    Learning to drag: the effects of social interactions in touch gestures learnability for older adults

    Considering the potential physical limitations of older adults, the naturalness of touch-based gestures as an interaction method is questionable. Assuming touch-based gestures are natural, they should be highly learnable and amenable to enhancement through social interactions. To investigate whether social interactions can enhance the learnability of touch gestures for older adults with low digital literacy, we conducted a study with 42 technology- naïve participants aged 64 to 82. They were paired and encouraged to play two games on an interactive tabletop with the expectation to use the drag gesture to complete the games socially. We then compared these results with a previous study of technology–naïve older adults playing the same games individually. The results of the comparisons show that dyadic interactions had some benefits for the participants in helping them to become comfortable with the drag gesture by negotiation and imitation. Further qualitative analysis suggested that playing pairs generally helped learners to comfortably explore the digital environment using the newly acquired skill

    Touchscreen cognitive tools for mild cognitive impairment and dementia used in primary care across diverse cultural and literacy populations: a systematic review

    Background: Touchscreen cognitive tools opened new promising opportunities for the early detection of cognitive impairment; however, most research studies are conducted in English-speaking populations and high-income countries, with a gap in knowledge about their use in populations with cultural, linguistic, and educational diversity.Objective: To review the touchscreen tools used in primary care settings for the cognitive assessment of mild cognitive impairment (MCI) and dementia, with a focus on populations of different cultures, languages, and literacy.Methods: This systematic review was conducted following the PRISMA guidelines. Studies were identified by searching across MEDLINE, EMBASE, EBSCO, OVID, SCOPUS, SCIELO, LILACS, and by cross-referencing. All studies that provide a first-level cognitive assessment for MCI and dementia with any touchscreen tools suitable to be used in the context of primary care were included.Results: Forty-two studies reporting on 30 tools and batteries were identified. Substantial differences among the tools emerged, in terms of theoretical framework, clinical validity, and features related to the application in clinical practice. A small proportion of the tools are available in multiple languages. Only 7 out of the 30 tools have a multiple languages validation. Only two tools are validated in low-educated samples, e.g., IDEA and mSTS-MCI.Conclusion: General practitioners can benefit from touchscreen cognitive tools. However, easy requirements of the device, low dependence on the examiner, fast administration, and adaptation to different cultures and languages are some of the main features that we need to take into consideration when implementing touchscreen cognitive tools in the culture and language of underrepresented populations

    Experiences with everyday technology and wellbeing in dementia

    This portfolio thesis consists of three parts: a systematic literature review, an empirical paper and a set of appendices. The thesis as a whole explores the use of everyday technologies in dementia and how everyday technology is experienced by individuals living with dementia and their care partners.Part one is a systematic literature review that explores how people living with dementia and their care partners experience and view using technologies to enhance wellbeing through social engagement, participation and leisure. The review additionally aimed to understand what views and experiences people with dementia and their care partners have in relation to the usability of everyday technology. Ten papers were reviewed, and the data were synthesised from a narrative approach. The findings indicate that everyday technology has the potential to have a positive impact of different areas of wellbeing for people living with dementia. The overall findings are discussed within the context of wider literature, with consideration of areas for further research and clinical implications.Part two of the portfolio is an empirical study that aimed to understand how couples, where one person is living with dementia, experience co-creating a digital life story book together. The study aimed to understand whether co-creating a digital life book supports couples’ wellbeing and, if so, how does it support their wellbeing. Four married couples (n=8) completed the 6 week study of creating their digital life story book and participated in an interview following the 6 weeks. The research used a qualitative method, gathering data using a Constructivist Grounded Theory approach. Because full data saturation was not met, the final data were analysed using Thematic Analysis. Four superordinate and eight subordinate themes emerged from the data. The findings are discussed around the wider context of literature exploring life story work and couples’ wellbeing in dementia, with the implications for clinical practice and further research discussed.Part three consists of a set of appendices relating to both the systematic literature review and the empirical paper. Included in these appendices are a reflective statement and an epistemological statement which explores the researcher’s journey throughout the research process, and the philosophical position including the underlying assumptions of the research

    The association of alcohol consumption with diet and cardiometabolic risk in two independent UK populations

    The association between alcohol consumption and cardiometabolic disease risk has been described as a J or U-shaped curve attributed to a combination of harmful and beneficial effects varying with volume of intake. Diet is an established risk factor for cardiometabolic disease and related traits However, few studies examining this association adequately control for residual confounding by dietary intake. Therefore, the aim of this thesis was to investigate the relationship between alcohol consumption and markers of cardiometabolic health independent of dietary intake. Cross sectional analyses were conducted using data from the Airwave Health Monitoring Study – a British occupational cohort (n = 9,581). Alcohol consumption behaviour was determined from questionnaire and 7-day diet record data. Diet quality was determined by measuring adherence to the Dietary Approaches to Stop Hypertension (DASH) diet using data from the 7-day diet records. Markers of cardiometabolic health included: adiposity (body mass index and waist circumference), blood pressure, cholesterol, HbA1c and C-reactive protein. All analyses were replicated in an independent UK cohort using UK Biobank data (n =146,888). As part of this thesis, genetic analyses were conducted to investigate whether genes implicated in the regulation of HDL-c may facilitate some of the cardioprotective effects attributed to alcohol intake. Heavy alcohol consumption is associated with a deterioration in cardiometabolic health. The risk of cardiometabolic disease as indicated by a cardiometabolic risk score was lowest amongst moderate drinkers and highest amongst never, and heavy drinkers. Increasing alcohol intake was associated with a deterioration in diet quality and had an additive effect of total energy intake. Diet quality did not modify the relationship between moderate alcohol intake and cardiometabolic risk. Alcohol intake did not alter the effect of genes implicated in the regulation of HDL-c. These findings were validated in a sub-sample of the UK Biobank cohort. To conclude, the findings from this thesis show that alcohol consumption plays a key role in determining diet quality and cardiometabolic risk. Specifically, risk of developing obesity.Open Acces

    Accessible Autonomy: Exploring Inclusive Autonomous Vehicle Design and Interaction for People who are Blind and Visually Impaired

    Autonomous vehicles are poised to revolutionize independent travel for millions of people experiencing transportation-limiting visual impairments worldwide. However, the current trajectory of automotive technology is rife with roadblocks to accessible interaction and inclusion for this demographic. Inaccessible (visually dependent) interfaces and lack of information access throughout the trip are surmountable, yet nevertheless critical barriers to this potentially lifechanging technology. To address these challenges, the programmatic dissertation research presented here includes ten studies, three published papers, and three submitted papers in high impact outlets that together address accessibility across the complete trip of transportation. The first paper began with a thorough review of the fully autonomous vehicle (FAV) and blind and visually impaired (BVI) literature, as well as the underlying policy landscape. Results guided prejourney ridesharing needs among BVI users, which were addressed in paper two via a survey with (n=90) transit service drivers, interviews with (n=12) BVI users, and prototype design evaluations with (n=6) users, all contributing to the Autonomous Vehicle Assistant: an award-winning and accessible ridesharing app. A subsequent study with (n=12) users, presented in paper three, focused on prejourney mapping to provide critical information access in future FAVs. Accessible in-vehicle interactions were explored in the fourth paper through a survey with (n=187) BVI users. Results prioritized nonvisual information about the trip and indicated the importance of situational awareness. This effort informed the design and evaluation of an ultrasonic haptic HMI intended to promote situational awareness with (n=14) participants (paper five), leading to a novel gestural-audio interface with (n=23) users (paper six). Strong support from users across these studies suggested positive outcomes in pursuit of actionable situational awareness and control. Cumulative results from this dissertation research program represent, to our knowledge, the single most comprehensive approach to FAV BVI accessibility to date. By considering both pre-journey and in-vehicle accessibility, results pave the way for autonomous driving experiences that enable meaningful interaction for BVI users across the complete trip of transportation. This new mode of accessible travel is predicted to transform independent travel for millions of people with visual impairment, leading to increased independence, mobility, and quality of life

    Digital support for self-management and meaningful activities of people with mild dementia:Development, implementation and feasibility of a person-centred touch-screen intervention

    This thesis reports on the research that was conducted to develop and pilot test a person-centred touchscreen-based program (FindMyApps intervention) that supports people with mild dementia and their informal carers in how to use a tablet and apps for self-management and meaningful activities. The program consists of the FindMyApps training for informal carers in supporting people with mild dementia in using a tablet and the FindMyApps selection tool designed to help people with mild dementia find apps for self-management and meaningful activities that fit their needs, wishes and abilities. The program was developed following the Medical Research Council (MRC) Framework for the design and evaluation of complex interventions and in co-creation with end users. The chapters of this thesis describe the first three phases of this framework i.e.: the preclinical or theoretical phase (0); the modelling phase (I) and the exploratory trial (II). The execution of an RCT (III), and further implementation (IV) in case the intervention proves effective, are beyond the scope of this thesis. In the first phase user needs studies were performed to identify user requirements for desired self-management and meaningful activities and to identify user-requirements related to the use of apps. Eight focus groups were performed in which people with mild cognitive impairment (MCI) or mild dementia (n=13) and informal carers (n=15) participated. We also conducted a literature review to explore which training interventions are most effective for people with mild dementia in (re)learning how to use technologies, including handheld touchscreen devices. An electronic search was conducted in the following databases: PubMed, APA PsycInfo (EBSCO) and CINAHL (EBSCO). In total 16 studies met the inclusion criteria. Our review contributed to the growing amount of promising evidence on the potential impact of Errorless learning training interventions for people with mild to moderate dementia in (re)learning how to use technology. Based on these results we developed the FindMyApps training. In the second phase of the framework we developed a first concept of the FindMyApps selection tool based on identified user requirements. To ensure its usability, the web-based tool was developed using a ‘user-participatory design’ involving the close collaboration of potential users, a development team (researchers, developers and designers) and an expert team (experts in person-centred dementia care and ICT). In three short iterative rounds – so called ‘sprints’ – the users (people with dementia (n=8) and (in)formal carers (n=10)) were invited to test whether the prototypes were in line with their needs, wishes and abilities. This generated important insights into user-interface aspects relating to (i) useful content and (ii) a user-friendly tool design. In the third phase we pilot tested the FindMyApps intervention by means of individual semi-structured interviews and a pilot randomized controlled exploratory trial. Twenty people with mild dementia and informal carer dyads were randomly assigned to the FindMyApps group (n=10), receiving either the FindMyApps training and selection tool, or a control condition (n=10), receiving only a short tablet training. Pre- and post-test measurements at a three month follow-up, consisted of questionnaires and post-test semi-structured interviews. Based on the qualitative results and the effect sizes on the outcomes measured in this study, we consider that the FindMyApps intervention has the potential to positively influence the self-management and engagement in meaningful activities in people with dementia. Future studies with a larger sample should better indicate whether this expectation can be confirmed. The intervention will be further improved and tested in a larger pilot-RCT study and its effectiveness subsequently evaluated in a definite RCT
