    Advanced analyses of physiological signals and their role in Neonatal Intensive Care

    Preterm infants admitted to the neonatal intensive care unit (NICU) face an array of life-threatening diseases requiring procedures such as resuscitation and invasive monitoring, and other risks related to exposure to the hospital environment, all of which may have lifelong implications. This thesis examined a range of applications for advanced signal analyses in the NICU, from identifying of physiological patterns associated with neonatal outcomes, to evaluating the impact of certain treatments on physiological variability. Firstly, the thesis examined the potential to identify infants at risk of developing intraventricular haemorrhage, often interrelated with factors leading to preterm birth, mechanical ventilation, hypoxia and prolonged apnoeas. This thesis then characterised the cardiovascular impact of caffeine therapy which is often administered to prevent and treat apnoea of prematurity, finding greater pulse pressure variability and enhanced responsiveness of the autonomic nervous system. Cerebral autoregulation maintains cerebral blood flow despite fluctuations in arterial blood pressure and is an important consideration for preterm infants who are especially vulnerable to brain injury. Using various time and frequency domain correlation techniques, the thesis found acute changes in cerebral autoregulation of preterm infants following caffeine therapy. Nutrition in early life may also affect neurodevelopment and morbidity in later life. This thesis developed models for identifying malnutrition risk using anthropometry and near-infrared interactance features. This thesis has presented a range of ways in which advanced analyses including time series analysis, feature selection and model development can be applied to neonatal intensive care. There is a clear role for such analyses in early detection of clinical outcomes, characterising the effects of relevant treatments or pathologies and identifying infants at risk of later morbidity

    Tese de mestrado integrado, Engenharia Biomédica e Biofísica (Engenharia Clínica e Instrumentação Médica) Universidade de Lisboa, Faculdade de Ciências, 2018Todos os anos, mais de 95 mil recém-nascidos são admitidos nas Unidades de Cuidados Intensivos Neonatais (UCIN) do Reino Unido, devido principalmente a partos prematuros ou outras complicações que pudessem ter ocorrido, como é o caso da encefalopatia hipóxico-isquémica (EHI), que assume 3% de todas as admissões nas unidades referidas. EHI é o termo que define uma complicação inesperada durante o parto, que resulta em lesões neurológicas a longo prazo e até em morte neonatal, devido à privação de oxigénio e fluxo sanguíneo ao recém-nascido durante o nascimento. Estima-se que tenha uma incidência de um a seis casos por 1000 nascimentos. Nos países desenvolvidos, a hipotermia é utilizada como método preventivo-terapêutico para esta condição. No entanto, existem dois grandes obstáculos para a obtenção da neuroprotecção pretendida e totalmente benéfica, na prática clínica. Em primeiro lugar, esta técnica é eficaz se for iniciada dentro de seis horas após o parto. Visto que o estado clínico da encefalopatia neonatal evolui nos dias posteriores ao nascimento, a sua deteção precoce é um grande desafio. Tal situação pode levar a diversos erros nas UCIN, tal como indivíduos sujeitos à terapia de hipotermia desnecessariamente, ou ainda mais grave, casos em que recém-nascidos foram inicialmente considerados como saudáveis, não tendo sido submetidos à terapia referida, apresentarem sinais de EHI após seis horas de vida. A segunda questão prende-se com o facto de a neuroprotecção poder ser perdida se o bebé estiver stressado durante o tratamento. Para além disso, não existe nenhuma ferramenta válida para a avaliação da dor dos recém-nascidos submetidos a esta terapia. Os obstáculos frisados anteriormente demonstram duas necessidades ainda não correspondidas: a carência de um método não invasivo e largamente adaptável a diferentes cenários para uma correta identificação de recém-nascidos com maior probabilidade de HIE, dentro de uma margem de seis horas após o parto, mas também um método preciso de stress em tempo real, não invasivo, que possa orientar tanto pessoal médico, como pais, de modo a oferecer um tratamento mais responsável, célere e individualizado. Deste modo, a análise do ritmo cardíaco demostra um enorme potencial para ser um biomarcador de encefalopatia neonatal, bem como um medidor de stress, através da eletrocardiografia (ECG), visto que é um importante indicador de homeostase, mas também de possíveis condições que podem afetar o sistema nervoso autónomo e, consequentemente, o equilíbrio do corpo humano. É extremamente difícil a obtenção de um parâmetro fisiológico, sem a presença de artefactos, especialmente no caso de recém-nascidos admitidos nas UCIN. Tanto no caso da aquisição de ECGs, como de outros parâmetros, existe uma maior probabilidade de o sinal ser corrompido por artefactos, visto que são longas aquisições, normalmente dias, onde o bebé é submetido a diversas examinações médicas, está rodeado de equipamentos eletrónicos, entre outros. Artefactos são definidos como uma distorção do sinal, podendo ser causados por diversas fontes, fisiológicas ou não. A sua presença nos dados adquiridos influencia e dissimula as informações corretas e reais, podendo mesmo levar a diagnósticos e opções terapêuticas erradas e perigosas para o paciente. Apesar de existirem diversos algoritmos de identificação de artefactos adequados para o sinal cardíaco adulto, são poucos os que funcionam corretamente para o de recém-nascido. Para além disso, é necessário bastante tempo tanto para o staff clínico, como para os investigadores, para o processo de visualização e identificação de artefactos no eletrocardiograma manualmente. Deste modo, o projeto desenvolvido na presente dissertação propõe um novo algoritmo de identificação e marcação de artefactos no sinal cardíaco de recém-nascidos. Para tal, foi criado um modelo híbrido de um método que tem em consideração todas as relações matemáticas de batimento para batimento cardíaco, com outro que tem como objetivo a remoção de spikes no mesmo sinal. O algoritmo final para além de cumprir com o objetivo descrito acima, é também adaptável a diferentes tipos de artefactos presentes no sinal, permitindo ao utilizador, de uma forma bastante intuitiva, escolher o tipo de parâmetros e passos a aplicar, podendo ser facilmente utilizado por profissionais de diferentes áreas. Deste modo, este algoritmo é uma mais-valia quando aplicado no processamento de sinal pretendido, evitando assim uma avaliação visual demorada de todo o sinal. Para obter a melhor performance possível, durante o desenvolvimento do algoritmo foram sempre considerados os resultados de validação, tais como exatidão, sensibilidade, entre outros. Para tal, foram analisados e comparados eletrocardiogramas de 4 recém-nascidos saudáveis e 4 recém-nascidos com encefalopatia. Todos possuíam aproximadamente 5 horas de sinal cardíaco adquirido após o nascimento, com diferentes níveis de presença de artefactos. O algoritmo final, obteve uma taxa de sensibilidade de 96.2% (±2.4%) e uma taxa de exatidão de 92.6% (±3.2%). Como se pode verificar pelos valores obtidos, o algoritmo obteve percentagens altas nos vários parâmetros de classificação, o que significa uma deteção correta. A taxa de exatidão apresenta um valor mais baixo, comparativamente ao parâmetro da sensibilidade, pois em diversas situações, normalmente perto de artefactos, os batimentos normais são considerados como artefactos, pelo algoritmo. Contudo, essa taxa não é alarmante, tendo sido considerada uma taxa reduzida, pelo pessoal médico. Após o processamento do sinal cardíaco dos grupos mencionados acima, um estudo comparativo, utilizando parâmetros da variabilidade do ritmo cardíaco, foi realizado. Diferenças significativas foram encontradas entre os dois grupos, onde o saudável assumiu sempre valores maiores. SDNN e baixa frequência foram os parâmetros que traduziram uma diferença maior entre os dois grupos, com um p-value <0.01. De modo a corresponder ao segundo obstáculo referido nesta dissertação, outro objetivo desta tese foi a criação de um algoritmo que pudesse identificar e diferenciar uma situação de stress nesta faixa etária, com recurso ao ritmo cardíaco. Um estudo multidimensional foi aplicado aos diferentes métodos de entropia utilizados nesta tese (approximate entropy, sample entropy, multiscales entopy e fuzzy entropy) de modo a estudar como os diferentes métodos de entropia interagem entre si e quais são os resultados dessa relação, especialmente na distinção de estados normais e stressantes. Para tal, a utilização de clusters foi essencial. Dado que para todos os ECGs de bebés saudáveis analisados neste projeto foram registados todas as possíveis situações de stress, como é o caso de choro, examinações médicas, mudança de posição, entre outros, foram escolhidos 10 minutos do sinal do ritmo cardíaco adquirido, antes da situação, para análise. Infelizmente, associado a um evento stressante, na maioria dos casos encontra-se uma percentagem bastante alta do sinal corrompida por artefactos. No entanto, em alguns casos foi possível observar uma clara distinção de grupos de clusters, indicando que naquele período de tempo, houve uma mudança de estado. Foi também realizado um estudo intensivo de diversos métodos de entropia aplicados ao grupo de sujeitos apresentados nesta dissertação, onde foi provado que o método mais adequado a nível de diferenciação é a Fuzzy Entropy (p=0.0078). Ainda é possível sugerir alguns aspetos e apontar algumas limitações, no âmbito de poderem ser ultrapassadas no futuro. Em primeiro lugar, é necessária a aquisição de mais eletrocardiogramas, quer de recém-nascidos saudáveis, quer com encefalopatia hipóxico-isquémica, de modo a aumentar o tamanho da amostra e, deste modo diminuir os valores do desvio-padrão em todos os parâmetros calculados. Relativamente ao estudo do stress, seria interessante, com uma amostra maior, a definição de clusters, de modo a ter uma identificação precisa de situações stressantes. Para além disso, a transformação do software atualmente escrito em MATLAB para GUI (interface gráfica do utilizador), a fim de tornar mais acessível a sua utilização por profissionais de diversas áreas.In Neonatal Intensive Care Unit (NICU), the heart rate (HR) offers significant insight into the autonomic function of sick newborns, especially with hypoxic ischemic encephalopathy condition (HIE). However, the intensity of clinical care and monitoring contributes to the electrocardiogram (ECG) to be often noisy and contaminated with artefacts from various sources. These artefacts, defined as any distortion of the signal caused by diverse sources, being physiological or non-physiological features, interfere with the characterization and subsequent evaluation of the heart rate, leading to grave consequences, both in diagnostic and therapeutic decisions. Besides, its manual inspection in the ECG trace is highly time-consuming, which is not feasible in clinical monitoring, especially in NICU. In this dissertation, it is proposed an algorithm capable of automatically detect and mark artefacts in neonatal ECG data, mainly dealing with mathematical aspects of the heart rate, starting from the raw signal. Also, it is proposed an adjacent algorithm, using complexity science applied to HR data, with the objective of identifying stress scenarios. Periods of 10-minute ECG were considered from 8 newborns (4 healthy and 4 HIE) to the identification of stress situations, whereas for the artefacts removal algorithm small portions varying in time length according to the amount of noise present in the originally 5 hours long samples were utilised. In this report it is also present several comparisons utilising heart rate parameters between healthy and HIE groups. Fuzzy Entropy was considered the best method to differentiate both groups (p=0.00078). In this report, substantial differences in heart rate variability were found between healthy and HIE groups, especially in SDNN and low frequency (p<0.01), confirming results of previous literature. For the final artefact removal algorithm, it is illustrated significant differences between raw and post-processed ECG signals. This method had a Recall rate of 96.2% (±2.4%) and a Precision Rate of 92.6% (±3.2%), demonstrating high efficiency in ECG noise removal. Regarding stress measures, associated with a stressful event, in most cases there is a high percentage of the signal corrupted by artefacts. However, in some cases it was possible to see a clear distinction between groups of clusters, indicating that in that period, there was a change of state. Not all the time segments from subjects demonstrated differences in stress stages, indicating that there is still room for improvement in the method developed

    Automatic neonatal sleep stage classification:A comparative study

    Sleep is an essential feature of living beings. For neonates, it is vital for their mental and physical development. Sleep stage cycling is an important parameter to assess neonatal brain and physical development. Therefore, it is crucial to administer newborn's sleep in the neonatal intensive care unit (NICU). Currently, Polysomnography (PSG) is used as a gold standard method for classifying neonatal sleep patterns, but it is expensive and requires a lot of human involvement. Over the last two decades, multiple researchers are working on automatic sleep stage classification algorithms using electroencephalography (EEG), electrocardiography (ECG), and video. In this study, we present a comprehensive review of existing algorithms for neonatal sleep, their limitations and future recommendations. Additionally, a brief comparison of the extracted features, classification algorithms and evaluation parameters is reported in the proposed study

    Vastasyntyneelle ja imeväisikäiselle nukkuminen on elintärkeä toiminto, ja se on välttämätöntä aivoverkkojen kehitykselle. Tiedetään, että huono unenlaatu aiheuttaa pitkällä tähtäimellä muun muassa kasvun hidastumista ja käyttäytymisongelmia. Imeväisikäisillä melko yleisesti esiintyvät unihäiriöt, kuten yöheräily ja nukahtamisvaikeudet aiheuttavat merkittävää rasitusta ja huolta vanhemmille. Objektiivisen mittausmenetelmän puutteen vuoksi ei ole kuitenkaan voitu selvittää imeväisikäisen unen kehittymistä kotiolosuhteissa. Tässä tutkimuksessa tarkasteltiin puettaviin pöksyihin kiinnitetyn liikeanturin ja EKG-kangaselektrodien soveltuvuutta vastasyntyneiden ja imeväisikäisten vauvojen unenaikaisen hengityksen ja sykkeen tarkkailuun. Tutkimuksen ensimmäisessä vaiheessa päiväaikaisten uni-EEG-tutkimuksien yhteydessä verrattiin liikeanturin mittauskanavien rekisteröimiä mittauskäyriä pietsoanturilla varustettuun hengitysvyöhön. Saatujen tutkimustuloksien perusteella liikeanturin gyroskooppi osoittautui tarkimmaksi hengitystaajuutta mittaavaksi parametriksi, kun taas anturin välittämä EKG-signaali oli tulkintakelpoisin osin luotettavaa. Tutkimuksen toisessa vaiheessa vauvaperheille annettiin unipöksyt ja älypuhelimet kotiin arvioidaksemme yön yli kestävää kotikäyttöä. Tutkimustulokset viittaavat siihen, että eri unitilojen tunnistaminen hengityksen vaihtelusta olisi todennäköisesti mahdollista gyroskooppisignaalista. Vanhemmilta saadun palautteen perusteella unipöksyjä pidettiin käytännöllisinä ja helppokäyttöisinä. Tulevissa tutkimuksissa tulisi keskittyä liikeanturin validointiin kliinisesti hyväksyttyjen mittausparametrien avulla, jotta algoritmeja voisi opettaa tunnistamaan eri uni-valve rytmejä automaattisesti. Näin puettava liikeanturi voisi tarjota tietoa vauvan luonnollisen unirakenteen kehittymisestä pitkällä aikavälillä. Lisäksi anturin kliininen validointi voisi mahdollistaa imeväisikäisten kardiorespiratoristen ongelmien ja liikehäiriöiden diagnostisen lisätyökalun kehittämisen.Sleep is one of the most vital functions of newborns and infants, and it is essential for neuronal network development. Therefore, long-term sleep disturbances have been associated with growth delays and behavioral disorders. Commonly reported infant sleep disturbances, such as night awakenings and difficulties falling asleep, cause distress to parents. Yet, the development of infant sleep in the home environment has not been fully elucidated due to lack of objective measurement parameters. In the current study, we assessed the feasibility of a motion sensor, attached to wearable pants, and ECG textile electrodes to monitor sleep-related respiration and heart rate of newborns and infants. First, we compared signals recorded by the motion sensor’s measurement channels to the standard respiratory piezo effort belt’s signal during daytime EEG recordings. According to our results, the motion sensor’s gyroscope proved to measure respiratory rate most accurately, while the ECG signal transmitted by the sensor was reliable in interpretable sections. We then provided wearable garments and smartphones to families with infants to assess overnight home-use. Our results indicate that different sleep states could likely be identified based on respiration fluctuation visible in the gyroscope’s signals. Moreover, the wearable system was considered practical and easy to use by the parents. Future studies should focus on validating the sensor with clinically approved measures, in order to train the algorithms to automatically identify different sleep-wake states. By doing so, the wearable sensor could provide information on natural infant sleep structure development over long time periods. Additionally, clinical validation of the sensor may result in the development of a companion diagnostic tool for infant cardiorespiratory and movement disorders

    Accurate neonatal heart rate monitoring using a new wireless, cap mounted device

Acta Paediatrica published by John Wiley & Sons Ltd on behalf of Foundation Acta Paediatrica Aim: A device for newborn heart rate (HR) monitoring at birth that is compatible with delayed cord clamping and minimises hypothermia risk could have advantages over current approaches. We evaluated a wireless, cap mounted device (fhPPG) for monitoring neonatal HR. Methods: A total of 52 infants on the neonatal intensive care unit (NICU) and immediately following birth by elective caesarean section (ECS) were recruited. HR was monitored by electrocardiogram (ECG), pulse oximetry (PO) and the fhPPG device. Success rate, accuracy and time to output HR were compared with ECG as the gold standard. Standardised simulated data assessed the fhPPG algorithm accuracy. Results: Compared to ECG HR, the median bias (and 95% limits of agreement) for the NICU was fhPPG −0.6 (−5.6, 4.9) vs PO −0.3 (−6.3, 6.2) bpm, and ECS phase fhPPG −0.5 (−8.7, 7.7) vs PO −0.1 (−7.6, 7.1) bpm. In both settings, fhPPG and PO correlated with paired ECG HRs (both R2=0.89). The fhPPG HR algorithm during simulations demonstrated a near-linear correlation (n=1266, R2=0.99). Conclusion: Monitoring infants in the NICU and following ECS using a wireless, cap mounted device provides accurate HR measurements. This alternative approach could confer advantages compared with current methods of HR assessment and warrants further evaluation at birth

    A prospective observational study on newborn resuscitation in a high-resource setting

    Bakgrunn: Omkring åtte prosent av verdens nyfødte har behov for pustehjelp for å klare overgangen fra intra- til ekstrauterint liv. Nøyaktig forekomst er usikker og varierer antagelig mellom ulike settinger, men resuscitering av nyfødte er likevel en av de vanligste akuttbehandlinger i sykehus rundt om i verden. Internasjonale retningslinjer for nyfødtresuscitering skal sikre lik og optimal behandling av syke nyfødte. Imidlertid er kunnskapsgrunnlaget for internasjonale retningslinjer mangelfullt, og baserer seg i stor grad på prekliniske studier uten sikker forankring i den kliniske hverdagen. Pustestøtte ansees som det viktigste tiltaket, og retningslinjer presiserer at overtrykksventilering bør starte innen ett minutt fra fødsel hos barn som ikke puster selv. Lav hjertefrekvens kan indikere behov for pustestøtte, og ved eventuelle tiltak vil rask bedring i hjertefrekvens indikere at behandlingen er effektiv. Retningslinjer anbefaler derfor tidlig vurdering av barnets hjertefrekvens, og at hjertefrekvens overvåkes under resuscitering ved hjelp av pulsoksymetri (PO) eller elektrokardiografi (EKG). Likevel finnes det lite kunnskap om hva som faktisk er normal hjertefrekvens de første minuttene etter fødsel, og man vet ikke hvilken metode som mest effektivt overvåker barnets hjertefrekvens under resuscitering. T-stykke ventilator er blitt et vanlig apparat for å gi luftveisstøtte til nyfødte. Forskning på bruk av disse apparatene under resuscitering i hovedsak utprøvd på premature nyfødte. Resultater fra forskning på nyfødtresuscitering danner et viktig grunnlag for videre utvikling av evidensbaserte anbefalinger. Mål: Mål for dette prosjektet var å i) studere forekomst av, karakteristika ved, og utfall av nyfødtresuscitering på kort sikt i en høyressurs setting, ii) studere etterlevelsen av retningslinjer for nyfødtresuscitering og undersøke hvor effektivt PO og EKG er til å overvåke hjertefrekvens under resuscitering, iii) beskrive normal hjertefrekvens hos friske nyfødte etter vaginal forløsning og sen avnavling, og iv) beskrive hvilke trykk og volum som leveres ved overtrykksventilering av ikke-pustende nyfødte til termin, når man bruker en T-stykke ventilator. Metode: Denne sammenstillingen bygger på fire prospektive observasjonsstudier. Alle studiene er utført ved Stavanger Universitetssjukehus. Studie I benyttet seg av rapporteringsskjema og videofilming over 12 måneder for å registrere og analysere tiltak ved nyfødtresuscitering. Vi registrerte forekomst av overtrykksventilering, kontinuerlig positivt luftveistrykk (CPAP), intubasjon, hjertekompresjoner og intravenøs administrasjon av adrenalin. Utfall etter resuscitering ble hentet fra elektroniske pasientjournaler. I studie II brukte vi videofilmer fra resuscitering av ikke-pustende barn ≥ 34 gestasjonsuker sammen med PO og/eller EKG-signal fra pasientmonitor. Vi målte tid fra fødsel til vurdering av hjerterytme og tid til oppstart av overtrykksventilering. Videre målte vi tid til pålitelig signal fra PO og EKG. I studie III målte vi hjertefrekvens de første fem minuttene etter fødsel hos friske, vaginalforløste terminbarn med sen avnavling, ved hjelp av en nyutviklet hjertefrekvensmåler med tørrelektrode-EKG (NeoBeat). Vi brukte ‘locally estimated scatterplot smoothing’ for å beregne og tegne percentiler. I studie IV brukte vi en ventilasjonsmonitor for å måle og analysere venilasjonsparametre under overtrykksventilering av terminbarn etter fødsel med T-stykke ventilator som var innstilt etter internasjonale anbefalinger (30/5 cmH2O). Vi analyserte de første 100 innblåsingene i hver resuscitering, og delte dem inn i tidlig (1.-20. innblåsing) og sen (21.-100. innblåsing) fase. Vi brukte ‘general estimating equations’ for å analysere assosiasjoner mellom tidalvolum og topptrykk, innblåsingstid og ventilasjonsfrekvens. Resultat: I studie I inkluderte vi 4693 nyfødte. Av disse ble 291 (6.2%) behandlet med pustehjelp eller annen støtte umiddelbart etter fødsel. Antall nyfødte som ble behandlet med overtrykksventilering, CPAP, intubasjon, brystkompresjoner og intravenøs administrasjon av adrenalin var henholdsvis 170 (3.6%), 121 (2.6%), 19 (0.4%), ti (0.2%), og tre (0.1%). Median (IQR) varighet av overtrykksventilasjon var 106 (54-221) sekunder. 63% av de resusciterte nyfødte ≥34 gestasjonsuker ble igjen hos foreldre etter resusciteringen. I studie II analyserte vi resusciteringer av 104 nyfødte som ikke pustet etter fødsel. I bare 35% av tilfellene ble hjertefrekvens vurdert (ved palpasjon eller auskultasjon) og overtrykksventilering startet innen 60 sekunder etter fødsel. Tiden fra fødsel til vurdering av hjertefrekvens og oppstart av overtrykksventilering var henholdsvis 70 (47-118) og 78 (42-118) sekunder. Tiden fra fødsel til pålitelig registrering av hjertefrekvens fra PO og EKG var henholdsvis 348 (217-524) og 174 (105-227) sekunder (p<0.001). Tiden fra PO måler eller EKG elektroder ble festet på barnet og til pålitelig registrering av hjertefrekvens var henholdsvis 199 (77-352) og 16 (11-22) sekunder (p<0.001). I studie III målte vi hjertefrekvens etter fødsel hos 898 friske nyfødte terminbarn. Hjertefrekvensen økte raskt fra 123 (98-147) slag per minutt ved 5 sekunders alder til 175 (157-189) slag per minutt ved 61 sekunders alder. I studie IV analyserte vi venilasjonsparametre under resuscitering av 129 nyfødte terminbarn. Topptrykket var 30 (28-31) mbar i tidlig fase og 30 (27-31) mbar i sen fase. Tidalvolum var 4.5 (1.6-7.8) ml/kg i tidlig fase og 5.7 (2.2-9.8) ml/kg i sen fase. Innblåsingstid på mer enn 0.41 sekunder i tidlig fase og 0.50 sekunder i sen fase var assosiert med de høyeste tidalvolumene. Ventilasjonsfrekvens på mer enn 32 innblåsinger per minutt i tidlig fase og 41 innblåsinger i per minutt i sen fase var assosiert med reduserte tidalvolum. Konklusjon: Nyfødtresuscitering forekom hyppig i denne høyressurs-settingen. De fleste nyfødte responderte raskt på luftveisstøtte. Etterlevelsen av gjeldende retningslinjer var dårlig. Under resuscitering av nyfødte ble pålitelig overvåkning av hjertefrekvens etablert raskere med EKG enn med PO. Vi har presentert percentiler for normal hjertefrekvens etter fødsel hos friske vaginalforløste terminbarn etter sen avnavling. Når man ventilerte nyfødte terminbarn ved hjelp av en T-stykke ventilator ble det levert stabile topptrykk, men det var vesentlig variasjon i tidalvolum. Innblåsingstid på omtrent 0.5 sekunder og ventilasjonsfrekvens på 30-40 innblåsinger i minuttet var assosiert med det høyeste tidalvolumet.Background: An estimated eight percent of newborns globally need respiratory support at birth to make the transition from intra- to extra uterine life. Although these estimates are uncertain, and presumably vary between settings, newborn resuscitation remains one of the most commonly occurring emergencies in the hospital. Resuscitation guidelines should ensure optimal treatment of compromised newborns; however, there is a general lack of evidence to support the different treatment recommendations. Existing knowledge is in large part derived from pre-clinical studies, and the transferability to real-world resuscitations is uncertain. Guidelines highlight support of breathing as the single most important task during newborn resuscitation, and positive pressure ventilation (PPV) should be initiated within the first minute of life in apnoeic newborns. Furthermore, guidelines acknowledge the newborn heart rate as an important factor to guide resuscitative interventions, and recommend continuous heart rate monitoring during resuscitation by either pulse oximetry (PO) or electrocardiography (ECG). However, there is limited data on the normal heart rate in healthy newborns, and the optimal method for monitoring heart rate during newborn resuscitation remains unknown. The flow driven T-piece resuscitator is a widely used device for respiratory support at birth. However, research into its ventilation performance during resuscitation is limited to premature newborns. Studies on newborn resuscitations provide important feedback to support the process of evolving evidence based resuscitation guidelines. Aim: The aim of this thesis was to i) study the incidence, characteristics and short-term outcomes in newborn resuscitation in a high-resource setting, ii) study compliance with resuscitation algorithms and efficacy of PO versus ECG as heart rate monitoring during resuscitation, iii) describe the normal heart rate in vaginally delivered healthy term newborns after delayed cord clamping, and iv) describe delivered pressures and tidal volumes during positive pressure ventilation of apnoeic term newborns with a T-piece resuscitator. Method: This thesis consists of four prospective observational studies. All studies were conducted at Stavanger University Hospital in Norway. Study I used incident report forms and video recordings to register and analyse resuscitative interventions during a period of 12 months. We recorded the incidence of PPV, continuous positive airway pressure (CPAP), intubation, chest compressions and intravenous administration of adrenaline. From electronic patient records we extracted short-term outcomes after resuscitation. In study II, we combined video recordings of resuscitations with PO and ECG signals from the patient monitor, to analyse guideline compliance and efficacy of heart rate monitoring in newborns ≥34 weeks of gestation receiving PPV after birth. We recorded the time from birth to initiation of PPV and time from birth to initial heart rate assessment by palpation or stethoscope. We compared time to accurate heart rate monitoring between PO and 3-lead ECG. For study III, we used a novel dry electrode ECG heart rate meter (NeoBeat, Laerdal Medical, Stavanger, Norway) to record physiological newborn heart rate in healthy vaginally born newborns after delayed cord clamping the first five minutes after birth. Heart rate centiles were drawn using a local regression model. In study IV we combined video recordings of resuscitations and a respiratory function monitor to record and analyse ventilation parameters during PPV of apnoeic term newborns after birth, using a T-piece resuscitator at standard internationally recommended settings of 30/5 cmH2O. We analysed the first 100 inflations from each resuscitation, and divided them into an early (inflation 1-20) and a late (inflation 21-100) phase. We applied general estimating equations to analyse the association between delivered tidal volumes, and peak inflating pressure, inflation time, and inflation rate. Results: Study I included 4693 newborns. Of those, 291 (6.2%) received interventions after birth. The incidence of PPV, CPAP (only), intubation, chest compressions, and intravenous administration of adrenaline were 170 (3.6%), 121 (2.6%), 19 (0.4%), ten (0.2%), and three (0.1%), respectively. Median (IQR) duration of PPV was 106 (54-221) seconds. 63% of newborns ≥34 weeks of gestation were returned to parental care immediately after resuscitation. For study II, we analysed video- and heart rate recordings of 104 resuscitations. Initial heart rate assessment (stethoscope or palpation) and initiation of PPV were achieved within 60 seconds for 35% of the resuscitated newborns. The time from birth to initial heart rate assessment and initiation of PPV was 70 (47-118) and 78 (42-118) seconds, respectively. Time from birth to provision of a reliable heart rate signal was 348 (217-524) seconds for PO, and 174 (105-227) seconds for ECG (p<0.001). Time from sensor application to a reliable heart rate signal was 199 (77-352) seconds for PO, and 16 (11-22) seconds for ECG (p<0.001). In study III, we recorded heart rates from five seconds to five minutes in 898 healthy, vaginally delivered term newborns. Following birth, the heart rate increased rapidly from 123 (98-147) beats per minute at five seconds after birth to 175 (157-189) beats per minute at 61 seconds after birth. In study IV we analysed ventilation parameters from the resuscitation of 129 term newborns. PIP was 30 (28-31) mbar in the early phase and 30 (27-31) mbar in the late phase. Tidal volume was 4.5 (1.6-7.8) ml/kg in the early phase and 5.7 (2.2-9.8) ml/kg in the late phase. Inflation times exceeding 0.41 seconds in the early phase and 0.50 seconds in the late phase were associated with the highest delivered tidal volumes. Inflation rates exceeding 32 per minute during the early phase and 41 per minute in the late phase were associated with a decrease in tidal volumes. Conclusion: The need of resuscitative interventions after birth was frequent in this high-resource setting, and most newborns responded quickly to airway support. The adherence to guidelines was poor. ECG provided a reliable heart rate signal significantly faster than PO during newborn resuscitation. We presented normal heart rate centiles in vaginally delivered term newborns after delayed cord clamping. When ventilating apnoeic newborns at birth with a T-piece resuscitator, there was a consistent delivery of PIP, however, tidal volumes varied substantially. Inflation time of approximately 0.5 seconds and rates of approximately 30-40 per minute were associated with the highest delivered tidal volumes.Doktorgradsavhandlin

    Exploring Biosignals for Quantitative Pain Assessment in Cancer Patients: A Proof of Concept

    Perception and expression of pain in cancer patients are influenced by distress levels, tumor type and progression, and the underlying pathophysiology of pain. Relying on traditional pain assessment tools can present limitations due to the highly subjective and multifaceted nature of the symptoms. In this scenario, objective pain assessment is an open research challenge. This work introduces a framework for automatic pain assessment. The proposed method is based on a wearable biosignal platform to extract quantitative indicators of the patient pain experience, evaluated through a self-assessment report. Two preliminary case studies focused on the simultaneous acquisition of electrocardiography (ECG), electrodermal activity (EDA), and accelerometer signals are illustrated and discussed. The results demonstrate the feasibility of the approach, highlighting the potential of EDA in capturing skin conductance responses (SCR) related to pain events in chronic cancer pain. A weak correlation (R = 0.2) is found between SCR parameters and the standard deviation of the interbeat interval series (SDRR), selected as the Heart Rate Variability index. A statistically significant (p < 0.001) increase in both EDA signal and SDRR is detected in movement with respect to rest conditions (assessed by means of the accelerometer signals) in the case of motion-associated cancer pain, thus reflecting the relationship between motor dynamics, which trigger painful responses, and the subsequent activation of the autonomous nervous system. With the objective of integrating parameters obtained from biosignals to establish pain signatures within different clinical scenarios, the proposed framework proves to be a promising research approach to define pain signatures in different clinical contexts
