A Multi-level Analysis on Implementation of Low-Cost IVF in Sub-Saharan Africa: A Case Study of Uganda.
Introduction: Globally, infertility is a major reproductive disease that affects an estimated 186 million people worldwide. In Sub-Saharan Africa, the burden of infertility is considerably high, affecting one in every four couples of reproductive age. Furthermore, infertility in this context has severe psychosocial, emotional, economic and health consequences. Absence of affordable fertility services in Sub-Saharan Africa has been justified by overpopulation and limited resources, resulting in inequitable access to infertility treatment compared to developed countries. Therefore, low-cost IVF (LCIVF) initiatives have been developed to simplify IVF-related treatment, reduce costs, and improve access to treatment for individuals in low-resource contexts. However, there is a gap between the development of LCIVF initiatives and their implementation in Sub-Saharan Africa. Uganda is the first country in East and Central Africa to undergo implementation of LCIVF initiatives within its public health system at Mulago Women’s Hospital.
Methods: This was an exploratory, qualitative, single, case study conducted at Mulago Women’s Hospital in Kampala, Uganda. The objective of this study was to explore how LCIVF initiatives have been implemented within the public health system of Uganda at the macro-, meso- and micro-level. Primary qualitative data was collected using semi-structured interviews, hospital observations informal conversations, and document review. Using purposive and snowball sampling, a total of twenty-three key informants were interviewed including government officials, clinicians (doctors, nurses, technicians), hospital management, implementers, patient advocacy representatives, private sector practitioners, international organizational representatives, educational institution, and professional medical associations. Sources of secondary data included government and non-government reports, hospital records, organizational briefs, and press outputs. Using a multi-level data analysis approach, this study undertook a hybrid inductive/deductive thematic analysis, with the deductive analysis guided by the Consolidated Framework for Implementation Research (CFIR).
Findings: Factors facilitating implementation included international recognition of infertility as a reproductive disease, strong political advocacy and oversight, patient needs & advocacy, government funding, inter-organizational collaboration, tension to change, competition in the private sector, intervention adaptability & trialability, relative priority, motivation &advocacy of fertility providers and specialist training. While barriers included scarcity of embryologists, intervention complexity, insufficient knowledge, evidence strength & quality of intervention, inadequate leadership engagement & hospital autonomy, poor public knowledge, limited engagement with traditional, cultural, and religious leaders, lack of salary incentives and concerns of revenue loss associated with low-cost options.
Research contributions: This study contributes to knowledge of factors salient to implementation of LCIVF initiatives in a Sub-Saharan context. Effective implementation of these initiatives requires (1) sustained political support and favourable policy & legislation, (2) public sensitization and engagement of traditional, cultural, and religious leaders (3) strengthening local innovation and capacity building of fertility health workers, in particular embryologists (4) sustained implementor leadership engagement and inter-organizational collaboration and (5) proven clinical evidence and utilization of LCIVF initiatives in innovator countries. It also adds to the literature on the applicability of the CFIR framework in explaining factors that influence successful implementation in developing countries and offer opportunities for comparisons across studies
The Future of Work and Digital Skills
The theme for the events was "The Future of Work and Digital Skills". The 4IR caused a
hollowing out of middle-income jobs (Frey & Osborne, 2017) but COVID-19 exposed the digital gap as survival depended mainly on digital infrastructure and connectivity. Almost overnight, organizations that had not invested in a digital strategy suddenly realized the need for such a strategy and the associated digital skills. The effects have been profound for those who struggled to adapt, while those who stepped up have reaped quite the reward.Therefore, there are no longer certainties about what the world will look like in a few years from now. However, there are certain ways to anticipate the changes that are occurring and plan on how to continually adapt to an increasingly changing world. Certain jobs will soon be lost and will not come back; other new jobs will however be created. Using data science and other predictive sciences, it is possible to anticipate, to the extent possible, the rate at which certain jobs will be replaced and new jobs created in different industries. Guilt, Blame, and Oppression: A Feminist Philosophy of Scapegoating
In this dissertation I develop a philosophical theory of scapegoating that explains the role of blame-shifting and guilt avoidance in the endurance of oppression. I argue that scapegoating masks and justifies oppression by shifting unwarranted blame onto marginalized groups and away from systems of oppression and those who benefit from them, such that people in dominant positions are less inclined to notice or challenge its workings. I first identify a gap in our understanding of oppression, namely how oppression endures despite widespread formal commitments to principles of equality and justice. I argue that prominent theories of oppression do not place enough weight on the question of how oppression is justified and concealed from us through blame-shifting, in ways that enable its persistence without our explicit approval of its systems. Scapegoating, a concept as old as the Bible, offers potential insight into the means through which we shift blame and avoid responsibility. However, my survey of the genealogy of scapegoating shows that existing conceptualizations of scapegoating are limited in their scope and cannot be applied to explain the endurance of oppression. I propose an ameliorative theory of scapegoating that accounts for deficiencies in prevailing theories of both oppression and scapegoating. Distinct from interpersonal theories and psychologistic analyses of scapegoating, my theory characterizes scapegoating according to its social function in oppression, thereby explaining structural dimensions that are not already captured by other accounts.With the motivation and ingredients in place, I develop my theory of scapegoating as made up of three sub-mechanisms: essentialization of marginalized groups as blameworthy, collective interest in protection against a threat, and social exclusion of the blamed. These sub-mechanisms work together to construct certain groups as scapegoats and encourage us to treat them accordingly through various structural and interpersonal means. I argue that scapegoating has important implications for the formation of social identities; namely, scapegoating constructs social identities in an oppressive arrangement that is largely hidden from us but informs our social and affective relations. By constructing some identities as essentially blameworthy and threatening, dominantly situated identity groups are encouraged to internalize a protected status, act together in defense of their status, and maintain systems of oppressive exclusion. Finally, I elaborate the epistemic dimensions of my theory of scapegoating to argue that scapegoating functions within our social imaginaries and structural epistemic practices. In particular, I focus on the ways that ignorance functions to maintain the scapegoat mechanism, and how scapegoating helps insulate structural forms of ignorance. Digital asset management via distributed ledgers
Distributed ledgers rose to prominence with the advent of Bitcoin, the first provably secure protocol to solve consensus in an open-participation setting. Following, active research and engineering efforts have proposed a multitude of applications and alternative designs, the most prominent being Proof-of-Stake (PoS). This thesis expands the scope of secure and efficient asset management over a distributed ledger around three axes: i) cryptography; ii) distributed systems; iii) game theory and economics. First, we analyze the security of various wallets. We start with a formal model of hardware wallets, followed by an analytical framework of PoS wallets, each outlining the unique properties of Proof-of-Work (PoW) and PoS respectively. The latter also provides a rigorous design to form collaborative participating entities, called stake pools. We then propose Conclave, a stake pool design which enables a group of parties to participate in a PoS system in a collaborative manner, without a central operator. Second, we focus on efficiency. Decentralized systems are aimed at thousands of users across the globe, so a rigorous design for minimizing memory and storage consumption is a prerequisite for scalability. To that end, we frame ledger maintenance as an optimization problem and design a multi-tier framework for designing wallets which ensure that updates increase the ledger’s global state only to a minimal extent, while preserving the security guarantees outlined in the security analysis. Third, we explore incentive-compatibility and analyze blockchain systems from a micro and a macroeconomic perspective. We enrich our cryptographic and systems' results by analyzing the incentives of collective pools and designing a state efficient Bitcoin fee function. We then analyze the Nash dynamics of distributed ledgers, introducing a formal model that evaluates whether rational, utility-maximizing participants are disincentivized from exhibiting undesirable infractions, and highlighting the differences between PoW and PoS-based ledgers, both in a standalone setting and under external parameters, like market price fluctuations. We conclude by introducing a macroeconomic principle, cryptocurrency egalitarianism, and then describing two mechanisms for enabling taxation in blockchain-based currency systems

A high-performance open-source framework for multiphysics simulation and adjoint-based shape and topology optimization
The first part of this thesis presents the advances made in the Open-Source software SU2,
towards transforming it into a high-performance framework for design and optimization of
multiphysics problems. Through this work, and in collaboration with other authors, a tenfold
performance improvement was achieved for some problems. More importantly, problems that
had previously been impossible to solve in SU2, can now be used in numerical optimization
with shape or topology variables. Furthermore, it is now exponentially simpler to study new
multiphysics applications, and to develop new numerical schemes taking advantage of modern
high-performance-computing systems.
In the second part of this thesis, these capabilities allowed the application of topology optimiza-
tion to medium scale fluid-structure interaction problems, using high-fidelity models (nonlinear
elasticity and Reynolds-averaged Navier-Stokes equations), which had not been done before
in the literature. This showed that topology optimization can be used to target aerodynamic
objectives, by tailoring the interaction between fluid and structure. However, it also made ev-
ident the limitations of density-based methods for this type of problem, in particular, reliably
converging to discrete solutions. This was overcome with new strategies to both guarantee and
accelerate (i.e. reduce the overall computational cost) the convergence to discrete solutions in
fluid-structure interaction problems.Open Acces
SUBSUMPTION AS DEVELOPMENT: A WORLD-ECOLOGICAL CRITIQUE OF THE SOUTH KOREAN "MIRACLE"
This work offers a critical reinterpretation of South Korean "economic development" from the perspectives of Marxian form critique and Jason Moore's world-ecology. Against the "production in general" view of economic life that dominates the extant debates, it analyzes the rise, spread, and deepening of capitalism's historically specific social forms in twentieth-century (South) Korea: commodity, wage-labor, value, and capital. Eschewing the binary language of development and underdevelopment, we adopt Marx's non-stagist distinctions regarding the relative degree of labor's (and society's) subsumption under capital: hybrid, formal, and real. Examining the (South) Korean experience across three dialectically interrelated scales – regional, global, and "national" – we outline the historical-geographical contingency surrounding South Koreas emergence by c.1980 as a regime of (industrialized) real subsumption, one of the only non-Western societies ever to do so. Crucial to this was the generalization of commodification and proletarianization that betokened deep structural changes in (South) Korea's class structure, but also a host of often-mentioned issues such as land reform, foreign aid, the developmental state, and a "heaven sent" position within the US-led Cold War order. Despite agreeing on the importance of these latter factors, however, the conclusions we draw from them differ radically from those of the extant analyses. For although regimes of real subsumption are the most materially, socially, and technologically dynamic, they are also the most socio-ecologically unsustainable and alienating due to the dualistic tensions inherent to capital's "fully developed" forms, in particular the temporal grounding of value. US protestations about the generalizability of these relations aside, moreover, these regimes have always been in the extreme minority and, crucially, have depended on less developed societies for their success. Historically, this has been achieved through widening the net of capitalist value relations; however, four decades of neoliberalization has all but eliminated any further large-scale "frontier strategies" of this sort. Due to its relatively dense population vis-a-vis its geographical size, contemporary South Korea faces stark challenges that render it anything but a model of "sustainable development," but rather signal the growing anachronism of value as the basis for regulating the future of nature-society relations in the "developed world" and beyond
