    Inertial Sensing to Determine Movement Disorder Motion Present before and after Treatment

    There has been a lot of interest in recent years in using inertial sensors (accelerometers and gyroscopes) to monitor movement disorder motion and monitor the efficacy of treatment options. Two of the most prominent movement disorders, which are under evaluation in this research paper, are essential tremor (ET) and Parkinson’s disease (PD). These movement disorders are first evaluated to show that ET and PD motion often depict more (tremor) motion content in the 3–12 Hz frequency band of interest than control data and that such tremor motion can be characterized using inertial sensors. As well, coherence analysis is used to compare between pairs of many of the six degrees-of-freedom of motions under evaluation, to determine the similarity in tremor motion for the various degrees-of-freedom at different frequency bands of interest. It was quite surprising that this coherence analysis depicts that there is a statistically significant relationship using coherence analysis when differentiating between control and effectively medicated PD motion. The statistical analysis uncovers the novel finding that PD medication induced dyskinesia is depicted within coherence data from inertial signals. Dyskinesia is involuntary motion or the absence of intended motion, and it is a common side effect among medicated PD patients. The results show that inertial sensors can be used to differentiate between effectively medicated PD motion and control motion; such a differentiation can often be difficult to perform with the human eye because effectively medicated PD patients tend to not produce much tremor. As well, the finding that PD motion, when well medicated, does still differ significantly from control motion allows for researchers to quantify potential deficiencies in the use of medication. By using inertial sensors to spot such deficiencies, as outlined in this research paper, it is hoped that medications with even a larger degree of efficacy can be created in the future

    APPLICATIONS IN VIBROARTHROGRAPHY: ASSESSMENTS OF INSTABILITY IN TOTAL HIP ARTHROPLASTY, CAM-POST ENGAGEMENT IN TOTAL KNEE ARTHROPLASTY, AND VISCOSUPPLEMENTATION IN OSTEOARTHRITIC KNEES

    Measurement of joint sounds and vibrations for non-invasive orthopaedic diagnostic purposes has slowly advanced since the 1960s. Most work has been focused in the development of methods for screening of abnormal knees. To date the technique has not gained clinical traction as is it fraught with various obstacles and skepticism. This doctoral thesis is neither an argument in favor of nor against the clinical use of vibroarthrography for musculoskeletal diagnostics in humans, but rather an exploration of its potential in cases of orthopaedic interest. These areas include 1) instability in total hip arthroplasty, 2) cam-post engagement in posterior stabilized total knee arthroplasty, and 3) viscosupplementation in osteoarthritic knees. It was expected that each of these unique cases would be characterized by dynamic phenomena that could be measured in the form of surface vibrations at the skin.Methods previously presented in various vibroarthrography research were adopted, modified, and expounded upon to best suit the needs of each experiment. In a mechanical hip simulator, it was found that vibroarthrography could be effectively used to distinguish the difference between 1 mm and 2 mm of hip separation. In posterior stabilized total knee arthroplasty subjects, it was found that multiple vibroarthrographic features may be used to approximate the occurrence of cam-post engagement, and that vibrations measured at the joint surface may be correlated to cam-post engagement velocity. In osteoarthritic knees, the relationship between clinical evidence, viscosupplementation, and vibroarthrography varied on a case by case basis.To the knowledge of the author, all three of these experiments are the first of their kind.     Assessing Performance, Role Sharing, and Control Mechanisms in Human-Human Physical Interaction for Object Manipulation

    abstract: Object manipulation is a common sensorimotor task that humans perform to interact with the physical world. The first aim of this dissertation was to characterize and identify the role of feedback and feedforward mechanisms for force control in object manipulation by introducing a new feature based on force trajectories to quantify the interaction between feedback- and feedforward control. This feature was applied on two grasp contexts: grasping the object at either (1) predetermined or (2) self-selected grasp locations (“constrained” and “unconstrained”, respectively), where unconstrained grasping is thought to involve feedback-driven force corrections to a greater extent than constrained grasping. This proposition was confirmed by force feature analysis. The second aim of this dissertation was to quantify whether force control mechanisms differ between dominant and non-dominant hands. The force feature analysis demonstrated that manipulation by the dominant hand relies on feedforward control more than the non-dominant hand. The third aim was to quantify coordination mechanisms underlying physical interaction by dyads in object manipulation. The results revealed that only individuals with worse solo performance benefit from interpersonal coordination through physical couplings, whereas the better individuals do not. This work showed that naturally emerging leader-follower roles, whereby the leader in dyadic manipulation exhibits significant greater force changes than the follower. Furthermore, brain activity measured through electroencephalography (EEG) could discriminate leader and follower roles as indicated power modulation in the alpha frequency band over centro-parietal areas. Lastly, this dissertation suggested that the relation between force and motion (arm impedance) could be an important means for communicating intended movement direction between biological agents.Dissertation/ThesisDoctoral Dissertation Biomedical Engineering 201
