Knowledge-Intensive Processes: Characteristics, Requirements and Analysis of Contemporary Approaches
Engineering of knowledge-intensive processes (KiPs) is far from being mastered, since they are genuinely knowledge- and data-centric, and require substantial flexibility, at both design- and run-time. In this work, starting from a scientific literature analysis in the area of KiPs and from three real-world domains and application scenarios, we provide a precise characterization of KiPs. Furthermore, we devise some general requirements related to KiPs management and execution. Such requirements contribute to the definition of an evaluation framework to assess current system support for KiPs. To this end, we present a critical analysis on a number of existing process-oriented approaches by discussing their efficacy against the requirements
Monitoring and detection of agitation in dementia: towards real-time and big-data solutions
The changing demographic profile of the population has potentially challenging social, geopolitical, and financial consequences for individuals, families, the wider society, and governments globally. The demographic change will result in a rapidly growing elderly population with healthcare implications which importantly include Alzheimer type conditions (a leading cause of dementia). Dementia requires long term care to manage the negative behavioral symptoms which are primarily exhibited in terms of agitation and aggression as the condition develops. This paper considers the nature of dementia along with the issues and challenges implicit in its management. The Behavioral and Psychological Symptoms of Dementia (BPSD) are introduced with factors (precursors) to the onset of agitation and aggression. Independent living is considered, health monitoring and implementation in context-aware decision-support systems is discussed with consideration of data analytics. Implicit in health monitoring are technical and ethical constraints, we briefly consider these constraints with the ability to generalize to a range of medical conditions. We postulate that health monitoring offers exciting potential opportunities however the challenges lie in the effective realization of independent assisted living while meeting the ethical challenges, achieving this remains an open research question remains.Peer ReviewedPostprint (author's final draft

Flexible Support of Healthcare Processes
Traditionally, healthcare information systems have focused on the support of predictable and repetitive clinical processes. Even though the latter can be often prespecified in formal process models, process flexibility in terms of dynamic adaptability is indispensable to cope with exceptions and unforeseen situations. Flexibility is further required to accommodate the need for evolving healthcare processes and to properly support healthcare process variability. In addition, process-aware information systems are increasingly used to support less structured healthcare processes (i.e., patient treatment processes), which can be characterized as knowledge-intensive. Healthcare processes of this category are neither fully predictable nor repetitive and, therefore, they cannot be fully prespecified at design time. The partial unpredictability of these processes, in turn, demands a certain amount of looseness. This chapter deals with the characteristic flexibility needs of both prespecified and loosely specified healthcare processes. In addition, it presents fundamental flexibility features required to address these flexibility needs as well as to accommodate them in healthcare practice
Asynchronous Remote Medical Consultation for Ghana
Computer-mediated communication systems can be used to bridge the gap between
doctors in underserved regions with local shortages of medical expertise and
medical specialists worldwide. To this end, we describe the design of a
prototype remote consultation system intended to provide the social,
institutional and infrastructural context for sustained, self-organizing growth
of a globally-distributed Ghanaian medical community. The design is grounded in
an iterative design process that included two rounds of extended design
fieldwork throughout Ghana and draws on three key design principles (social
networks as a framework on which to build incentives within a self-organizing
network; optional and incremental integration with existing referral
mechanisms; and a weakly-connected, distributed architecture that allows for a
highly interactive, responsive system despite failures in connectivity). We
discuss initial experiences from an ongoing trial deployment in southern Ghana.Comment: 10 page
Research Opportunities and Visions for Smart and Pervasive Health
Improving the health of the nation's population and increasing the
capabilities of the US healthcare system to support diagnosis, treatment, and
prevention of disease is a critical national and societal priority. In the past
decade, tremendous advances in expanding computing capabilities--sensors, data
analytics, networks, advanced imaging, and cyber-physical systems--have, and
will continue to, enhance healthcare and health research, with resulting
improvements in health and wellness. However, the cost and complexity of
healthcare continues to rise alongside the impact of poor health on
productivity and quality of life. What is lacking are transformative
capabilities that address significant health and healthcare trends: the growing
demands and costs of chronic disease, the greater responsibility placed on
patients and informal caregivers, and the increasing complexity of health
challenges in the US, including mental health, that are deeply rooted in a
