Human-centered Electric Prosthetic (HELP) Hand
Through a partnership with Indian non-profit Bhagwan Mahaveer Viklang Sahayata Samiti, we designed a functional, robust, and and low cost electrically powered prosthetic hand that communicates with unilateral, transradial, urban Indian amputees through a biointerface. The device uses compliant tendon actuation, a small linear servo, and a wearable garment outfitted with flex sensors to produce a device that, once placed inside a prosthetic glove, is anthropomorphic in both look and feel. The prosthesis was developed such that future groups can design for manufacturing and distribution in India
Human-machine-centered design and actuation of lower limb prosthetic systems
People with lower limb loss or congenital limb absence require a technical substitute that restores biomechanical function and body integrity. In the last decades, mechatronic prostheses emerged and especially actuated ones increased the biomechanical functionality of their users. Yet, various open issues regarding the energy efficiency of powered systems and the impact of user-experience of the prosthesis on technical design remain. As tackeling the latter aspect urgently requires the consideration of user demands, this thesis proposes a novel human-machine-centered design (HMCD) approach for lower limb prosthetics. Further, it contributes to the design and control of elastic (prosthetic) actuation.
The HMCD approach describes a framework that equally considers technical and human factors. Therefore, seven human factors influencing lower limb prosthetic design are determined, analyzed, and modeled using human survey data: Satisfaction, Feeling of Security, Body Schema Integration, Support, Socket, Mobility, and Outer Appearance. Based on the application of quality function deployment (QFD), those factors can be considered as a HMCD focus in systems engineering. As an exemplary application, a powered prosthetic knee concept is elaborated with the HMCD approach. The comparison of the HMCD focus with a purely technical one, which is determined with a control group, reveals distinct differences in the weighting of requirements. Hence, the proposed method should lead to different prosthetic designs that might improve the subjective user-experience. To support this by integrating users throughout the systems engineering process, two concepts for human-in-the-loop experiments are suggested.
As an enabling technology of powered lower limb prostheses, variable (series) elastic actuation and especially such with variable torsion stiffness (VTS) is investigated. Inverse dynamics simulations with synthetic and human trajectories as well as experiments show that the consideration of the actuator inertia is crucial: Only by including it in advanced models, the whole range of natural dynamics and antiresonance can be exploited to minimize power consumption. A corresponding control strategy adapts the actuator to achieve energy efficiency over a wide range of operational states using these models.
The exemplary design of the powered prosthetic knee with respect to the HMCD prioritization of requirements confirms the fundamental suitability of VTS for integration in prosthetic components. In this, considering actuator inertia enables the determination of an optimal stiffness for serial elastic actuation of the human knee during walking that is not found in previous studies. A first simulation considering the changed dynamics of prosthetic gait indicates the potential to reveal lower design requirements. The designed knee concept combines promising biomechanical functionality and long operating time due to elastic actuation and energy recuperation.
Beyond lower limb prosthetics, the proposed HMCD framework can be used in other applications with distinct human-machine interrelations by adjusting the human and technical factors. Likewise, the insights into variable elastic actuation design and control can be transferred to other systems demanding energy-efficient performance of cyclic tasks
How a Diverse Research Ecosystem Has Generated New Rehabilitation Technologies: Review of NIDILRR’s Rehabilitation Engineering Research Centers
Over 50 million United States citizens (1 in 6 people in the US) have a developmental, acquired, or degenerative disability. The average US citizen can expect to live 20% of his or her life with a disability. Rehabilitation technologies play a major role in improving the quality of life for people with a disability, yet widespread and highly challenging needs remain. Within the US, a major effort aimed at the creation and evaluation of rehabilitation technology has been the Rehabilitation Engineering Research Centers (RERCs) sponsored by the National Institute on Disability, Independent Living, and Rehabilitation Research. As envisioned at their conception by a panel of the National Academy of Science in 1970, these centers were intended to take a “total approach to rehabilitation”, combining medicine, engineering, and related science, to improve the quality of life of individuals with a disability. Here, we review the scope, achievements, and ongoing projects of an unbiased sample of 19 currently active or recently terminated RERCs. Specifically, for each center, we briefly explain the needs it targets, summarize key historical advances, identify emerging innovations, and consider future directions. Our assessment from this review is that the RERC program indeed involves a multidisciplinary approach, with 36 professional fields involved, although 70% of research and development staff are in engineering fields, 23% in clinical fields, and only 7% in basic science fields; significantly, 11% of the professional staff have a disability related to their research. We observe that the RERC program has substantially diversified the scope of its work since the 1970’s, addressing more types of disabilities using more technologies, and, in particular, often now focusing on information technologies. RERC work also now often views users as integrated into an interdependent society through technologies that both people with and without disabilities co-use (such as the internet, wireless communication, and architecture). In addition, RERC research has evolved to view users as able at improving outcomes through learning, exercise, and plasticity (rather than being static), which can be optimally timed. We provide examples of rehabilitation technology innovation produced by the RERCs that illustrate this increasingly diversifying scope and evolving perspective. We conclude by discussing growth opportunities and possible future directions of the RERC program
Distributed Sensing and Stimulation Systems Towards Sense of Touch Restoration in Prosthetics
Modern prostheses aim at restoring the functional and aesthetic characteristics of the lost limb. To foster prosthesis embodiment and functionality, it is necessary to restitute both volitional control and sensory feedback. Contemporary feedback interfaces presented in research use few sensors and stimulation units to feedback at most two discrete feedback variables (e.g. grasping force and aperture), whereas the human sense of touch relies on a distributed network of mechanoreceptors providing high-fidelity spatial information. To provide this type of feedback in prosthetics, it is necessary to sense tactile information from artificial skin placed on the prosthesis and transmit tactile feedback above the amputation in order to map the interaction between the prosthesis and the environment. This thesis proposes the integration of distributed sensing systems (e-skin) to acquire tactile sensation, and non-invasive multichannel electrotactile feedback and virtual reality to deliver high-bandwidth information to the user. Its core focus addresses the development and testing of close-loop sensory feedback human-machine interface, based on the latest distributed sensing and stimulation techniques for restoring the sense of touch in prosthetics. To this end, the thesis is comprised of two introductory chapters that describe the state of art in the field, the objectives and the used methodology and contributions; as well as three studies distributed over stimulation system level and sensing system level.
The first study presents the development of close-loop compensatory tracking system to evaluate the usability and effectiveness of electrotactile sensory feedback in enabling real-time close-loop control in prosthetics. It examines and compares the subject\u2019s adaptive performance and tolerance to random latencies while performing the dynamic control task (i.e. position control) and simultaneously receiving either visual feedback or electrotactile feedback for communicating the momentary tracking error. Moreover, it reported the minimum time delay needed for an abrupt impairment of users\u2019 performance. The experimental results have shown that electrotactile feedback performance is less prone to changes with longer delays. However, visual feedback drops faster than electrotactile with increased time delays. This is a good indication for the effectiveness of electrotactile feedback in enabling close- loop control in prosthetics, since some delays are inevitable.
The second study describes the development of a novel non-invasive compact multichannel interface for electrotactile feedback, containing 24 pads electrode matrix, with fully programmable stimulation unit, that investigates the ability of able-bodied human subjects to localize the electrotactile stimulus delivered through the electrode matrix. Furthermore, it designed a novel dual parameter -modulation (interleaved frequency and intensity) and compared it to conventional stimulation (same frequency for all pads). In addition and for the first time, it compared the electrotactile stimulation to mechanical stimulation. More, it exposes the integration of virtual prosthesis with the developed system in order to achieve better user experience and object manipulation through mapping the acquired real-time collected tactile data and feedback it simultaneously to the user. The experimental results demonstrated that the proposed interleaved coding substantially improved the spatial localization compared to same-frequency stimulation. Furthermore, it showed that same-frequency stimulation was equivalent to mechanical stimulation, whereas the performance with dual-parameter modulation was significantly better.
The third study presents the realization of a novel, flexible, screen- printed e-skin based on P(VDF-TrFE) piezoelectric polymers, that would cover the fingertips and the palm of the prosthetic hand (particularly the Michelangelo hand by Ottobock) and an assistive sensorized glove for stroke patients. Moreover, it developed a new validation methodology to examine the sensors behavior while being solicited. The characterization results showed compatibility between the expected (modeled) behavior of the electrical response of each sensor to measured mechanical (normal) force at the skin surface, which in turn proved the combination of both fabrication and assembly processes was successful. This paves the way to define a practical, simplified and reproducible characterization protocol for e-skin patches
In conclusion, by adopting innovative methodologies in sensing and stimulation systems, this thesis advances the overall development of close-loop sensory feedback human-machine interface used for restoration of sense of touch in prosthetics. Moreover, this research could lead to high-bandwidth high-fidelity transmission of tactile information for modern dexterous prostheses that could ameliorate the end user experience and facilitate it acceptance in the daily life
Frankenhand: An Intelligent Prosthetic
There are over 1.6 million Americans who are missing limbs; and estimated to double by 2050. There are many problems with current prosthetic devices for the upper body. According to a 2011 study, a staggering 70% of users believed they were limited with what types of activities they were able to perform; more than 60% felt that they were restricted by their prosthetic with how much work they could complete. Our project aims to rectify some of the problems encountered with current technology. The goal of our project is to increase the usability of a prosthetic device and increase the capabilities of the prostheses. Our hand and forearm will be able to adjust the forces applied to an object, and be self-aware in regards to its spatial orientation and distance from an object
Advancing the Underactuated Grasping Capabilities of Single Actuator Prosthetic Hands
The last decade has seen significant advancements in upper limb prosthetics, specifically in the myoelectric control and powered prosthetic hand fields, leading to more active and social lifestyles for the upper limb amputee community. Notwithstanding the improvements in complexity and control of myoelectric prosthetic hands, grasping still remains one of the greatest challenges in robotics. Upper-limb amputees continue to prefer more antiquated body-powered or powered hook terminal devices that are favored for their control simplicity, lightweight and low cost; however, these devices are nominally unsightly and lack in grasp variety. The varying drawbacks of both complex myoelectric and simple body-powered devices have led to low adoption rates for all upper limb prostheses by amputees, which includes 35% pediatric and 23% adult rejection for complex devices and 45% pediatric and 26% adult rejection for body-powered devices [1]. My research focuses on progressing the grasping capabilities of prosthetic hands driven by simple control and a single motor, to combine the dexterous functionality of the more complex hands with the intuitive control of the more simplistic body-powered devices with the goal of helping upper limb amputees return to more active and social lifestyles. Optimization of a prosthetic hand driven by a single actuator requires the optimization of many facets of the hand. This includes optimization of the finger kinematics, underactuated mechanisms, geometry, materials and performance when completing activities of daily living. In my dissertation, I will present chapters dedicated to improving these subsystems of single actuator prosthetic hands to better replicate human hand function from simple control. First, I will present a framework created to optimize precision grasping – which is nominally unstable in underactuated configurations – from a single actuator. I will then present several novel mechanisms that allow a single actuator to map to higher degree of freedom motion and multiple commonly used grasp types. I will then discuss how fingerpad geometry and materials can better grasp acquisition and frictional properties within the hand while also providing a method of fabricating lightweight custom prostheses. Last, I will analyze the results of several human subject testing studies to evaluate the optimized hands performance on activities of daily living and compared to other commercially available prosthesis
A Human-Robot Interaction Perspective on Assistive and Rehabilitation Robotics
Assistive and rehabilitation devices are a promising and challenging field of recent robotics research. Motivated by societal needs such as aging populations, such devices can support motor functionality and subject training. The design, control, sensing, and assessment of the devices become more sophisticated due to a human in the loop. This paper gives a human–robot interaction perspective on current issues and opportunities in the field. On the topic of control and machine learning, approaches that support but do not distract subjects are reviewed. Options to provide sensory user feedback that are currently missing from robotic devices are outlined. Parallels between device acceptance and affective computing are made. Furthermore, requirements for functional assessment protocols that relate to real-world tasks are discussed. In all topic areas, the design of human-oriented frameworks and methods is dominated by challenges related to the close interaction between the human and robotic device. This paper discusses the aforementioned aspects in order to open up new perspectives for future robotic solutions

In developing countries such as India, there is a higher rate of amputations among the population but a lack of viable, low cost solutions. Through a partnership with Indian non-profit Bhagwan Mahaveer Viklang Sahayata Samiti (BMVSS), the team designed a functional, robust, and low cost electrically powered prosthetic hand that communicates with people with unilateral, transradial amputations in urban India through a biointerface. The device uses compliant tendon actuation, small linear servos, and a wearable sleeve outfitted with electromyography (EMG) sensors to produce a device that, once placed inside a prosthetic glove, is anthropomorphic in both look and feel. The hand is capable of forming three grips through the use of a manually adjustable opposable thumb: the key, pinch, and wrap grips. The hand also provides vibrotactile user feedback upon completion of a grip. The design includes a prosthetic gel liner to provide a layer of cushion and comfort for safe use by the user. These results show that it is possible to create a low cost, electrically powered prosthetic hand for users in developing countries without sacrificing functionality. In order for this design to be truly adjustable to each user, the creation of an easily navigable graphical user interface (GUI) will have to be a future goal.

A Generalized Method for Predictive Simulation-Based Lower Limb Prosthesis Design
Lower limb prostheses are designed to replace the functions and form of the missing biological anatomy. These functions are hypothesized to improve user outcome measures which are negatively affected by receiving an amputation – such as metabolic cost of transport, preferred walking speed, and perceived discomfort during walking. However, the effect of these design functions on the targeted outcome measures is highly variable, suggesting that these relationships are not fully understood. Biomechanics simulation and modeling tools are increasingly capable of analyzing the effects of a design on the resulting user gait. In this work, prothesis-aided gait is optimized in simulation to reduce both muscle effort and peak loads on the residual limb using a generalized prosthesis model. Compared to a traditional revolute powered ankle joint model, a two degree-of freedom generalized model reduced muscle activations by 50% and peak loads by 15%. Simulated prosthesis behaviors corresponding to the optimal gait patterns were translated into a two degree-of-freedom ankle-foot prosthesis design with powered bidirectional linear translation and plantarflexion. The prototype is capable of delivering up to 171 N-m of plantarflexion torque and 499 N of translation force, with 15° dorsi-/35° plantarflexion and 10 cm translation range of motion. The mass and height of the ankle-foot are 2.29 kg and 19.5 cm, respectively. The mass of the entire system including the wearable offboard system is 8.58 kg. This platform is designed to emulate the behavior of the simulated prosthesis, as well as be configurable to emulate alternate behaviors obtained from simulations with different optimization objectives. The prototype is controlled to replicate simulated walking patterns using a high level finite state controller, mid-level stiffness controller, and low level load controller. Closed loop load control has bandwidth of 15 Hz in translation and 7.2 Hz in flexion. Load tracking during walking with a single able-bodied human subject ranges from 93 to 159 N in translation and 4.6 to 21.3 N-m in flexion. The contribution of this work is to provide a framework for predictive simulation-based prosthesis design, evidence of its practical implementation, and the experimental tools to validate future predictive simulation studies
