    Applying the trigger review method after a brief educational intervention: potential for teaching and improving safety in GP specialty training?

    <p>Background: The Trigger Review Method (TRM) is a structured approach to screening clinical records for undetected patient safety incidents (PSIs) and identifying learning and improvement opportunities. In Scotland, TRM participation can inform GP appraisal and has been included as a core component of the national primary care patient safety programme that was launched in March 2013. However, the clinical workforce needs up-skilled and the potential of TRM in GP training has yet to be tested. Current TRM training utilizes a workplace face-to-face session by a GP expert, which is not feasible. A less costly, more sustainable educational intervention is necessary to build capability at scale. We aimed to determine the feasibility and impact of TRM and a related training intervention in GP training.</p> Methods We recruited 25 west of Scotland GP trainees to attend a 2-hour TRM workshop. Trainees then applied TRM to 25 clinical records and returned findings within 4-weeks. A follow-up feedback workshop was held. <p>Results: 21/25 trainees (84%) completed the task. 520 records yielded 80 undetected PSIs (15.4%). 36/80 were judged potentially preventable (45%) with 35/80 classified as causing moderate to severe harm (44%). Trainees described a range of potential learning and improvement plans. Training was positively received and appeared to be successful given these findings. TRM was valued as a safety improvement tool by most participants.</p> <p>Conclusion: This small study provides further evidence of TRM utility and how to teach it pragmatically. TRM is of potential value in GP patient safety curriculum delivery and preparing trainees for future safety improvement expectations.</p&gt

    2011 Strategic roadmap for Australian research infrastructure

    The 2011 Roadmap articulates the priority research infrastructure areas of a national scale (capability areas) to further develop Australia’s research capacity and improve innovation and research outcomes over the next five to ten years. The capability areas have been identified through considered analysis of input provided by stakeholders, in conjunction with specialist advice from Expert Working Groups   It is intended the Strategic Framework will provide a high-level policy framework, which will include principles to guide the development of policy advice and the design of programs related to the funding of research infrastructure by the Australian Government. Roadmapping has been identified in the Strategic Framework Discussion Paper as the most appropriate prioritisation mechanism for national, collaborative research infrastructure. The strategic identification of Capability areas through a consultative roadmapping process was also validated in the report of the 2010 NCRIS Evaluation. The 2011 Roadmap is primarily concerned with medium to large-scale research infrastructure. However, any landmark infrastructure (typically involving an investment in excess of $100 million over five years from the Australian Government) requirements identified in this process will be noted. NRIC has also developed a ‘Process to identify and prioritise Australian Government landmark research infrastructure investments’ which is currently under consideration by the government as part of broader deliberations relating to research infrastructure. NRIC will have strategic oversight of the development of the 2011 Roadmap as part of its overall policy view of research infrastructure

    Assessment of Essential Drug List in Lahore Retail Pharmacies

    The World Health Organization made an essential drug list for pharmacies which help in promoting the patient health by satisfying the needs of majority of population, and these medicines are available at affordable prices for patients. For the assessment of essential dug list in different areas of Lahore retail pharmacies. To check that these pharmacies follow essential drug list or not and other factors regarding essential drug list. The study was done by taking 60 pharmacies and it is survey based simple sampling in nature using both qualitative and quantitative data from retail pharmacies. After collecting data, data entry is done by using SPSS software. Data is analyzed, report is prepared based on data results from SPSS .Then discussion which include suggestions, improvements, recommendations, limitations and conclusion includes comparison. The study was based on the assessment of essential drugs prescribing indicators, patient affordability, and availability of essential drug list in pharmacies as stipulated by World Health Organization. A total of 60 retail Lahore pharmacies were sampled. The study established that a high percentage of Lahore retail pharmacies having essential drug list and most of the pharmacies follow both list which WHO EDL and NEDL. Mostly prescriptions received contain essential drugs on daily bases because most of the drugs prescribed from the essential drug list. The study also established that most of the essential drugs were properly stored and adequately labeled. In general, the prescribing and dispensing practices of essential drugs in the pharmacies are fairly good and are not far from the standard WHO requirements. The study showed that the pharmacies were doing well on essential drug list. It also assess that number of essential drugs prescribed, most commonly essential drugs in received prescription, essential drugs prescribed by generic names; drugs actually dispensed; correct patient knowledge of dosage, pharmacist and pharmacy technician awareness about essential drugs availability of essential drugs stock; drugs prescribed on EDL and availability of prescribers, drugs available at affordable prices, adequately essential drugs stored, dispensed and properly labeled.     OntoPharma: ontology based clinical decision support system to reduce medication prescribing errors

    Background: Clinical decision support systems (CDSS) have been shown to reduce medication errors. However, they are underused because of different challenges. One approach to improve CDSS is to use ontologies instead of relational databases. The primary aim was to design and develop OntoPharma, an ontology based CDSS to reduce medication prescribing errors. Secondary aim was to implement OntoPharma in a hospital setting. Methods: A four-step process was proposed. (1) Defining the ontology domain. The ontology scope was the medication domain. An advisory board selected four use cases: maximum dosage alert, drug-drug interaction checker, renal failure adjustment, and drug allergy checker. (2) Implementing the ontology in a formal representation. The implementation was conducted by Medical Informatics specialists and Clinical Pharmacists using Protégé-OWL. (3) Developing an ontology-driven alert module. Computerised Physician Order Entry (CPOE) integration was performed through a REST API. SPARQL was used to query ontologies. (4) Implementing OntoPharma in a hospital setting. Alerts generated between July 2020/ November 2021 were analysed. Results: The three ontologies developed included 34,938 classes, 16,672 individuals and 82 properties. The domains addressed by ontologies were identification data of medicinal products, appropriateness drug data, and local concepts from CPOE. When a medication prescribing error is identified an alert is shown. OntoPharma generated 823 alerts in 1046 patients. 401 (48.7%) of them were accepted. Conclusions: OntoPharma is an ontology based CDSS implemented in clinical practice which generates alerts when a prescribing medication error is identified. To gain user acceptance OntoPharma has been designed and developed by a multidisciplinary team. Compared to CDSS based on relational databases, OntoPharma represents medication knowledge in a more intuitive, extensible and maintainable manner
