    A Data-augmented 3D Morphable Model of the Ear

    Morphable models are useful shape priors for biometric recognition tasks. Here we present an iterative process of refinement for a 3D Morphable Model (3DMM) of the human ear that employs data augmentation. The process employs the following stages 1) landmark-based 3DMM fitting; 2) 3D template deformation to overcome noisy over-fitting; 3) 3D mesh editing, to improve the fit to manual 2D landmarks. These processes are wrapped in an iterative procedure that is able to bootstrap a weak, approximate model into a significantly better model. Evaluations using several performance metrics verify the improvement of our model using the proposed algorithm. We use this new 3DMM model-booting algorithm to generate a refined 3D morphable model of the human ear, and we make this new model and our augmented training dataset public

    Symmetric Shape Morphing for 3D Face and Head Modelling

    We propose a shape template morphing approach suitable for any class of shapes that exhibits approximate reflective symmetry over some plane. The human face and full head are examples. A shape morphing algorithm that constrains all morphs to be symmetric is a form of deformation regulation. This mitigates undesirable effects seen in standard morphing algorithms that are not symmetry-aware, such as tangential sliding. Our method builds on the Coherent Point Drift (CPD) algorithm and is called Symmetry-aware CPD (SA-CPD). Global symmetric deformations are obtained by removal of asymmetric shear from CPD's global affine transformations. Symmetrised local deformations are then used to improve the symmetric template fit. These symmetric deformations are followed by Laplace-Beltrami regularized projection which allows the shape template to fit to any asymmetries in the raw shape data. The pipeline facilitates construction of statistical models that are readily factored into symmetrical and asymmetrical components. Evaluations demonstrate that SA-CPD mitigates tangential sliding problem in CPD and outperforms other competing shape morphing methods, in some cases substantially. 3D morphable models are constructed from over 1200 full head scans, and we evaluate the constructed models in terms of age and gender classification. The best performance, in the context of SVM classification, is achieved using the proposed SA-CPD deformation algorithm

    Using Contours as Boundary Conditions for Elastic Registration during Minimally Invasive Hepatic Surgery

    International audienceWe address in this paper the ill-posed problem of initial alignment of pre-operative to intra-operative data for augmented reality during minimally invasive hepatic surgery. This problem consists of finding the rigid transformation that relates the scanning reference and the endoscopic camera pose, and the non-rigid transformation undergone by the liver w.r.t its scanned state. Most of the state-of-the-art methods assume a known initial registration. Here, we propose a method that permits to recover the deformation undergone by the liver while simultaneously finding the rotational and translational parts of the transformation. Our formulation considers the boundaries of the liver with its surrounding tissues as hard constraints directly encoded in an energy minimization process. We performed experiments on real in-vivo data of human hepatic surgery and synthetic data, and compared our method with related works

    Registration of ultrasound and computed tomography for guidance of laparoscopic liver surgery

    Laparoscopic Ultrasound (LUS) imaging is a standard tool used for image-guidance during laparoscopic liver resection, as it provides real-time information on the internal structure of the liver. However, LUS probes are di cult to handle and their resulting images hard to interpret. Additionally, some anatomical targets such as tumours are not always visible, making the LUS guidance less e ective. To solve this problem, registration between the LUS images and a pre-operative Computed Tomography (CT) scan using information from blood vessels has been previously proposed. By merging these two modalities, the relative position between the LUS images and the anatomy of CT is obtained and both can be used to guide the surgeon. The problem of LUS to CT registration is specially challenging, as besides being a multi-modal registration, the eld of view of LUS is signi cantly smaller than that of CT. Therefore, this problem becomes poorly constrained and typically an accurate initialisation is needed. Also, the liver is highly deformed during laparoscopy, complicating the problem further. So far, the methods presented in the literature are not clinically feasible as they depend on manually set correspondences between both images. In this thesis, a solution for this registration problem that may be more transferable to the clinic is proposed. Firstly, traditional registration approaches comprised of manual initialisation and optimisation of a cost function are studied. Secondly, it is demonstrated that a globally optimal registration without a manual initialisation is possible. Finally, a new globally optimal solution that does not require commonly used tracking technologies is proposed and validated. The resulting approach provides clinical value as it does not require manual interaction in the operating room or tracking devices. Furthermore, the proposed method could potentially be applied to other image-guidance problems that require registration between ultrasound and a pre-operative scan

    Registration of magnetic resonance and ultrasound images for guiding prostate cancer interventions

    Prostate cancer is a major international health problem with a large and rising incidence in many parts of the world. Transrectal ultrasound (TRUS) imaging is used routinely to guide surgical procedures, such as needle biopsy and a number of minimally-invasive therapies, but its limited ability to visualise prostate cancer is widely recognised. Magnetic resonance (MR) imaging techniques, on the other hand, have recently been developed that can provide clinically useful diagnostic information. Registration (or alignment) of MR and TRUS images during TRUS-guided surgical interventions potentially provides a cost-effective approach to augment TRUS images with clinically useful, MR-derived information (for example, tumour location, shape and size). This thesis describes a deformable image registration framework that enables automatic and/or semi-automatic alignment of MR and 3D TRUS images of the prostate gland. The method combines two technical developments in the field: First, a method for constructing patient-specific statistical shape models of prostate motion/deformation, based on learning from finite element simulations of gland motion using geometric data from a preoperative MR image, is proposed. Second, a novel “model-to-image” registration framework is developed to register this statistical shape model automatically to an intraoperative TRUS image. This registration approach is implemented using a novel model-to-image vector alignment (MIVA) algorithm, which maximises the likelihood of a particular instance of a statistical shape model given a voxel-intensity-based feature vector that represents an estimate of the surface normal vectors at the boundary of the organ in question. Using real patient data, the MR-TRUS registration accuracy of the new algorithm is validated using intra-prostatic anatomical landmarks. A rigorous and extensive validation analysis is also provided for assessing the image registration experiments. The final target registration error after performing 100 MR–TRUS registrations for each patient have a median of 2.40 mm, meaning that over 93% registrations may successfully hit the target representing a clinically significant lesion. The implemented registration algorithms took less than 30 seconds and 2 minutes for manually defined point- and normal vector features, respectively. The thesis concludes with a summary of potential applications and future research directions

    Technologies for Biomechanically-Informed Image Guidance of Laparoscopic Liver Surgery

    Laparoscopic surgery for liver resection has a number medical advantages over open surgery, but also comes with inherent technical challenges. The surgeon only has a very limited field of view through the imaging modalities routinely employed intra-operatively, laparoscopic video and ultrasound, and the pneumoperitoneum required to create the operating space and gaining access to the organ can significantly deform and displace the liver from its pre-operative configuration. This can make relating what is visible intra-operatively to the pre-operative plan and inferring the location of sub-surface anatomy a very challenging task. Image guidance systems can help overcome these challenges by updating the pre-operative plan to the situation in theatre and visualising it in relation to the position of surgical instruments. In this thesis, I present a series of contributions to a biomechanically-informed image-guidance system made during my PhD. The most recent one is work on a pipeline for the estimation of the post-insufflation configuration of the liver by means of an algorithm that uses a database of segmented training images of patient abdomens where the post-insufflation configuration of the liver is known. The pipeline comprises an algorithm for inter and intra-subject registration of liver meshes by means of non-rigid spectral point-correspondence finding. My other contributions are more fundamental and less application specific, and are all contained and made available to the public in the NiftySim open-source finite element modelling package. Two of my contributions to NiftySim are of particular interest with regards to image guidance of laparoscopic liver surgery: 1) a novel general purpose contact modelling algorithm that can be used to simulate contact interactions between, e.g., the liver and surrounding anatomy; 2) membrane and shell elements that can be used to, e.g., simulate the Glisson capsule that has been shown to significantly influence the organ’s measured stiffness

    Automatic analysis of medical images for change detection in prostate cancer

    Prostate cancer is the most common cancer and second most common cause of cancer death in men in the UK. However, the patient risk from the cancer can vary considerably, and the widespread use of prostate-specific antigen (PSA) screening has led to over-diagnosis and over-treatment of low-grade tumours. It is therefore important to be able to differentiate high-grade prostate cancer from the slowly- growing, low-grade cancer. Many of these men with low-grade cancer are placed on active surveillance (AS), which involves constant monitoring and intervention for risk reclassification, relying increasingly on magnetic resonance imaging (MRI) to detect disease progression, in addition to TRUS-guided biopsies which are the routine clinical standard method to use. This results in a need for new tools to process these images. For this purpose, it is important to have a good TRUS-MR registration so corresponding anatomy can be located accurately between the two. Automatic segmentation of the prostate gland on both modalities reduces some of the challenges of the registration, such as patient motion, tissue deformation, and the time of the procedure. This thesis focuses on the use of deep learning methods, specifically convolutional neural networks (CNNs), for prostate cancer management. Chapters 4 and 5 investigated the use of CNNs for both TRUS and MRI prostate gland segmentation, and reported high segmentation accuracies for both, Dice Score Coefficients (DSC) of 0.89 for TRUS segmentations and DSCs between 0.84-0.89 for MRI prostate gland segmentation using a range of networks. Chapter 5 also investigated the impact of these segmentation scores on more clinically relevant measures, such as MRI-TRUS registration errors and volume measures, showing that a statistically significant difference in DSCs did not lead to a statistically significant difference in the clinical measures using these segmentations. The potential of these algorithms in commercial and clinical systems are summarised and the use of the MRI prostate gland segmentation in the application of radiological prostate cancer progression prediction for AS patients are investigated and discussed in Chapter 8, which shows statistically significant improvements in accuracy when using spatial priors in the form of prostate segmentations (0.63 ± 0.16 vs. 0.82 ± 0.18 when comparing whole prostate MRI vs. only prostate gland region, respectively)
