    Wearable sensors system for an improved analysis of freezing of gait in Parkinson's disease using electromyography and inertial signals

    We propose a wearable sensor system for automatic, continuous and ubiquitous analysis of Freezing of Gait (FOG), in patients affected by Parkinson's disease. FOG is an unpredictable gait disorder with different clinical manifestations, as the trembling and the shuffling-like phenotypes, whose underlying pathophysiology is not fully understood yet. Typical trembling-like subtype features are lack of postural adaptation and abrupt trunk inclination, which in general can increase the fall probability. The targets of this work are detecting the FOG episodes, distinguishing the phenotype and analyzing the muscle activity during and outside FOG, toward a deeper insight in the disorder pathophysiology and the assessment of the fall risk associated to the FOG subtype. To this aim, gyroscopes and surface electromyography integrated in wearable devices sense simultaneously movements and action potentials of antagonist leg muscles. Dedicated algorithms allow the timely detection of the FOG episode and, for the first time, the automatic distinction of the FOG phenotypes, which can enable associating a fall risk to the subtype. Thanks to the possibility of detecting muscles contractions and stretching exactly during FOG, a deeper insight into the pathophysiological underpinnings of the different phenotypes can be achieved, which is an innovative approach with respect to the state of art

    A Hidden Markov Model Based Detecting Solution for Detecting the Situation of Balance During Unsupported Standing Using the Electromyography of Ankle Muscles

    Quiet Standing; Hidden Markov Model; Electromyography; Dynamic Balance.: In this study, three detecting approaches have been proposed and evaluated for online detection of balance situations during quiet standing. The applied methods were based on electromyography of the gastrocnemius muscles adopting the hidden Markov models.Methods: The levels of postural stability during quiet standing were regarded as the hidden states of the Markov models while the zones in which the center of pressure lies within determines the level of stability. The Markov models were trained by using the well-known Baum-Welch algorithm. The performance of a single hidden Markov model, the multiple hidden Markov model, and the multiple hidden Markov model alongside an adaptive neuro-fuzzy inference system (ANFIS), were compared as three different detecting methods.Results: The obtained results show the better and more promising performance of the method designed based on a combination of the hidden Markov models and optimized neuro-fuzzy system.Conclusion: According to the results, using the combined detecting method yielded promising results

    Surface Electromyography and Artificial Intelligence for Human Activity Recognition - A Systematic Review on Methods, Emerging Trends Applications, Challenges, and Future Implementation

    Human activity recognition (HAR) has become increasingly popular in recent years due to its potential to meet the growing needs of various industries. Electromyography (EMG) is essential in various clinical and biological settings. It is a metric that helps doctors diagnose conditions that affect muscle activation patterns and monitor patients’ progress in rehabilitation, disease diagnosis, motion intention recognition, etc. This review summarizes the various research papers based on HAR with EMG. Over recent years, the integration of Artificial Intelligence (AI) has catalyzed remarkable advancements in the classification of biomedical signals, with a particular focus on EMG data. Firstly, this review meticulously curates a wide array of research papers that have contributed significantly to the evolution of EMG-based activity recognition. By surveying the existing literature, we provide an insightful overview of the key findings and innovations that have propelled this field forward. It explore the various approaches utilized for preprocessing EMG signals, including noise reduction, baseline correction, filtering, and normalization, ensure that the EMG data is suitably prepared for subsequent analysis. In addition, we unravel the multitude of techniques employed to extract meaningful features from raw EMG data, encompassing both time-domain and frequency-domain features. These techniques are fundamental to achieving a comprehensive characterization of muscle activity patterns. Furthermore, we provide an extensive overview of both Machine Learning (ML) and Deep Learning (DL) classification methods, showcasing their respective strengths, limitations, and real-world applications in recognizing diverse human activities from EMG signals. In examining the hardware infrastructure for HAR with EMG, the synergy between hardware and software is underscored as paramount for enabling real-time monitoring.     Surface mechanomyography and electromyography provide non-invasive indices of inspiratory muscle force and activation in healthy subjects

    The current gold standard assessment of human inspiratory muscle function involves using invasive measures of transdiaphragmatic pressure (Pdi) or crural diaphragm electromyography (oesEMGdi). Mechanomyography is a non-invasive measure of muscle vibration associated with muscle contraction. Surface electromyogram and mechanomyogram, recorded transcutaneously using sensors placed over the lower intercostal spaces (sEMGlic and sMMGlic respectively), have been proposed to provide non-invasive indices of inspiratory muscle activation, but have not been directly compared to gold standard Pdi and oesEMGdi measures during voluntary respiratory manoeuvres. To validate the non-invasive techniques, the relationships between Pdi and sMMGlic, and between oesEMGdi and sEMGlic were measured simultaneously in 12 healthy subjects during an incremental inspiratory threshold loading protocol. Myographic signals were analysed using fixed sample entropy (fSampEn), which is less influenced by cardiac artefacts than conventional root mean square. Strong correlations were observed between: mean Pdi and mean fSampEn |sMMGlic| (left, 0.76; right, 0.81), the time-integrals of the Pdi and fSampEn |sMMGlic| (left, 0.78; right, 0.83), and mean fSampEn oesEMGdi and mean fSampEn sEMGlic (left, 0.84; right, 0.83). These findings suggest that sMMGlic and sEMGlic could provide useful non-invasive alternatives to Pdi and oesEMGdi for the assessment of inspiratory muscle function in health and disease.Peer ReviewedPostprint (published version

    Fall Prediction and Prevention Systems: Recent Trends, Challenges, and Future Research Directions.

    Fall prediction is a multifaceted problem that involves complex interactions between physiological, behavioral, and environmental factors. Existing fall detection and prediction systems mainly focus on physiological factors such as gait, vision, and cognition, and do not address the multifactorial nature of falls. In addition, these systems lack efficient user interfaces and feedback for preventing future falls. Recent advances in internet of things (IoT) and mobile technologies offer ample opportunities for integrating contextual information about patient behavior and environment along with physiological health data for predicting falls. This article reviews the state-of-the-art in fall detection and prediction systems. It also describes the challenges, limitations, and future directions in the design and implementation of effective fall prediction and prevention systems

    A Review of EMG Techniques for Detection of Gait Disorders

    Electromyography (EMG) is a commonly used technique to record myoelectric signals, i.e., motor neuron signals that originate from the central nervous system (CNS) and synergistically activate groups of muscles resulting in movement. EMG patterns underlying movement, recorded using surface or needle electrodes, can be used to detect movement and gait abnormalities. In this review article, we examine EMG signal processing techniques that have been applied for diagnosing gait disorders. These techniques span from traditional statistical tests to complex machine learning algorithms. We particularly emphasize those techniques are promising for clinical applications. This study is pertinent to both medical and engineering research communities and is potentially helpful in advancing diagnostics and designing rehabilitation devices

    Sensor-Based Adaptive Control and Optimization of Lower-Limb Prosthesis.

    Recent developments in prosthetics have enabled the development of powered prosthetic ankles (PPA). The advent of such technologies drastically improved impaired gait by increasing balance and reducing metabolic energy consumption by providing net positive power. However, control challenges limit performance and feasibility of today’s devices. With addition of sensors and motors, PPA systems should continuously make control decisions and adapt the system by manipulating control parameters of the prostheses. There are multiple challenges in optimization and control of PPAs. A prominent challenge is the objective setup of the system and calibration parameters to fit each subject. Another is whether it is possible to detect changes in intention and terrain before prosthetic use and how the system should react and adapt to it. In the first part of this study, a model for energy expenditure was proposed using electromyogram (EMG) signals from the residual lower-limbs PPA users. The proposed model was optimized to minimize energy expenditure. Optimization was performed using a modified Nelder-Mead approach with a Latin Hypercube sampling. Results of the proposed method were compared to expert values and it was shown to be a feasible alternative for tuning in a shorter time. In the second part of the study, the control challenges regarding lack of adaptivity for PPAs was investigated. The current PPA system used is enhanced with impedance-controlled parameters that allow the system to provide different assistance. However, current systems are set to a fixed value and fail to acknowledge various terrain and intentions throughout the day. In this study, a pseudo-real-time adaptive control system was proposed to predict the changes in the gait and provide a smoother gait. The proposed control system used physiological, kinetic, and kinematic data and fused them to predict the change. The prediction was done using machine learning-based methods. Results of the study showed an accuracy of up to 89.7 percent for prediction of change for four different cases
