The rationale of e-health evaluation: The case of NHS Direct
An important area of research is that of the evaluation of e-health services. A holistic e-health evaluation framework should address the aspects that are hampering healthcare services from embracing the full potential of information and communication technologies towards successful e-health initiatives. Towards building a holistic evaluation framework for e-health services, this paper is intended to examine the rationale of e-health evaluation, as the paper argues that this aspect should be addressed first in the development of such a framework. NHS Direct which is one of the largest e-health services in the world has been chosen to discuss and validate a set of evaluation rationales and their applicability in practice
Nurses’ Perception of Discharging the Medically Complex Pediatric Patient
The purpose of this study is to query the nurses for their perceptions of the barriers and facilitators of discharging medically complex pediatric patients from a freestanding children’s hospital in central California. Using a mixed methods research design via an online survey, 90 nurses identified 3 distinct themes that act as barriers. Those barriers include: 1) knowing the plan of care, 2) time, and 3) disposition of the family. Several implications for improving the discharge process for medically complex patients and overcoming the identified barriers include strategies to improve multidisciplinary communication, implementation of a Family Learning Center, use of video interpreters when in-person interpreters are not available, and respect for discharge readiness. Recognizing and implementing the appropriate interventions based on nurses’ feedback have the potential to improve quality and patient safety
Randomised controlled trials of complex interventions and large-scale transformation of services
Complex interventions and large-scale transformations of services are necessary to meet the health-care challenges of the 21st century. However, the evaluation of these types of interventions is challenging and requires methodological development.
Innovations such as cluster randomised controlled trials, stepped-wedge designs, and non-randomised evaluations provide options to meet the needs of decision-makers. Adoption of theory and logic models can help clarify causal assumptions, and process evaluation can assist in understanding delivery in context. Issues of implementation must also be considered throughout intervention design and evaluation to ensure that results can be scaled for population benefit. Relevance requires evaluations conducted under real-world conditions, which in turn requires a pragmatic attitude to design. The increasing complexity of interventions and evaluations threatens the ability of researchers to meet the needs of decision-makers for rapid results. Improvements in efficiency are thus crucial, with electronic health records offering significant potential

A review of interventions used to improve adherence with medication in older
Background: Medication mismanagement is a continuous problem particularly in older
people. Numerous interventions have been developed and tested in an attempt to improve
adherence with medication in this client group.
Objectives: This review aimed to examine the simple to complex interventions that have
been used to assess and improve adherence with medication in older people.
Design: An extensive review of the literature was performed and 20 relevant research
papers and one report were chosen.
Findings: Research papers were evaluated according to design, RCTs were analysed
using the JADAD scoring system, systematic reviews and reviews of the literature were
reviewed using the Critical Appraisal Skills Programme and subjected to a narrative
analysis. This process assisted the development to emerging themes. Four themes were
developed: patient barriers, health professional involvement, and health related outcomes
and formation giving.
Conclusions: The salient findings of the review infer that there is no clear definition of
non–adherent behavior. Quantitative interventions fail to assess patient choice in relation
to medication taking. Pharmacist-driven interventions are resource intensive. Health care outcomes and clinical effectiveness are seldom addressed in interventions. Among the
many difficulties encountered when attempting to use interventions to promote adherence
with medication in older people is their perceptions and beliefs, the appropriateness of the
medicines prescribed as well as the impact of lifestyle patterns; smoking, alcohol and
diet. Many intervention studies are of poor quality and do not include a theoretical
framework to underpin the interventions being used. More focused research is needed to
improve understanding of the theoretical knowledge that underpins the complexities of
adherence with medication in older people. In-depth qualitative studies can be used to
develop such theory. In addition, the quality of intervention research can be improved by
the inclusion of a research framework such as the Medical Research Council model
Theorising interprofessional pedagogic evaluation: framework for evaluating the impact of interprofessional CPD on practice change
This paper outlines the development of a conceptual framework to guide the evaluation of the impact of the pedagogy employed in continuing professional development for professionals in education, health and social care. The work is developed as part of the Centre for Excellence in Teaching and Learning: Interprofessional Learning across the Public Sector (CETL: IPPS) at the University of Southampton. The paper briefly outlines the field for pedagogic research and comments on the underpinning theories that have so far been used to guide research into interprofessional learning (IPL). It maps out the development of interprofessional CPD in its specific context as part of the CETL: IPPS with its links to a local authority undergoing service reorganisation and the role of the continuing professional development (CPD) in effecting change. It then brings together a theoretical framework with the potential toexplore, explain and evaluate the essential features of the model of pedagogy used in interprofessional CPD, in which professionals from education have for the first time been included alongside those from health and social care. The framework draws upon elements of situated learning theory, Activity Theory and Dreier’s work (2002, 1999) on trajectories of participation, particularly Personal Action Potency. By combining the resulting analytic framework with an adapted version of an established evaluation model, a theoretically-driven, practicable evaluation matrix is developed. The matrix has potential use in evaluating the impact of pedagogic input on practice change. The paper models a process for developing a conceptual framework to steer pedagogic evaluation. Such a process and the resulting matrix may be of use to other researchers who are similarly developing pedagogic evaluation
