    What factors are associated with the use of teletrauma in northern British Columbia?

    Despite the existence of universal health care for Canadians, health inequalities persist. Those residing in rural regions of Canada may be at a disadvantage for accessing appropriate services. To enhance access, a teletrauma program was implemented in the Robson Valley, connecting rural clinicians during emergency cases. This study was undertaken to better understand the experiences of teletrauma users and why teletrauma is utilized. Data were gathered from 14 interviews with clinicians, health administrators, a researcher, and a health executive. Guided by interpretive description methodology, four major themes emerged, including: teletrauma affects the entire system of care; teletrauma enables a network of care built on interprofessional relationships; reasons clinicians use teletrauma are multifaceted and interrelated; and, interconnectedness of the healthcare system. Information from this study provides insight into the role and function of teletrauma in northern British Columbia and how it may better serve the needs of rural clinicians

    A training effectiveness evaluation of LeatherNet

    The author evaluates a Distributed Interactive Simulation (DIS) system known as LeatherNet to determine its abilities in fulfilling the long term Modeling and Simulation (M&S) goals of the United States Marine Corps. LeatherNet is a DIS compatible, virtual simulation system that is being developed to demonstrate the capabilities of today's simulation technology for military applications. The author developed an evaluation methodolgy that evaluates LeatherNet's ability to enhance traditional Marine Corps training. An evaluation was conducted by subject matter experts at the Marine Corps Air Ground Combat Center, 29 Palms, Ca. Analysis of results show that LeatherNet is effective in enhancing traditional live fire training methods despite its limited development. The author presents lessons learned related to this type of evaluation, and provides recommendations for further research. Additionally, the author recommends actions to be taken by the Marine Corps to optimize the benefits of LeatherNet and similar simulation products.     Turning a blind eye: the mobilization of radiology services in resource-poor regions

    While primary care, obstetrical, and surgical services have started to expand in the world's poorest regions, there is only sparse literature on the essential support systems that are required to make these operations function. Diagnostic imaging is critical to effective rural healthcare delivery, yet it has been severely neglected by the academic, public, and private sectors. Currently, a large portion of the world's population lacks access to any form of diagnostic imaging. In this paper we argue that two primary imaging modalities--diagnostic ultrasound and X-Ray--are ideal for rural healthcare services and should be scaled-up in a rapid and standardized manner. Such machines, if designed for resource-poor settings, should a) be robust in harsh environmental conditions, b) function reliably in environments with unstable electricity, c) minimize radiation dangers to staff and patients, d) be operable by non-specialist providers, and e) produce high-quality images required for accurate diagnosis. Few manufacturers are producing ultrasound and X-Ray machines that meet the specifications needed for rural healthcare delivery in resource-poor regions. A coordinated effort is required to create demand sufficient for manufacturers to produce the desired machines and to ensure that the programs operating them are safe, effective, and financially feasible

    Organizational structure of telehealth care : an examination of four types of telemedicine systems

    The purpose of this study is to make recommendations for a sustainable telemedicine system by examining the structural attributes of telemedicine across and within different types of organizations. A survey instrument with two categories, background questions and evaluation questions, was developed and used as a guide to interview eight key informants from four different types of telehealth systems. The eight transcribed interviews were coded using NUD*IST qualitative software. Research publications, archival documents, and government reports were collected to triangulate, or cross check, interview data. Multi-case study methodology was used as a guide to design the research, analyze date, compare results, and make recommendations. A telehealth typology is proposed as well as a simplified conceptual telehealth model and a diffusion of telemedicine model. While there are some structural differences among the different types of programs, there are many more similarities. All receive funding fiom multiple sources, and all employ a combination of full and part-time employees. Three core staff categories include administrative, medical, and technical support. Additional categories may include evaluation researchers and project coordinators

    Describing the use of social media as a point-of-care tool in facility-based emergency care in Africa

    Background Despite privacy and legal concerns, social media is used to provide real-time clinical support to emergency care providers. It can be particularly beneficial for those in Africa, who might lack adequate training or access to information. This PhD aimed to describe the use of social media as a point-of-care telemedicine tool in facility-based emergency care in Africa, to further inform its use. Methods A scoping review was conducted to map available literature on use, benefits, and risks associated with social media as a point-of-care platform. A mixed methods approach was then taken using a cross-sectional survey and semi-structured interviews to obtain a comprehensive description of use of social media as a point-of-care tool in facility-based emergency care in Africa. Results The scoping review identified 13 publications describing use of social media as a point-of-care tool in emergency medical settings. No studies were located in low-income countries. All studies evaluated WhatsApp use for real-time consultations, and those that assessed reliability found it to be highly reliable for consultations. A total of 70 emergency care providers in African facilities responded to the survey; nearly all worked in low- or lower-middle-income countries. Responses showed that clinicians use social media multiple times each day, primarily to share and receive advice. The majority felt social media positively impacts patient and provider experiences and improves speed and safety. Finally, eight African emergency care providers were interviewed to gain an in-depth understanding of how social media use impacts emergency care. All participants noted routine use for a range of professional purposes, including consultations, administrative tasks, and education. Concerns were mentioned by all participants, including legality, privacy, and lack of employer regulations. Conclusions This dissertation provides insight into social media use of African emergency care physicians, showing that social media use in this group is ubiquitous. Most clinicians use social media multiple times each day for a range of point-of-care purposes, and many feel social media is positively impacting both the patient and provider experiences. Post-doctoral work will focus on developing a framework to guide use of social media in facility-based emergency care in the African setting

    After detailed research work in the web for software solutions, intended for distance consultations, we have reached to the conclusion that adaptation of foreign development would be harder and maybe even unsuccessful. For the Bulgarian healthcare system, as every other, there are strict requirements for healthcare delivery and payment organization. Before starting the practical work for development of the software we have analyzed the introductory hospitals structures, working personnel, technology infrastructure – both PC periphery and specialized medical apparatuses, and etc. We have developed special Questionnaires with 69 questions for each user type, that is expected to work with the software
