    The development and deployment of a maintenance operations safety survey

    Objective: Based on the line operations safety audit (LOSA), two studies were conducted to develop and deploy an equivalent tool for aircraft maintenance: the maintenance operations safety survey (MOSS). Background: Safety in aircraft maintenance is currently measured reactively, based on the number of audit findings, reportable events, incidents, or accidents. Proactive safety tools designed for monitoring routine operations, such as flight data monitoring and LOSA, have been developed predominantly for flight operations. Method: In Study 1, development of MOSS, 12 test peer-to-peer observations were collected to investigate the practicalities of this approach. In Study 2, deployment of MOSS, seven expert observers collected 56 peer-to-peer observations of line maintenance checks at four stations. Narrative data were coded and analyzed according to the threat and error management (TEM) framework. Results: In Study 1, a line check was identified as a suitable unit of observation. Communication and third-party data management were the key factors in gaining maintainer trust. Study 2 identified that on average, maintainers experienced 7.8 threats (operational complexities) and committed 2.5 errors per observation. The majority of threats and errors were inconsequential. Links between specific threats and errors leading to 36 undesired states were established. Conclusion: This research demonstrates that observations of routine maintenance operations are feasible. TEM-based results highlight successful management strategies that maintainers employ on a day-to-day basis. Application: MOSS is a novel approach for safety data collection and analysis. It helps practitioners understand the nature of maintenance errors, promote an informed culture, and support safety management systems in the maintenance domain

    Supporting Defect Causal Analysis in Practice with Cross-Company Data on Causes of Requirements Engineering Problems

    [Context] Defect Causal Analysis (DCA) represents an efficient practice to improve software processes. While knowledge on cause-effect relations is helpful to support DCA, collecting cause-effect data may require significant effort and time. [Goal] We propose and evaluate a new DCA approach that uses cross-company data to support the practical application of DCA. [Method] We collected cross-company data on causes of requirements engineering problems from 74 Brazilian organizations and built a Bayesian network. Our DCA approach uses the diagnostic inference of the Bayesian network to support DCA sessions. We evaluated our approach by applying a model for technology transfer to industry and conducted three consecutive evaluations: (i) in academia, (ii) with industry representatives of the Fraunhofer Project Center at UFBA, and (iii) in an industrial case study at the Brazilian National Development Bank (BNDES). [Results] We received positive feedback in all three evaluations and the cross-company data was considered helpful for determining main causes.     Preventing Incomplete/Hidden Requirements: Reflections on Survey Data from Austria and Brazil

    Many software projects fail due to problems in requirements engineering (RE). The goal of this paper is analyzing a specific and relevant RE problem in detail: incomplete/hidden requirements. We replicated a global family of RE surveys with representatives of software organizations in Austria and Brazil. We used the data to (a) characterize the criticality of the selected RE problem, and to (b) analyze the reported main causes and mitigation actions. Based on the analysis, we discuss how to prevent the problem. The survey includes 14 different organizations in Austria and 74 in Brazil, including small, medium and large sized companies, conducting both, plan-driven and agile development processes. Respondents from both countries cited the incomplete/hidden requirements problem as one of the most critical RE problems. We identified and graphically represented the main causes and documented solution options to address these causes. Further, we compiled a list of reported mitigation actions.     Does Telemedical Support of First Responders Improve Guideline Adherence in an Offshore Emergency Scenario? A Simulator-Based Prospective Study

    OBJECTIVE: To investigate, in a simulator-based prospective study, whether telemedical support improves quality of emergency first response (performance) by medical non-professionals to being non-inferior to medical professionals. SETTING: In a simulated offshore wind power plant, duos (teams) of offshore engineers and teams of paramedics conducted the primary survey of a simulated patient. PARTICIPANTS: 38 offshore engineers and 34 paramedics were recruited by the general email invitation. INTERVENTION: Teams (randomised by lot) were supported by transmission technology and a remote emergency physician in Berlin. OUTCOME MEASURES: From video recordings, performance (17 item checklist) and required time (up to 15 min) were quantified by expert rating for analysis. Differences were analysed using two-sided exact Mann-Whitney U tests for independent measures, non-inferiority was analysed using Schuirmann one-sided test. The significance level of 5 % was Holm-Bonferroni adjusted in each family of pairwise comparisons. RESULTS: Nine teams of engineers with, nine without, nine teams of paramedics with and eight without support completed the task. Two experts quantified endpoints, insights into rater dependence were gained. Supported engineers outperformed unsupported engineers (p<0.01), insufficient evidence was found for paramedics (p=0.11). Without support, paramedics outperformed engineers (p<0.01). Supported engineers' performance was non-inferior (at one item margin) to that by unsupported paramedics (p=0.03). Supported groups were slower than unsupported groups (p<0.01). CONCLUSIONS: First response to medical emergencies in offshore wind farms with substantially delayed professional care may be improved by telemedical support. Future work should test our result during additional scenarios and explore interdisciplinary and ecosystem aspects of this support. TRIAL REGISTRATION NUMBER: DRKS0001437

    Reducing Central Line-Associated Bloodstream Infections in the Intensive Care Unit by Improving Compliance with Dressing Changes and Curos Caps Use

    Background: Reducing central line-associated bloodstream Infections (CLABSI) is a topic of global importance due to associated risk of sepsis, increased length of hospital stay, high healthcare cost, and increased morbidity and mortality among critically ill patients in the intensive care unit (ICU). The mortality rate of patients who acquire CLABSI 15-25% The cost of CLABSI treatment per event is greater than US $16,500. Central cine-associated Bloodstream Infections affect the organization’s national ranking in patient quality and safety as well as reimbursement from the Centers for Medicare and Medicaid (CMS). The aim of the quality improvement project was to improve the compliance of dressing changes and Curos caps use using peer validation and surveillance checklist to reduce CLABSIs in ICU. Methods: This was an observational evidence-based project that investigated whether peer validation and and daily surveillance checklist can improve compliance with central line dressing change and Curos cap use in ICU Results:The compliance of dressing changes improved by 55.9% (44.1% to 100%) and the Curos caps use compliance improved by 75.1% during the post intervention period (18% to 98%

    Measurement and reporting of climate-smart agriculture: technical guidance for a countrycentric process

    Given the extent of climate-smart agriculture (CSA) initiatives at project, national, regional and global levels, there is increasing interest in tracking progress in implementing CSA at national level. CSA is also expected to contribute to higher-level goals (e.g., the Paris Agreement, Africa Union’s Vision 25x25, and the Sustainable Development Goals [SDGs], etc.). Measurement and reporting of climate-smart agriculture (MR of CSA) provides intelligence on necessary the status, effectiveness, efficiency and impacts of interventions, which is critical for meeting stakeholders’ diverse management and reporting needs. In this paper, we build the case for a stakeholder-driven, country-centric framework for MR of CSA, which aims to increase coordination and coherence across stakeholders’ MR activities, while also aligning national reporting with reporting on international commitments. We present practical guidance on how to develop an integrated MR framework, drawing on findings from a multi-country assessment of needs, opportunities and capacities for national MR of CSA. The content of a unified MR framework is determined by stakeholders’ activities (how they promote CSA), needs (why MR is useful to them) and current capacities to conduct periodic monitoring, evaluation and reporting (how ready are institutions, staff and finances). Our analysis found that explicit demand for integration of data systems and active engagement of stakeholders throughout the entire process are key ingredients for building a MR system that is relevant, useful and acted upon. Based on these lessons, we identify a seven-step framework for stakeholders to develop a comprehensive information system for MR of progress in implementing CSA

    Good practice characteristics of diet and physical activity interventions and policies : an umbrella review

    BACKGROUND: This umbrella review aimed at eliciting good practice characteristics of interventions and policies aiming at healthy diet, increasing physical activity, and lowering sedentary behaviors. Applying the World Health Organization's framework, we sought for 3 types of characteristics, reflecting: (1) main intervention/policy characteristics, referring to the design, targets, and participants, (2) monitoring and evaluation processes, and (3) implementation issues. This investigation was undertaken by the DEDPIAC Knowledge Hub (the Knowledge Hub on the DEterminants of DIet and Physical ACtivity), which is an action of the European Union's joint programming initiative. METHODS: A systematic review of reviews and stakeholder documents was conducted. Data from 7 databases was analyzed (99 documents met inclusion criteria). Additionally, resources of 7 major stakeholders (e.g., World Health Organization) were systematically searched (10 documents met inclusion criteria). Overall, the review yielded 74 systematic reviews, 16 position review papers, and 19 stakeholders' documents. Across characteristics, 25% were supported by ≥ 4 systematic reviews. Further, 25% characteristics were supported by ≥ 3 stakeholders' documents. If identified characteristics were included in at least 4 systematic reviews or at least 3 stakeholders' documents, these good practice characteristics were classified as relevant. RESULTS: We derived a list of 149 potential good practice characteristics, of which 53 were classified as relevant. The main characteristics of intervention/policy (n = 18) fell into 6 categories: the use of theory, participants, target behavior, content development/management, multidimensionality, practitioners/settings. Monitoring and evaluation characteristics (n = 18) were grouped into 6 categories: costs/funding, outcomes, evaluation of effects, time/effect size, reach, the evaluation of participation and generalizability, active components/underlying processes. Implementation characteristics (n = 17) were grouped into eight categories: participation processes, training for practitioners, the use/integration of existing resources, feasibility, maintenance/sustainability, implementation partnerships, implementation consistency/adaptation processes, transferability. CONCLUSIONS: The use of the proposed list of 53 good practice characteristics may foster further development of health promotion sciences, as it would allow for identification of success vectors in the domains of main characteristics of interventions/policies, their implementation, evaluation and monitoring processes
