    CAPAbility: Comparison of the JOURNEY II Bi-Cruciate Stabilised and GENESIS II total knee arthroplasty in performance and functional ability: protocol of a randomised controlled trial

    Background: Osteoarthritis of the knee is a common condition that is expected to rise in the next two decades leading to an associated increase in total knee replacement (TKR) surgery. Although there is little debate regarding the safety and efficacy of modern TKR, up to 20% of patients report poor functional outcomes following surgery. This study will investigate the functional outcome of two TKRs; the JOURNEY II Bi-Cruciate Stabilised knee arthroplasty, a newer knee prosthesis designed to provide guided motion and improve knee kinematics by more closely approximating a normal knee, and the GENESIS II, a proven existing design. Aim: To compare the change in Patient-reported Outcome Measures (PROMs) scores of the JOURNEY II BCS and the GENESIS II from pre-operation to 6 months post operation. Methods: CAPAbility is a pragmatic, blinded, two-arm parallel, randomised controlled trial recruiting patients with primary osteoarthritis due to have unilateral TKR surgery across two UK hospitals. Eligible participants (n = 80) will be randomly allocated to receive either the JOURNEY II or the GENESIS II BCS knee prosthesis. Baseline measures will be taken prior to surgery. Patients will be followed at 1 week, 6 to 8 weeks and 6 months post-operatively. The primary outcome is the Oxford Knee Score (OKS) at 6 months post-operatively. Secondary outcomes include: Other PROMs, biomechanical, radiological (computerised tomography, (CT)), clinical efficacy and safety outcomes. An embedded qualitative study will also investigate patients' perspectives via interview pre and post surgery on variables known to affect the outcome of TKR surgery. A sub-sample (n = 30) will have additional in-depth interviews to explore the themes identified. The surgeons' perspectives on the operation will be investigated by a group interview after all participants have undergone surgery. Discussion: This trial will evaluate two generations of TKR using PROMS, kinematic and radiological analyses and qualitative outcomes from the patient perspective

    The obligatory passage point : abstracting the meaning in tacit knowledge

    The lived experience of individuals in the workplace results in an accumulation of dispositions to act. Such dispositions have been termed knowledge [Boisot 1998]. Further, this knowledge is considered to be tacit or explicit [Baumard 1999]. Therefore tacit knowledge is one of the precursors of new knowledge. There have been a number of hypotheses as to how such knowledge is transformed into explicit knowledge [Nonaka et al 2000], and then subsequently diffused [Boisot 1998]. Moreover it is impossible to know the magnitude of tacit knowledge that is not articulated, however insightful, original or crucial it may be. The transformation to the explicit rendition can and will act as a filter in an attempt to eliminate meaningless utterances. Therefore some tacit knowledge will be lost in this process. This is an Obligatory Passage Point (OPP) a concept normally associated with Actor Network Theory [Latour 2005]. This is where the decision is made as to ‘what counts’ as legitimate knowledge and it is irreversible. This obligatory passage point is for all tacit knowledge in a community, an organization or even a nation. The case study presented comprises a small number of individuals working in a third sector environment. Although there is agreement as to what is to be achieved, the how question remains open. Despite the common concrete experiences, the tacit appreciation of the perceived action world varies significantly. A process by which an inventory of tacit knowledge can be established, abstracted and combined to act as a base to affect dispositions and expectations is described. The path to the subsequent generation of actionable knowledge is plotted which can subsequently form the basis for an intervention. The delineation between tacit knowledge and explicit knowledge in this context is explored by the application of the obligatory passage point. Utilizing the principles of language by Karl Buller the notion of legitimation is discussed. The OPP is significant because when tacit knowledge is shared, there is a process of gaining inter-subjective agreement which legitimizes the explicit representation of the tacit knowledge. The eye can see and interpret the world, but it cannot see itself. All tacit knowledge is gained through the mind’s eye.     Indonesian higher education today is faced with serious challenges that will threaten the existence of some universities. An increasing international competition will require that universities take a progressive approach to attract enough students to ensure their survival. One way the universities must improve is in the quality of their administration management. While universities in Indonesia do not compare well internationally, steps can be taken to improve the quality. There are potential lessons to be learned from corporate experience in quality control management. Decision-making in the field of academic resource planning involves extensive analysis of many data originating from multiple systems. Academic resource planning management is concerned with management resources in order to effectively support the university’s educational framework (such as offered degrees, enrolment and retention, resources teaching, course structure and curriculum). We propose a methodology for managing and determining the proposed International class based on many criteria of academic performance in university. The approach has been implemented as a decision support system allowing evaluation of various criteria and scenarios. The system combines two different methods in decision support system: Analytical hierarchy Process (AHP) and linear weightage model, the proposed model uses the AHP pairwise comparisons and the measure scale to generate the weights for the criteria which are much better and guarantee more fairly preference of criteria. Applying the system as decision-support facility for the management has resulted in significant acceleration of planning procedures and implementation, raised the overall effectiveness with respect to the underlying methodology and ultimately enabled more efficient academic administration
