Primary care physicians' perceptions of barriers and facilitators to management of chronic kidney disease: A mixed methods study.
BackgroundGiven the high prevalence of chronic kidney disease (CKD), primary care physicians (PCPs) frequently manage early stage CKD. Nonetheless, there are challenges in providing optimal CKD care in the primary care setting. This study sought to understand PCPs' perceptions of barriers and facilitators to the optimal management of CKD.Study designMixed methods study.Settings and participantsCommunity-based PCPs in four US cities: Baltimore, MD; St. Louis, MO; Raleigh, NC and San Francisco, CA.MethodologyWe used a self-administered questionnaire and conducted 4 focus groups of PCPs (n = 8 PCPs/focus group) in each city to identify key barriers and facilitators to management of patients with CKD in primary care.Analytic approachWe conducted descriptive analyses of the survey data. Major themes were identified from audio-recorded interviews that were transcribed and coded by the research team.ResultsOf 32 participating PCPs, 31 (97%) had been in practice for >10 years, and 29 (91%) practiced in a non-academic setting. PCPs identified multiple barriers to managing CKD in primary care including at the level of the patient (e.g., low awareness of CKD, poor adherence to treatment recommendations), the provider (e.g., staying current with CKD guidelines), and the health care system (e.g., inflexible electronic medical record, limited time and resources). PCPs desired electronic prompts and lab decision support, concise guidelines, and healthcare financing reform to improve CKD care.ConclusionsPCPs face substantial but modifiable barriers in providing care to patients with CKD. Interventions that address these barriers and promote facilitative tools may improve PCPs' effectiveness and capacity to care for patients with CKD
Designing an automated clinical decision support system to match clinical practice guidelines for opioid therapy for chronic pain
Abstract Background Opioid prescribing for chronic pain is common and controversial, but recommended clinical practices are followed inconsistently in many clinical settings. Strategies for increasing adherence to clinical practice guideline recommendations are needed to increase effectiveness and reduce negative consequences of opioid prescribing in chronic pain patients. Methods Here we describe the process and outcomes of a project to operationalize the 2003 VA/DOD Clinical Practice Guideline for Opioid Therapy for Chronic Non-Cancer Pain into a computerized decision support system (DSS) to encourage good opioid prescribing practices during primary care visits. We based the DSS on the existing ATHENA-DSS. We used an iterative process of design, testing, and revision of the DSS by a diverse team including guideline authors, medical informatics experts, clinical content experts, and end-users to convert the written clinical practice guideline into a computable algorithm to generate patient-specific recommendations for care based upon existing information in the electronic medical record (EMR), and a set of clinical tools. Results The iterative revision process identified numerous and varied problems with the initially designed system despite diverse expert participation in the design process. The process of operationalizing the guideline identified areas in which the guideline was vague, left decisions to clinical judgment, or required clarification of detail to insure safe clinical implementation. The revisions led to workable solutions to problems, defined the limits of the DSS and its utility in clinical practice, improved integration into clinical workflow, and improved the clarity and accuracy of system recommendations and tools. Conclusions Use of this iterative process led to development of a multifunctional DSS that met the approval of the clinical practice guideline authors, content experts, and clinicians involved in testing. The GUIDES checklist: development of a tool to improve the successful use of guideline-based computerised clinical decision support
Background: Computerised decision support (CDS) based on trustworthy clinical guidelines is a key component
of a learning healthcare system. Research shows that the effectiveness of CDS is mixed.
Multifaceted context, system, recommendation and implementation factors may potentially affect the success of
CDS interventions. This paper describes the development of a checklist that is intended to support professionals to
implement CDS successfully.
Methods: We developed the checklist through an iterative process that involved a systematic review of evidence
and frameworks, a synthesis of the success factors identified in the review, feedback from an international expert
panel that evaluated the checklist in relation to a list of desirable framework attributes, consultations with patients
and healthcare consumers and pilot testing of the checklist.
Results: We screened 5347 papers and selected 71 papers with relevant information on success factors for
guideline-based CDS. From the selected papers, we developed a 16-factor checklist that is divided in four domains,
i.e. the CDS context, content, system and implementation domains. The panel of experts evaluated the checklist
positively as an instrument that could support people implementing guideline-based CDS across a wide range of
settings globally. Patients and healthcare consumers identified guideline-based CDS as an important quality
improvement intervention and perceived the GUIDES checklist as a suitable and useful strategy.
Conclusions: The GUIDES checklist can support professionals in considering the factors that affect the success of
CDS interventions. It may facilitate a deeper and more accurate understanding of the factors shaping CDS
effectiveness. Relying on a structured approach may prevent that important factors are missed

ContextClinical decision support systems (CDSSs) can help clinicians assess cardiovascular disease (CVD) risk and manage CVD risk factors by providing tailored assessments and treatment recommendations based on individual patient data. The goal of this systematic review was to examine the effectiveness of CDSSs in improving screening for CVD risk factors, practices for CVD-related preventive care services such as clinical tests and prescribed treatments, and management of CVD risk factors.Evidence acquisitionAn existing systematic review (search period, January 1975\u2013January 2011) of CDSSs for any condition was initially identified. Studies of CDSSs that focused on CVD prevention in that review were combined with studies identified through an updated search (January 2011\u2013October 2012). Data analysis was conducted in 2013.Evidence synthesisA total of 45 studies qualified for inclusion in the review. Improvements were seen for recommended screening and other preventive care services completed by clinicians, recommended clinical tests completed by clinicians, and recommended treatments prescribed by clinicians (median increases of 3.8, 4.0, and 2.0 percentage points, respectively). Results were inconsistent for changes in CVD risk factors such as systolic and diastolic blood pressure, total and low-density lipoprotein cholesterol, and hemoglobin A1C levels.ConclusionsCDSSs are effective in improving clinician practices related to screening and other preventive care services, clinical tests, and treatments. Use of m-Health Technology for Preventive Interventions to Tackle Cardiometabolic Conditions and Other Non-Communicable Diseases in Latin America- Challenges and Opportunities
In Latin America, cardiovascular disease (CVD) mortality rates will increase by an estimated 145% from 1990 to 2020. Several challenges related to social strains, inadequate public health infrastructure, and underfinanced healthcare systems make cardiometabolic conditions and non-communicable diseases (NCDs) difficult to prevent and control. On the other hand, the region has high mobile phone coverage, making mobile health (mHealth) particularly attractive to complement and improve strategies toward prevention and control of these conditions in low- and middle-income countries. In this article, we describe the experiences of three Centers of Excellence for prevention and control of NCDs sponsored by the National Heart, Lung, and Blood Institute with mHealth interventions to address cardiometabolic conditions and other NCDs in Argentina, Guatemala, and Peru. The nine studies described involved the design and implementation of complex interventions targeting providers, patients and the public. The rationale, design of the interventions, and evaluation of processes and outcomes of each of these studies are described, together with barriers and enabling factors associated with their implementation.Fil: Beratarrechea, Andrea Gabriela. Quality Improvement for Cardiovascular Disease Care in Low- and Middle-Income Countries: A Systematic Review
Background
The majority of global cardiovascular disease (CVD) burden falls on people living in low- and middle-income countries (LMICs). In order to reduce preventable CVD mortality and morbidity, LMIC health systems and health care providers need to improve the delivery and quality of CVD care.
Objectives
As part of the Disease Control Priorities Three (DCP3) Study efforts addressing quality improvement, we reviewed and summarized currently available evidence on interventions to improve quality of clinic-based CVD prevention and management in LMICs.
Methods
We conducted a narrative review of published comparative clinical trials that evaluated efficacy or effectiveness of clinic-based CVD prevention and management quality improvement interventions in LMICs. Conditions selected a priori included hypertension, diabetes, hyperlipidemia, coronary artery disease, stroke, rheumatic heart disease, and congestive heart failure. MEDLINE and EMBASE electronic databases were systematically searched. Studies were categorized as occurring at the system or patient/provider level and as treating the acute or chronic phase of CVD.
Results
From 847 articles identified in the electronic search, 49 met full inclusion criteria and were selected for review. Selected studies were performed in 19 different LMICs. There were 10 studies of system level quality improvement interventions, 38 studies of patient/provider interventions, and one study that fit both criteria. At the patient/provider level, regardless of the specific intervention, intensified, team-based care generally led to improved medication adherence and hypertension control. At the system level, studies provided evidence that introduction of universal health insurance coverage improved hypertension and diabetes control. Studies of system and patient/provider level acute coronary syndrome quality improvement interventions yielded inconclusive results. The duration of most studies was less than 12 months.
Conclusions
The results of this review suggest that CVD care quality improvement can be successfully implemented in LMICs. Most studies focused on chronic CVD conditions; more acute CVD care quality improvement studies are needed. Longer term interventions and follow-up will be needed in order to assess the sustainability of quality improvement efforts in LMICs

Computerized clinical decision support systems for chronic disease management: A decision-maker-researcher partnership systematic review
<p>Abstract</p> <p>Background</p> <p>The use of computerized clinical decision support systems (CCDSSs) may improve chronic disease management, which requires recurrent visits to multiple health professionals, ongoing disease and treatment monitoring, and patient behavior modification. The objective of this review was to determine if CCDSSs improve the processes of chronic care (such as diagnosis, treatment, and monitoring of disease) and associated patient outcomes (such as effects on biomarkers and clinical exacerbations).</p> <p>Methods</p> <p>We conducted a decision-maker-researcher partnership systematic review. We searched MEDLINE, EMBASE, Ovid's EBM Reviews database, Inspec, and reference lists for potentially eligible articles published up to January 2010. We included randomized controlled trials that compared the use of CCDSSs to usual practice or non-CCDSS controls. Trials were eligible if at least one component of the CCDSS was designed to support chronic disease management. We considered studies 'positive' if they showed a statistically significant improvement in at least 50% of relevant outcomes.</p> <p>Results</p> <p>Of 55 included trials, 87% (n = 48) measured system impact on the process of care and 52% (n = 25) of those demonstrated statistically significant improvements. Sixty-five percent (36/55) of trials measured impact on, typically, non-major (surrogate) patient outcomes, and 31% (n = 11) of those demonstrated benefits. Factors of interest to decision makers, such as cost, user satisfaction, system interface and feature sets, unique design and deployment characteristics, and effects on user workflow were rarely investigated or reported.</p> <p>Conclusions</p> <p>A small majority (just over half) of CCDSSs improved care processes in chronic disease management and some improved patient health. Policy makers, healthcare administrators, and practitioners should be aware that the evidence of CCDSS effectiveness is limited, especially with respect to the small number and size of studies measuring patient outcomes.</p

Hypertension: Enhancing Lifestyle Management Through Patient Education and Pender’s Health Promotion Model
Background: Uncontrolled hypertension is a significant problem in the United States with 46% of hypertensive adults not achieving adequate blood pressure control. Inadequately treated HTN results in increased risk of renal disease, cardiovascular disease, and associated mortality. Interventions aimed at improving blood pressure control have targeted providers, patients, or a combination of providers and patients. Methods: The objective of this project was to enhance lifestyle management of patients with uncontrolled hypertension as measured by patient self-report in post-discharge interviews. The intervention incorporated education for patients on lifestyle management recommendations for hypertension as well as health promotion strategies inspired by Nola Pender’s Health Promotion Model. Patients chose a goal behavior and collaborated with this DNP student to identify facilitating factors and barriers to achieve these goals utilizing a health promotion worksheet. This intervention also targeted planning strategies by providers and facilitated the development of patient-specific care plans. This DNP project was conducted on two inpatient medical/surgical units outside of Boston, MA. Results: This intervention was completed with a convenience sample of 20 patients. Outcomes, as measured by patient self-report using a Likert scale, revealed that 11 patients always completed the goal behavior (55%), 4 patients sometimes completed the goal behavior (20%) and 3 patients reported never achieving their goal behavior (15%); the outcomes of 2 patients were unknown (10%). Discussion: Although there are several limitations to be considered this intervention shows promise in enhancing lifestyle management of patients with uncontrolled hypertension
Measuring Adherence to Evidence-Based Practice Guidelines for the Management of Hypertension in Women
Approximately 1 in 3 adults In the United States (U. S.) with approximately half being women have hypertension, a significant risk factor for cardiovascular disease (CVD) and stroke (Nwankwo, Yoon, Burt, & Gu, 2013). Nearly 70% of people who had a first heart attack, 80% of those who had a first stroke, and 75% of those who had congestive heart failure (CHF) have hypertension (Mozaffarian et al., 2013). Hypertension was the underlying or contributing cause of more than 375,000 deaths in 2011. Despite multiple publications of national hypertension guidelines, only 50% of adults with hypertension have their blood pressure under control (Nwankwo et al., 2013). One possible explanation for such a high rate of uncontrolled blood pressure is providers may not be adhering to hypertension guidelines.
This capstone report is comprised of three manuscripts that focus on providers and their adherence to hypertension guidelines for women. The first manuscript presents a literature review pertaining to providers and their adherence to hypertension guidelines while discussing factors that affect their adherence such as patient age, gender, and comorbidities. The second manuscript presents an analysis of the 2014 Evidence-Based Practice Guideline for the Management of High Blood Pressure in Adults: Report from the Panel Members Appointed to the Eighth Joint National Committee (JNC 8), using an adapted version of the Appraisal of Guidelines for Research and Evaluation (AGREE) Instrument (2001). The content used to compile the first two manuscripts led to a descriptive study, which examined providers and their adherence to the 2014 JNC 8 evidence-based practice guideline primarily in women at a primary care university clinic. The third and final manuscript consists of the details of this study, and presents some practical implications for improving provider adherence to these evidence-based guidelines
