    A review of lean and agile management in humanitarian supply chains: analysing the pre-disaster and post-disaster phases and future directions

    Disasters have quadrupled over the last two decades leading to unprecedented loss of life. The objective of disaster-focussed humanitarian supply chains (HSCs) is to ensure saving maximum lives with limited resources; despite severe uncertainties. Therefore, significant research has investigated lean and agile in HSCs; to effectively source and speedily deploy resources, with minimum wastage; in each disaster life-cycle phase. However, the literature and research findings are currently highly disjointed regarding how lean and agile principles may be aligned with different HSC activities in the disaster management lifecycle; and do not provide a collective understanding for practitioners and researchers. This paper reviews and organises the literature on HSCs in relation to lean and agile paradigms, focussing on the pre-disaster (mitigation and preparedness) and post-disaster (response and recovery) phases. Findings reveal, all phases benefit from both lean and agile, with agile benefitting the response phase most. The phases are inter-dependent and identifying optimum decoupling points for lean and agile principles are crucial. Majority research has focussed on individual or a couple of phases. Therefore, authors recommend research on integrating the functions of the different phases by employing lean and agile principles, to generate rapid response, economies of scale and cost minimisation

Subsequently, the hospital system should become far more competitive with supply chains having to react to these changes as patient demand becomes less predictable. Using a qualitative case study methodology, hospital managers are interviewed on a range of issues. Views on the development of the hospital supply chain in different phases are derived, and are used to develop a map of the current hospital chain. The findings show hospital managers anticipating some significant changes to the hospital supply chain and its workings as Patient Choice expands. The research also maps the various aspects of the hospital supply chain as it moves through different operational phases and highlights underlying challenges and complexities. The hospital supply chain, as discussed and mapped in this research, is original work given there are no examples in the literature that provide holistic representations of hospital activity. At the end, specific recommendations are provided that will be of interest to service to managers, researchers, and policymakers

    Dynamic Analysis of Healthcare Service Delivery: Application of Lean and Agile Concepts

    Hospitals are looking to industry for proven tools to manage increasingly complex operations and reduce costs simultaneously with improving quality of care. Currently, €˜lean€™ is the preferred system redesign paradigm, which focuses on removing process waste and variation. However, the high level of complexity and uncertainty inherent to healthcare make it incredibly challenging to remove variability and achieve the stable process rates necessary for lean redesign efforts to be effective. This research explores the use of an alternative redesign paradigm €“ €˜agile€™ €“ which was developed in manufacturing to optimize product delivery in volatile demand environments with highly variable customer requirements. €˜Agile€™ redesign focuses on increasing system responsiveness to customers through improved resource coordination and flexibility. System dynamics simulation and empirical case study are used to explore the impact of following an agile redesign approach in healthcare on service access, care quality, and cost; determine the comparative effectiveness of individual agile redesign strategies; and identify opportunities where lean methods can contribute to the creation of responsive, agile enterprises by analyzing hybrid lean-agile approaches. This dissertation contributes to the emerging literature on applying supply chain management concepts in healthcare, and opens a new path for designing healthcare systems that provide the right care, at the right time, to the right patient, at the lowest price

    Modelling a Humanitarian Supply Chain using the Structured Analysis and Design Technique (SADT)

    Conflicts since the end of the Cold War have seen military and civilian assets deployed side by side as part of an overall UN mandated solution. In order to visually present the integrated nature of humanitarian supply chains ‘Structured Analysis and Design Technique’ (SADT) has been used. SADT was chosen as it provides a robust structured method to model hierarchical systems and for this research it provides an opportunity to define and analyse the coordination and co-operation in terms of the humanitarian supply chain process, humanitarian supply chain activities and the actors involved. This research demonstrates that the visualisation facility that SADT provides not only helps in understanding the interrelationships between the actors and stakeholders involved in a humanitarian supply chain but also to some extent explains how a more effective co-ordination of humanitarian operations by military and civilian organisations involved in a complex emergency can be achieved

    Practitioners\u27 learning about healthcare supply chain management in the COVID-19 pandemic: a public procurement perspective

    Purpose The procurement and supply of crucial healthcare products in the early stages of the COVID-19 emergency were chaotic. To prepare for future crises, we must be able to describe what went wrong, and why, and map out ways to build agility and resilience. How can this be done effectively, given the highly complex and diverse network of actors across governments, care providers and supply chains, and the extreme uncertainty and dynamism in the procurement system and supplier markets? The purpose of this study was to capture learning from practitioners in real time in a way that could frame and inform capacity building across healthcare systems with varying procurement and supply management maturity. Design/methodology/approach This exploratory study involved interviews with 58 senior public procurement practitioners in central and regional governments, NGOs and leaders of professional organizations from 23 countries, very early in the COVID crisis. Following the first, inductive phase of analysis leading to five descriptive dimensions, the awareness-motivation-capability (A-M-C) framework was applied in a further round of coding, to understand immediate challenges faced by procurement practitioners, how the complex, multi-level procurement system that shaped their motivations to respond and critical capabilities required to face these challenges. Findings Developments across 23 countries and practitioners\u27 learning about procurement and supply in the pandemic crisis can be captured in five overarching themes: governance and organization, knowledge and skills, information systems, regulation and supply base issues. Together these themes cover the strengths and gaps in procurement and supply capability encountered by procurement leaders and front-line personnel. They highlight the various facets of structure, resource and process which constitute organizational capability. However, to account better for the highly dynamic situation characterized by both unprecedented rivalry and cooperation, analysts must also pay attention to actors\u27 emerging awareness of the situation and their rapidly changing motivations. Originality/value The application of the A-M-C framework is unique in the healthcare supply chain and disaster management literature. It enables a comprehensive overview of healthcare procurement from a system perspective. This study shows how increasing system preparedness for future emergencies depends both on developing critical capabilities and understanding how awareness and motivation influence the effective deployment of those capabilities
