    Designing Primary Prevention for People Living with HIV

    Today, there are new reasons for a sharper focus on prevention for people living with HIV. Growing numbers of people with the disease are living more healthy, sexual lives. Recent evidence suggests that risk taking among both HIV-positive and negative people is increasing. After nearly two decades of life in the shadow of AIDS, communities are growing weary of traditional prevention messages and many people are openly grappling with difficult questions of intimacy and sex. Increasingly, people living with HIV also face multiple complex economic and substance abuse challenges that complicate prevention efforts.There is an urgent need -- and sufficient expertise -- to move forward with prevention campaigns focused on helping people living with HIV and AIDS avoid passing their infection along to others. Numerous innovative interventions for people with HIV show promise, including:a social marketing campaign for gay men and a five-session group intervention for women living with HIV in Massachusetts,a chat line for positives and a group session program for Latinas/Latinos in Los Angeles,Internet chat room interventions in Atlanta,a group session for gay Asian American-Pacific Islander Americans living with HIV in San Francisco, andPrevention Case Management programs newly funded by the Centers for Disease Control

    The Role of Internalized Homonegativity in the Faith and Psychological Health of Lesbians

    Among lesbians, faith-based beliefs and behaviors may be associated with negative psychological health due to the interplay between religious and sexual identities. The present study examined health outcomes, faith-based beliefs (views of God as loving and controlling), faith-based behaviors (personal spiritual practices, religious activities), and internalized homonegativity in a sample of 225 self-identified lesbians. We hypothesized that internalized homonegativity would moderate the relationship between health outcomes and faith-based beliefs and behaviors among lesbians. Generally, results indicated that some faith-based beliefs and behaviors were related to negative health outcomes among lesbians with higher levels of internalized homonegativity, but among those with lower levels of internalized homonegativity, the negative associations with health were mitigated

    This manual brings together the experiences of an Internet outreach project conducted by theWestern Australian AIDS Council (WAAC) and the Western Australian Centre for Health PromotionResearch (WACHPR) at Curtin University of Technology along with other selected research. It presents guidelines on the development and implementation of Internet outreach programs for health promotion practitioners and peer education workers.It is divided into sections for ease of navigation, including an overview of Internet outreach as a health promotion strategy and the CyberReach project (through the implementation of which the contents were produced). There are additional components to support agencies interested in learning more about whether Internet outreach may be a useful strategy for them in expanding the range of their current services.Who this manual is for: Anyone working in health service delivery with an interest in developing Internet-based health promotion outreach programs.     ANCOVA Study of Psychotherapy Treatment of Internet Pornography Addiction in Heterosexual Men

    Internet pornography has grown to become a problem that exists within the United States and across the globe. For those who suffer from this problematic behavior experience individual and familial problems as well as cause damage to the psyche, professionally and sexually. Those who suffer from addiction do not possess the ability to be able to stop the behavior on their own. Treatment is needed to help internet pornography users and addicts to recover while minimizing relapse from its consumption. This research proposal is a randomized, controlled, clinical ANCOVA study that will determine the effectiveness of Cognitive Behavioral Therapy and Acceptance Commitment Therapy for reducing Internet pornography viewing and behaviors among male heterosexual Internet pornography addicts. Treatment will take place for eight week period and consist of 75 male participants who are randomly assigned to either the CBT, ACT or wait-list control groups. Participants will be assessed at pretest (week 1), posttest (week 9) and three month follow-up (week 13) on the SIS/SES SAST-R, CPUI and AAQ-II measures. The study will define Internet pornography addictive behaviors, negative consequences and explain internal, external, construct and statistical construct validity with regard to the study’s design. Success of the study would provide effective treatment protocol and lessen the destruction of Internet pornography addiction on addicts who seek treatment as well as provide a format for therapists to follow as this is a new type of addiction, which has yet to be universally defined

    Increasing boys' and girls' intention to avoid teenage pregnancy: a cluster randomised control feasibility trial of an interactive video drama based intervention in post-primary schools in Northern Ireland

    Background: Adolescent men have a vital yet neglected role in reducing unintended teenage pregnancy (UTP). There is a need for gender-sensitive educational interventions. Objectives: To determine the value and feasibility of conducting an effectiveness trial of the If I Were Jack Relationship and Sexuality Education (RSE) intervention in a convenience quota sample of post-primary schools in Northern Ireland. Secondary objectives were to assess acceptability to schools, pupils (male/female, aged 14–15 years) and parents/guardians; to identify optimal delivery structures and systems; to establish participation rates and reach, including equality of engagement of different socioeconomic and religious types; to assess trial recruitment and retention rates; to assess variation in normal RSE practice; to refine survey instruments; to assess differences in outcomes for male and female pupils; to identify potential effect sizes that might be detected in an effectiveness trial and estimate appropriate sample size for that trial; and to identify costs of delivery and pilot methods for assessing cost-effectiveness. Design: Cluster randomised Phase II feasibility trial with an embedded process and economic evaluation. Intervention: A teacher-delivered classroom-based RSE resource – an interactive video drama (IVD) with classroom materials, teacher training and an information session for parents – to immerse young people in a hypothetical scenario of Jack, a teenager whose girlfriend is unintentionally pregnant. It addresses gender inequalities in RSE by focusing on young men and is designed to increase intentions to avoid UTP by encouraging young people to delay sexual intercourse and to use contraception consistently in sexual relationships. Main outcome measures: Abstinence from sexual intercourse (delaying initiation of sex or returning to abstinence) or avoidance of unprotected sexual intercourse (consistent correct use of contraception). Secondary outcomes included Knowledge, Attitudes, Skills and Intentions. Results: The intervention proved acceptable to schools, pupils and parents, as evidenced through positive process evaluation. One minor refinement to the parental component was required, namely the replacement of the teacher-led face-to-face information session for parents by online videos designed to deliver the intervention to parents/guardians into their home. School recruitment was successful (target 25%, achieved 38%). No school dropped out. Pupil retention was successful (target 85%, achieved 93%). The between-group difference in incidence of unprotected sex of 1.3% (95% confidence interval 0.55% to 2.2%) by 9 months demonstrated an effect size consistent with those reported to have had meaningful impact on UTP rates (resulting in an achievable sample size of 66 schools at Phase III). Survey instruments showed high acceptability and reliability of measures (Cronbach’s alpha: 0.5–0.7). Economic evaluation at Phase III is feasible because it was possible to (1) identify costs of delivering If I Were Jack (mean cost per pupil, including training of teachers, was calculated as £13.66); and (2) develop a framework for assessing cost-effectiveness. Conclusion: Trial methods were appropriate, and recruitment and retention of schools and pupils was satisfactory, successfully demonstrating all criteria for progression to a main trial. The perceived value of culture- and gender-sensitive public health interventions has been highlighted. Future work: Progression to a Phase III effectiveness trial.     School Counselor Experiences or Perspectives in an Age of Increasing Pornography Use and Sexual Behaviors of Students

    This study explored school counselor interactions or perspectives concerning the impact of pornography use and sexual behaviors among students in K-12 education. Participants, comprised of 51 school counselors in a suburban district in North Carolina, were given a qualitative survey in this case study for the purpose of exploring interactions such as challenging incidents and training needs. Results suggested that school counselors are involved with students impacted by pornography use and sexual behaviors through indirect and direct services, however, school counselors do not report feeling adequately trained to effectively help students in these areas. This study provides implications for further inquiry, further policy, and further training, programming, and implementation of services for school counselors in terms of interacting with students impacted by pornography use and sexual behaviors

    Teen Risk-Taking: Promising Prevention Programs and Approaches

    To help close the knowledge gap and to help program directors, practitioners, and community leaders enlarge the network of effective programs and approaches for at-risk youth, Urban Institute researchers reviewed what is known about successful prevention interventions and their dissemination. They identified 51 problem behavior prevention interventions whose initial effectiveness has been demonstrated through scientific evaluation. A subset of 21 programs was selected on the basis of the rigor of their evaluations or the strength of their results for closer examination of the program elements and/or delivery modes that appeared to be associated with their effectiveness. The researchers also explored with the assistance of experienced prevention scientists and school-based practitioners what might be the essential elements of schools' and other community organizations' readiness to undertake research-based problem behavior prevention programming. This guidebook to promising programs and approaches offers the fruits of that research.
