Professional Judgment in an Era of Artificial Intelligence and Machine Learning
Though artificial intelligence (AI) in healthcare and education now accomplishes diverse tasks, there are two features that tend to unite the information processing behind efforts to substitute it for professionals in these fields: reductionism and functionalism. True believers in substitutive automation tend to model work in human services by reducing the professional role to a set of behaviors initiated by some stimulus, which are intended to accomplish some predetermined goal, or maximize some measure of well-being. However, true professional judgment hinges on a way of knowing the world that is at odds with the epistemology of substitutive automation. Instead of reductionism, an encompassing holism is a hallmark of professional practice—an ability to integrate facts and values, the demands of the particular case and prerogatives of society, and the delicate balance between mission and margin. Any presently plausible vision of substituting AI for education and health-care professionals would necessitate a corrosive reductionism. The only way these sectors can progress is to maintain, at their core, autonomous professionals capable of carefully intermediating between technology and the patients it would help treat, or the students it would help learn
Information and communication technology solutions for outdoor navigation in dementia
INTRODUCTION:
Information and communication technology (ICT) is potentially mature enough to empower outdoor and social activities in dementia. However, actual ICT-based devices have limited functionality and impact, mainly limited to safety. What is an ideal operational framework to enhance this field to support outdoor and social activities?
METHODS:
Review of literature and cross-disciplinary expert discussion.
RESULTS:
A situation-aware ICT requires a flexible fine-tuning by stakeholders of system usability and complexity of function, and of user safety and autonomy. It should operate by artificial intelligence/machine learning and should reflect harmonized stakeholder values, social context, and user residual cognitive functions. ICT services should be proposed at the prodromal stage of dementia and should be carefully validated within the life space of users in terms of quality of life, social activities, and costs.
DISCUSSION:
The operational framework has the potential to produce ICT and services with high clinical impact but requires substantial investment
The Bionic Radiologist: avoiding blurry pictures and providing greater insights
Radiology images and reports have long been digitalized. However, the potential of the more than 3.6 billion radiology
examinations performed annually worldwide has largely gone unused in the effort to digitally transform health care. The Bionic
Radiologist is a concept that combines humanity and digitalization for better health care integration of radiology. At a practical
level, this concept will achieve critical goals: (1) testing decisions being made scientifically on the basis of disease probabilities and
patient preferences; (2) image analysis done consistently at any time and at any site; and (3) treatment suggestions that are closely
linked to imaging results and are seamlessly integrated with other information. The Bionic Radiologist will thus help avoiding missed
care opportunities, will provide continuous learning in the work process, and will also allow more time for radiologists’ primary
roles: interacting with patients and referring physicians. To achieve that potential, one has to cope with many implementation
barriers at both the individual and institutional levels. These include: reluctance to delegate decision making, a possible decrease in
image interpretation knowledge and the perception that patient safety and trust are at stake. To facilitate implementation of the
Bionic Radiologist the following will be helpful: uncertainty quantifications for suggestions, shared decision making, changes in
organizational culture and leadership style, maintained expertise through continuous learning systems for training, and role
development of the involved experts. With the support of the Bionic Radiologist, disparities are reduced and the delivery of care is
provided in a humane and personalized fashion

The take-up of such applications will inevitably depend on their ability to produce an acceptable level of service over congested and unreliable public networks. However, although the problem of multimedia application-level performance is closely linked to both the user perspective of the experience as well as to the service provided by the underlying network, it is rarely studied from an integrated viewpoint. To alleviate this problem, we propose an intelligent mechanism that integrates user-related requirements with the more technical characterization of quality of service, obtaining a priority order of low-level quality of service parameters, which would ensure that user-centred quality of perception is maintained at an optimum level. We show how our framework is capable of suggesting appropriately tailored transmission protocols, by incorporating user requirements in the remote delivery of e-health solutions
