    Performance and enhancement for HD videoconference environment

    In this work proposed here is framed in the project of research V3 (Video, Videoconference, and Visualization) of the Foundation i2CAT, that has for final goal to design and development of a platform of video, videoconference and independent visualization of resolution in high and super though inside new generation IP networks. i2CAT Foundation uses free software for achieving its goals. UltraGrid for the transmission of HD video is used and SAGE is used for distributed visualization among multiple monitors. The equipment used for management (capturing, sending, visualization, etc) of the high definition stream of work environment it has to be optimized so that all the disposable resources can be used, in order to improve the quality and stability of the platform. We are speaking about the treatment of datum flows of more of 1 Gbps with raw formats, so that the optimization of the use of the disposable resources of a system is given back a need. In this project it is evaluated the requirements for the high definition streams without compressing and a study of the current platform is carried out, in order to extract the functional requirements that an optimum system has to have to work in the best conditions. From this extracted information, a series of systems tests are carried out in order to improve the performance, from level of network until level of application. Different distributions of the Linux operating system have been proved in order to evaluate their performance. These are Debian 4 and openSUSE 10.3. The creation of a system from sources of software has also been proved in order to optimize its code in the compilation. It has been carried out with the help of Linux From Scratch project. It has also been tried to use systems Real Time (RT) with the distributions used. It offers more stability in the stream frame rate. Once operating systems has been test, it has proved different compilers in order to evaluate their efficiency. The GCC and the Intel C++ Compilers have proved, this second with more satisfactory results. Finally a Live CD has been carried out in order to include all the possible improvements in a system of easy distribution

    OBJECTIVE: A review of systematic reviews of the effectiveness and feasibility of videoconference-based telepsychiatry services for resource constrained environments was conducted. Specifically with the aim of producing an evidence-based review of the effectiveness and feasibility of videoconference-based telepsychiatry services in resource constrained countries like South Africa. METHODS: Eight key questions on telepsychiatry effectiveness and feasibility were identified and inclusion and exclusion criteria were developed. Review of citations from 2000-2011 from CINAHL, Medline, Pubmed, PsycInfo, EBSCOhost, SABINET, Cochrane Database of Systematic Reviews (CCTR), Cochrane Controlled Trial Registry (CCTR), Database of Abstracts of Reviews of Effectiveness (DARE), unpublished abstracts through NEXUS and internet search engines (Google/Google scholar) was conducted. RESULTS: Ten systematic reviews were included for review. Despite the methodological limitations and heterogeneity of the systematic reviews, there appears to be good evidence of effectiveness (reliability and improved outcomes) and feasibility (use, satisfaction, acceptability and cost) for videoconference-based telepsychiatry internationally. The application of this evidence in lower middle income countries is dependent on the integration of telepsychiatry into local health system contexts. CONCLUSION: Based on the evidence, resource constrained countries such as South Africa should be encouraged to develop telepsychiatry programs along with rigorous evaluation methods.Web of Scienc

    Telepresence learning environments for opera singing, a case study

    The present study analyzes the data obtained in the execution of the Opera eLearning project, a multidisciplinary effort to develop a solution for Opera singing distance lessons at the graduate level, using high bandwidth to deliver quality audio and video experience that has been evaluated by singing teachers, chorus and orchestra directors, singers and other professional musicians. The research work includes the phases of design, execution and evaluation of pilot tests, followed by further development and execution of several experimental exercises with the system, all of them carried out between July 2008 and April 2009. This is an empirical research, an exploratory case study that has provided enough data to arrive to a sustainable model for a telepresence learning environment. Different usability methods have been implemented in order to assure users of the quality of the product. The main objective is to prove whether the system or artifact proposed can be used to deliver a complete remote singing class at a higher education level; for that purpose, we have defined several research categories that describe the usability of the system in multiple dimensions. We have used “design as research” approaches to promote innovation in the technological area. The theoretical framework is based on a wide variety of fields; from acoustics, physics, music, professional singing to telecommunications and multimedia technology. However, the common thread and central issue under analysis is distance education, through the construction of a remote learning system. El trabajo de investigación incluye las fases de diseño, ejecución y evaluación de las pruebas piloto, seguido del posterior desarrollo y ejecución de varios ejercicios experimentales con el sistema, todos ellos efectuados entre Julio de 2008 y Abril de 2009. Esta es una investigación empírica, un caso de estudio exploratorio que ha obtenido datos suficientes como para definir un modelo sostenible de entorno de enseñanza por telepresencia. Diversos métodos de usabilidad fueron implementados con el fin de asegurar a los usuarios la calidad del producto. All right reserved.Background: Despite the potential of digital mental health to provide cost-effective mental health care, its adoption in clinical settings is limited, and little is known about the perspectives and practices of mental health professionals regarding its implementation or the factors influencing these perspectives and practices. Objective: This study aims to characterize in depth the perspectives and practices of mental health professionals regarding the implementation of digital mental health and explore the factors affecting such perspectives and practices. Methods: A qualitative study using in-depth semistructured interviews with Portuguese mental health professionals (N=13)-psychologists and psychiatrists-was conducted. The transcribed interviews were thematically analyzed. Results: Mental health professionals deemed important or engaged in the following practices during the implementation of digital mental health: indication evaluation, therapeutic contract negotiation, digital psychological assessment, technology setup and management, and intervention delivery and follow-up. Low-threshold accessibility and professionals' perceived duty to provide support to their clients facilitated the implementation of digital mental health. Conversely, the lack of structured intervention frameworks; the unavailability of usable, validated, and affordable technology; and the absence of structured training programs inhibited digital mental health implementation by mental health professionals. Conclusions: The publication of practice frameworks, development of evidence-based technology, and delivery of structured training seem key to expediting implementation and encouraging the sustained adoption of digital mental health by mental health professionals.publishersversionpublishe

    Grounds for a New Normal: Integration of Telenephrology in Rural Communities

    Patients with kidney disease represent a group of people who are known to have comorbidities, high costs of care, decreased quality of life, and invasive interventions. In rural communities of Western Washington State, many nephrology patients experience factors evidenced to impede access to quality and timely management of their disease, compounding the risk for poor outcomes. Telenephrology is a novel modality of service delivery with the potential to make care more efficient, cost-effective, flexible, and accessible. The author created this project to develop a telenephrology program plan and evaluation for rural settings in Western Washington, informed by relevant epidemiology, pathophysiology, standards of care, and evidence in the literature. As there are a paucity of program plans specific to this service delivery in rural Western Washington, development of this project aims to substantiate the need for evidence-based implementation in this setting. The appraisal of barriers to care for this patient population was used to guide aspects of telenephrology program planning and evaluation to ensure intervention outcomes are equitable. This project provides grounds to support the adoption a new normal for nephrology care and utilizes the existing evidence to inform the planning and evaluating of telenephrology services, implementation processes, budget, and outcomes to assist healthcare providers and their institutions in successful integration of telenephrology in rural communities of Western Washington State
