevance and attitudes

    Undergraduate psychology students have been largely excluded from interprofessional education (IPE) initiatives. In contrast to many health professions, undergraduate psychology students do not engage in work placements as part of their degree, and many enter careers outside the health care context. However, the collaborative skills gained through an IPE experience may well be beneficial to students who work in this wider context. This research examines whether undergraduate psychology students’ views of IPE vary according to their planned career directions, and if so, whether the perceived relevance of IPE mediates the relationships. A sample of 188 Australian university undergraduate psychology students completed an online questionnaire following completion of a first-year IPE health sciences program. Path analysis indicated that psychology students’ attitudes towards IPE are associated with both professional identification and practitioner orientation, fully mediated through the perceived relevance of IPE to future career and study plans. Stronger professional identification and practitioner orientation were associated with greater perceived relevance and more positive and less negative attitudes towards IPE. Placing a stronger emphasis on the generalizability of IP skills taught may increase students’ awareness of the relevance outside of the health context, reducing disengagement of students planning alternative careers

    CELLS: A Parallel Corpus for Biomedical Lay Language Generation

    Recent lay language generation systems have used Transformer models trained on a parallel corpus to increase health information accessibility. However, the applicability of these models is constrained by the limited size and topical breadth of available corpora. We introduce CELLS, the largest (63k pairs) and broadest-ranging (12 journals) parallel corpus for lay language generation. The abstract and the corresponding lay language summary are written by domain experts, assuring the quality of our dataset. Furthermore, qualitative evaluation of expert-authored plain language summaries has revealed background explanation as a key strategy to increase accessibility. Such explanation is challenging for neural models to generate because it goes beyond simplification by adding content absent from the source. We derive two specialized paired corpora from CELLS to address key challenges in lay language generation: generating background explanations and simplifying the original abstract. We adopt retrieval-augmented models as an intuitive fit for the task of background explanation generation, and show improvements in summary quality and simplicity while maintaining factual correctness. Taken together, this work presents the first comprehensive study of background explanation for lay language generation, paving the path for disseminating scientific knowledge to a broader audience.     Interpretation of an international terminology standard in the development of a logic-based compositional terminology

    Purpose: Version 1.0 of the International Classification for Nursing Practice (ICNP®) is a logic-based compositional terminology. International Organization for Standardization (ISO) 18104:2003 Health Informatics¿Integration of a reference terminology model for nursing is an international standard to support the development, testing and implementation of nursing terminologies. Methods: This study examines how ISO 18104:2003 has been interpreted in the development of ICNP® Version 1.0 by identifying mappings between ICNP® and the ISO standard. Representations of diagnostic and interventional statements within ICNP® are also analyzed according to the requirements mandated by the ISO standard. Results: All structural components of ISO 18104:2003 i.e. semantic categories, semantic domains, qualifiers and semantic links are represented either directly or in interpreted form within ICNP®. The formal representations within ICNP® of diagnostic and interventional statements meet the requirement of the ISO standard. Conclusions: The findings of this study demonstrate that ICNP® Version 1.0 conforms to ISO 18104:2003. More importantly perhaps, this study provides practical examples of how components of a terminology standard might be interpreted and it examines how such a standard might be used to support the definition of high-level schemata in developing logic-based compositional terminologies

    Explanatory Models, Unit Standards, and Personalized Learning in Educational Measurement

    The papers by Jack Stenner included in this book document the technical details of an art and science of measurement that creates new entrepreneurial business opportunities. Jack brought theory, instruments, and data together in ways that are applicable not only in the context of a given test of reading or mathematics ability, but which more importantly catalyzed literacy and numeracy capital in new fungible expressions. Though Jack did not reflect in writing on the inferential, constructive processes in which he engaged, much can be learned by reviewing his work with his accomplishments in mind. A Foreword by Stenner's colleague and co-author on multiple works, William P. Fisher, Jr., provides key clues concerning (a) how Jack's understanding of measurement and its values aligns with social and historical studies of science and technology, and (b) how recent developments in collaborations of psychometricians and metrologists are building on and expanding Jack's accomplishments. ​ This is an open access book
