Lumir: The EHR-S In The Basilicata Region
The Lucania – Medici in Rete (LuMiR) project aims to support the changing environment in the Italian National Health Systems, embodying a shift from organisation-centric to patient-centric healthcare service delivery in the Basilicata Region. The project main objective is to foster collaborative, cross-organizational and patient-centric healthcare processes, with a suite of shared e-services supporting the interoperability of active stakeholders’ IT applications and the exchange of patient related clinical information. In the paper the LuMiR project approach and its aim to overcome some limitations of the EHR-S national recommendation are discussed. The methodology adopted in the design and development of the LuMiR system to comply with institutional constraints and to better support a gradual change in the daily working practice of healthcare professionals is described, together with details on the LuMiR system architecture and remarks on the interventions scheduled to cope with possible hindrances for the large scale adoption of the LuMiR system itself
Sharing health data among general practitioners: The Nu.Sa. project
Today, e-health has entered the everyday work flow in the form of a variety of healthcare providers. General practitioners (GPs) are the largest category in the public sanitary service, with about 60,000 GPs throughout Italy. Here, we present the Nu.Sa. project, operating in Italy, which has established one of the first GP healthcare information systems based on heterogeneous data sources. This system connects all providers and provides full access to clinical and health-related data. This goal is achieved through a novel technological infrastructure for data sharing based on interoperability specifications recognised at the national level for messages transmitted from GP providers to the central domain. All data standards are publicly available and subjected to continuous improvement. Currently, the system manages more than 5,000 GPs with about 5,500,000 patients in total, with 4,700,000 pharmacological e-prescriptions and 1,700,000 e-prescriptions for laboratory exams per month. Hence, the Nu.Sa. healthcare system that has the capacity to gather standardised data from 16 different form of GP software, connecting patients, GPs, healthcare organisations, and healthcare professionals across a large and heterogeneous territory through the implementation of data standards with a strong focus on cybersecurity. Results show that the application of this scenario at a national level, with novel metrics on the architecture's scalability and the software's usability, affect the sanitary system and on GPs’ professional activities
Towards a European Health Research and Innovation Cloud (HRIC)
The European Union (EU) initiative on the Digital Transformation of Health and Care (Digicare) aims to provide the conditions necessary for building a secure, flexible, and decentralized digital health infrastructure. Creating a European Health Research and Innovation Cloud (HRIC) within this environment should enable data sharing and analysis for health research across the EU, in compliance with data protection legislation while preserving the full trust of the participants. Such a HRIC should learn from and build on existing data infrastructures, integrate best practices, and focus on the concrete needs of the community in terms of technologies, governance, management, regulation, and ethics requirements. Here, we describe the vision and expected benefits of digital data sharing in health research activities and present a roadmap that fosters the opportunities while answering the challenges of implementing a HRIC. For this, we put forward five specific recommendations and action points to ensure that a European HRIC: i) is built on established standards and guidelines, providing cloud technologies through an open and decentralized infrastructure; ii) is developed and certified to the highest standards of interoperability and data security that can be trusted by all stakeholders; iii) is supported by a robust ethical and legal framework that is compliant with the EU General Data Protection Regulation (GDPR); iv) establishes a proper environment for the training of new generations of data and medical scientists; and v) stimulates research and innovation in transnational collaborations through public and private initiatives and partnerships funded by the EU through Horizon 2020 and Horizon Europe
Linking Research and Policy: Assessing a Framework for Organic Agricultural Support in Ireland
This paper links social science research and agricultural policy through an analysis of support for organic agriculture and food. Globally, sales of organic food have experienced 20% annual increases for the past two decades, and represent the fastest growing segment of the grocery market. Although consumer interest has increased, farmers are not keeping up with demand. This is partly due to a lack of political support provided to farmers in their transition from conventional to organic production. Support policies vary by country and in some nations, such as the US, vary by state/province. There have been few attempts to document the types of support currently in place. This research draws on an existing Framework tool to investigate regionally specific and relevant policy support available to organic farmers in Ireland. This exploratory study develops a case study of Ireland within the framework of ten key categories of organic agricultural support: leadership, policy, research, technical support, financial support, marketing and promotion, education and information, consumer issues, inter-agency activities, and future developments. Data from the Irish Department of Agriculture, Fisheries and Food, the Irish Agriculture and Food Development Authority (Teagasc), and other governmental and semi-governmental agencies provide the basis for an assessment of support in each category. Assessments are based on the number of activities, availability of information to farmers, and attention from governmental personnel for each of the ten categories. This policy framework is a valuable tool for farmers, researchers, state agencies, and citizen groups seeking to document existing types of organic agricultural support and discover policy areas which deserve more attention
Emerging and Established Trends to Support Secure Health Information Exchange
This work aims to provide information, guidelines, established practices and standards,
and an extensive evaluation on new and promising technologies for the implementation
of a secure information sharing platform for health-related data. We focus strictly on
the technical aspects and specifically on the sharing of health information, studying
innovative techniques for secure information sharing within the health-care domain,
and we describe our solution and evaluate the use of blockchain methodologically for
integrating within our implementation. To do so, we analyze health information sharing
within the concept of the PANACEA project that facilitates the design, implementation,
and deployment of a relevant platform. The research presented in this paper provides
evidence and argumentation toward advanced and novel implementation strategies
for a state-of-the-art information sharing environment; a description of high-level
requirements for the transfer of data between different health-care organizations or
cross-border; technologies to support the secure interconnectivity and trust between
information technology (IT) systems participating in a sharing-data “community”;
standards, guidelines, and interoperability specifications for implementing a common
understanding and integration in the sharing of clinical information; and the use of cloud
computing and prospectively more advanced technologies such as blockchain. The
technologies described and the possible implementation approaches are presented in
the design of an innovative secure information sharing platform in the health-care domain
Clinical foundations and information architecture for the implementation of a federated health record service
Clinical care increasingly requires healthcare professionals to access patient record information that
may be distributed across multiple sites, held in a variety of paper and electronic formats, and
represented as mixtures of narrative, structured, coded and multi-media entries. A longitudinal
person-centred electronic health record (EHR) is a much-anticipated solution to this problem, but
its realisation is proving to be a long and complex journey.
This Thesis explores the history and evolution of clinical information systems, and establishes a set
of clinical and ethico-legal requirements for a generic EHR server. A federation approach (FHR) to
harmonising distributed heterogeneous electronic clinical databases is advocated as the basis for
meeting these requirements.
A set of information models and middleware services, needed to implement a Federated Health
Record server, are then described, thereby supporting access by clinical applications to a distributed
set of feeder systems holding patient record information. The overall information architecture thus
defined provides a generic means of combining such feeder system data to create a virtual
electronic health record. Active collaboration in a wide range of clinical contexts, across the whole
of Europe, has been central to the evolution of the approach taken.
A federated health record server based on this architecture has been implemented by the author
and colleagues and deployed in a live clinical environment in the Department of Cardiovascular
Medicine at the Whittington Hospital in North London. This implementation experience has fed
back into the conceptual development of the approach and has provided "proof-of-concept"
verification of its completeness and practical utility.
This research has benefited from collaboration with a wide range of healthcare sites, informatics
organisations and industry across Europe though several EU Health Telematics projects: GEHR,
Synapses, EHCR-SupA, SynEx, Medicate and 6WINIT.

Front-Line Physicians' Satisfaction with Information Systems in Hospitals
Day-to-day operations management in hospital units is difficult due to continuously varying situations, several actors involved and a vast number of information systems in use. The aim of this study was to describe front-line physicians' satisfaction with existing information systems needed to support the day-to-day operations management in hospitals. A cross-sectional survey was used and data chosen with stratified random sampling were collected in nine hospitals. Data were analyzed with descriptive and inferential statistical methods. The response rate was 65 % (n = 111). The physicians reported that information systems support their decision making to some extent, but they do not improve access to information nor are they tailored for physicians. The respondents also reported that they need to use several information systems to support decision making and that they would prefer one information system to access important information. Improved information access would better support physicians' decision making and has the potential to improve the quality of decisions and speed up the decision making process.Peer reviewe
