Patient safety in Europe: medication errors and hospital-acquired infections

The Report was commissioned by the European Federation of Nurses Associations (EFN) in November 2007 in order to support its policy statements on Patient Safety (June 2004). In that statement the EFN declares its belief that European Union health services should operate within a culture of safety that is based on working towards an open culture and the immediate reporting of mistakes; exchanging best practice and research; and lobbying for the systematic collection of information and dissemination of research findings. This Report adressess specifically the culture of highly reliable organisations using the work of James Reason (2000). Medication errors and hospital-acquired infections are examined in line with the Reprt´s parameters and a range of European studies are used as evidence. An extensive reference list is provided that allows EFN to explore work in greater detail as required

Circumventing Authority: Loopholes in the DMCA’s Access Controls

In a world where digital pirates freely roam the internet, seemingly plundering at will, the providers of digital content must find a way to protect their valuable assets. Digital fences afford that protection--but not very well. Fortunately (for content owners), 17 U.S.C. §1201, passed as part of the Digital Millennium Copyright Act of 1998, was designed to fill the numerous gaps in those fences by forbidding activities designed to circumvent them. In its present state, however, §1201 does not adequately serve that purpose. Substantial flaws in the language of the statute render it virtually powerless to thwart piracy. If §1201 is to fulfill its intended role (without the need for creative judicial interpretation), it must be amended to rectify the discrepancies between Congress’ supposed intent and the language it chose

Field study: assessing migration and mobility patterns, access to health services and vulnerabilities of female fish traders in the Kafue Flats fishery, Zambia: research design report

The WorldFish Center in Lusaka is implementing a regional collaborative programme entitled "Fisheries and HIV&AIDS in Africa; investing in sustainable solutions". The programme is funded by the Swedish International Development Cooperation Agency (Sida) and the Norwegian Ministry of Foreign Affairs. Under the Zambia component, the technical focus area "Options for reducing vulnerability along the fish marketing chain" focuses on the Kafue Flats fishery in Zambia. This study aims to identify vulnerability factors affecting fisher folk and fish traders in the Kafue Flats, especially those that make them susceptible and vulnerable to HIV&AIDS. The study will provide the analytical basis for pilot interventions to reduce specific vulnerability factors and enhance livelihoods for fisher folk and fish traders in the Kafue Flats fishery. How police investigators seek to secure that suspects speaking a second language understand their rights in investigative interviews

This study presents an empirical investigation of 54 police interviews in Norway conducted with suspects speaking Norwegian as a second language. Using Conversation Analysis as our method, we examine how investigators seek to ensure that suspects understand their rights. We find a range of practices for making rights more accessible compared to how they are formulated in legal acts. Investigators divide their turns into smaller segments and monitor the suspect for displays of understanding along the way. They simplify the syntax and substitute technical terms with plain vocabulary. Sometimes they supplement the wording of the prosecution instructions with explanations or reformulations. In a few instances, they also check understanding by asking a simple yes/no question. However, such questions only generate claims of understanding and no actual evidence of what has been understood. While there are isolated cases of requests to retell the rights, even these practices are problematic in generating strong evidence of understanding. This study discusses the importance of linguistic education and training in police forces for dealing with potential or emerging understanding problems during investigative interviews.publishedVersionPaid open acces
