    Efficiency as a determinant of loyalty among users of a community of clinical practice: a comparative study between the implementation and consolidation phases

    A community of clinical practice called the Online Communication Tool between Primary and Hospital Care (ECOPIH) was created to enable primary care and specialist care professionals to communicate with each other in order to resolve real clinical cases, thereby improving communication and coordination between care levels. The present work seeks to analyse whether ECOPIH makes it possible to reduce the number of referrals. To that end, the objectives are: (1) To find out the degree of loyalty among ECOPIH users, by comparing the medical professionals' profiles in the tool's implementation phase to those in its consolidation phase. (2) To evaluate the degree of fulfilment of users' expectations, by establishing the determining factors that had an influence on the physicians' intention to use ECOPIH in the implementation phase and observing whether its use had an effective, direct impact on the number of patient referrals that primary care physicians made to specialist care professionals. Two studies were conducted. Based on a survey of all the physicians in a Primary Care area, Study 1 was a descriptive study in ECOPIH's implementation phase. Study 2 was a randomised intervention study of ECOPIH users in the tool's consolidation phase. The results from both studies were compared. Various bivariate and multivariate statistical techniques (exploratory factor analysis, cluster analysis, logistic regression analysis and ANOVA) were used in both studies, which were conducted on a sample of 111 and 178 physicians, respectively. We confirmed the existence of an ECOPIH user profile stable across both phases: under-50-year-old women. Regarding the second objective, there were two particular findings. First, the discriminant factors that had an influence on greater ECOPIH use were habitual Social media website and app use and Perceived usefulness for reducing costs. Second, PC professionals who were ECOPIH members made fewer referrals to SC professionals in Cardiology, Endocrinology and Gastroenterology than older PC professionals who were not ECOPIH members. The use of a community of clinical practice by primary care and specialist care professionals helps to reduce the number of referrals among medical professionals

    Using language technologies to support individual formative feedback

    In modern educational environments for group learning it is often challenging for tutors to provide timely individual formative feedback to learners. Taking the case of undergraduate Medicine, we have found that formative feedback is generally provided to learners on an ad-hoc basis, usually at the group, rather than individual, level. Consequently, conceptual issues for individuals often remain undetected until summative assessment. In many subject domains, learners will typically produce written materials to record their study activities. One way for tutors to diagnose conceptual development issues for an individual learner would be to analyse the contents of the learning materials they produce, which would be a significant undertaking. CONSPECT is one of six core web-based services of the Language Technologies for Lifelong Learning (LTfLL) project. This European Union Framework 7-funded project seeks to make use of Language Technologies to provide semi-automated analysis of the large quantities of text generated by learners through the course of their learning. CONSPECT aims to provide formative feedback and monitoring of learners’ conceptual development. It uses a Natural Language Processing method, based on Latent Semantic Analysis, to compare learner materials to reference models generated from reference or learning materials.     Presenting a New Strategy to Extract Data Clustering Heartbeat Samples by Using Discrete Wavelet Transform

    This paper presents the improvement of detection system that normal and arrhythmia electrocardiogram classification. This classification is done to aid the ANFIS (Adaptive Neuro Fuzzy Inference System). The data used in this paper obtained from MIT-BIH normal sinus ECG database signal and MIT-BIH arrhythmia database signal. The main goal of our approach is to create an interpretable classifier that provides an acceptable accuracy. In this model, the feature extraction using DWT (Discrete Wavelet Transform) is obtained. The last stage of this extraction is introduced as the input of ANFIS model. In this paper, the ANFIS model has been trained with Quantum Behaved Particle Swarm Optimization (QPSO). In this study, for training of proposed model, four sample data have been used which result in acceleration of training data. On the test set, we achieved an outstanding sensitivity and accuracy 100%. Experimental results show that the proposed approach is very fast and accurate in improving classification. Using the proposed methodology and telemedicine technology can manage patient of heart disease

    Social networks : the future for health care delivery

    With the rapid growth of online social networking for health, health care systems are experiencing an inescapable increase in complexity. This is not necessarily a drawback; self-organising, adaptive networks could become central to future health care delivery. This paper considers whether social networks composed of patients and their social circles can compete with, or complement, professional networks in assembling health-related information of value for improving health and health care. Using the framework of analysis of a two-sided network – patients and providers – with multiple platforms for interaction, we argue that the structure and dynamics of such a network has implications for future health care. Patients are using social networking to access and contribute health information. Among those living with chronic illness and disability and engaging with social networks, there is considerable expertise in assessing, combining and exploiting information. Social networking is providing a new landscape for patients to assemble health information, relatively free from the constraints of traditional health care. However, health information from social networks currently complements traditional sources rather than substituting for them. Networking among health care provider organisations is enabling greater exploitation of health information for health care planning. The platforms of interaction are also changing. Patient-doctor encounters are now more permeable to influence from social networks and professional networks. Diffuse and temporary platforms of interaction enable discourse between patients and professionals, and include platforms controlled by patients. We argue that social networking has the potential to change patterns of health inequalities and access to health care, alter the stability of health care provision and lead to a reformulation of the role of health professionals. Further research is needed to understand how network structure combined with its dynamics will affect the flow of information and potentially the allocation of health care resources

    From Text to Knowledge with Graphs: modelling, querying and exploiting textual content

    This paper highlights the challenges, current trends, and open issues related to the representation, querying and analytics of content extracted from texts. The internet contains vast text-based information on various subjects, including commercial documents, medical records, scientific experiments, engineering tests, and events that impact urban and natural environments. Extracting knowledge from this text involves understanding the nuances of natural language and accurately representing the content without losing information. This allows knowledge to be accessed, inferred, or discovered. To achieve this, combining results from various fields, such as linguistics, natural language processing, knowledge representation, data storage, querying, and analytics, is necessary. The vision in this paper is that graphs can be a well-suited text content representation once annotated and the right querying and analytics techniques are applied. This paper discusses this hypothesis from the perspective of linguistics, natural language processing, graph models and databases and artificial intelligence provided by the panellists of the DOING session in the MADICS Symposium 2022
