A novel method of using accelerometry for upper limb FES control.
This paper reports on a novel approach to using a 3-axis accelerometer to capture body segment angle for upper limb functional electrical stimulation (FES) control. The approach calculates the angle between the accelerometer x -axis and the gravity vector, while avoiding poor sensitivity at certain angles and minimizing errors when true acceleration is relatively large in comparison to gravity. This approach was incorporated into a state-machine controller which is used for the real-time control of FES during up- per limb functional task performance. An experimental approach was used to validate the new method. Two participants with different upper limb impairments resulting from a stroke carried out four different FES-assisted tasks. Comparisons were made between angle calculated from arm-mounted accelerometer data using our algorithm and angle calculated from limb-mounted reflective marker data. After removal of coordinate misalignment error, mean error across tasks and subjects ranged between 1.4 and 2.9 °. The approach shows promise for use in the control of upper limb FES and other human movement applications where true acceleration is relatively small in comparison with gravity
Prediction of setup times for an advanced upper limb functional electrical stimulation system
Introduction: Rehabilitation devices take time to don, and longer or unpredictable setup time impacts on usage.
This paper reports on the development of a model to predict setup time for upper limb functional electrical stimulation.
Methods: Participants’ level of impairment (Fugl Meyer-Upper Extremity Scale), function (Action Research Arm Test)
and mental status (Mini Mental Scale) were measured. Setup times for each stage of the setup process and total setup
times were recorded. A predictive model of setup time was devised using upper limb impairment and task complexity.
Results: Six participants with stroke were recruited, mean age 60 (�17) years and mean time since stroke 9.8 (�9.6)
years. Mean Fugl Meyer-Upper Extremity score was 31.1 (�6), Action Research Arm Test 10.4 (�7.9) and Mini Mental
Scale 26.1 (�2.7). Linear regression analysis showed that upper limb impairment and task complexity most effectively
predicted setup time (51% as compared with 39%) (F(2,21) ÂĽ 12.782, adjusted R2 ÂĽ 0.506; p<.05).
Conclusions: A model to predict setup time based on upper limb impairment and task complexity accounted for 51% of
the variation in setup time. Further studies are required to test the model in real-world settings and to identify other
contributing factors
Neurological Tremor: Sensors, Signal Processing and Emerging Applications
Neurological tremor is the most common movement disorder, affecting more than 4% of elderly people. Tremor is a non linear and non stationary phenomenon, which is increasingly recognized. The issue of selection of sensors is central in the characterization of tremor. This paper reviews the state-of-the-art instrumentation and methods of signal processing for tremor occurring in humans. We describe the advantages and disadvantages of the most commonly used sensors, as well as the emerging wearable sensors being developed to assess tremor instantaneously. We discuss the current limitations and the future applications such as the integration of tremor sensors in BCIs (brain-computer interfaces) and the need for sensor fusion approaches for wearable solutions
Wearable inertial sensors for human movement analysis
Introduction: The present review aims to provide an overview of the most common uses of wearable inertial sensors in the field of clinical human movement analysis.Areas covered: Six main areas of application are analysed: gait analysis, stabilometry, instrumented clinical tests, upper body mobility assessment, daily-life activity monitoring and tremor assessment. Each area is analyzed both from a methodological and applicative point of view. The focus on the methodological approaches is meant to provide an idea of the computational complexity behind a variable/parameter/index of interest so that the reader is aware of the reliability of the approach. The focus on the application is meant to provide a practical guide for advising clinicians on how inertial sensors can help them in their clinical practice.Expert commentary: Less expensive and more easy to use than other systems used in human movement analysis, wearable sensors have evolved to the point that they can be considered ready for being part of routine clinical routine
The effectiveness of upper-limb wearable technology for improving activity and participation in adult stroke survivors : a systematic review
Background: With advances in technology, the adoption of wearable devices has become a viable adjunct in poststroke rehabilitation. Upper limb (UL) impairment affects up to 77% of stroke survivors impacting on their ability to carry out everyday activities. However, despite an increase in research exploring these devices for UL rehabilitation, little is known of their effectiveness.
Objective: This review aimed to assess the effectiveness of UL wearable technology for improving activity and participation in adult stroke survivors.
Methods: Randomized controlled trials (RCTs) and randomized comparable trials of UL wearable technology for poststroke rehabilitation were included. Primary outcome measures were validated measures of activity and participation as defined by the International Classification of Functioning, Disability, and Health. Databases searched were MEDLINE, Web of Science (Core collection), CINAHL, and the Cochrane Library. The Cochrane Risk of Bias Tool was used to assess the methodological quality of the RCTs and the Downs and Black Instrument for the quality of non RCTs.
Results: In the review, we included 11 studies with collectively 354 participants at baseline and 323 participants at final follow-up including control groups and participants poststroke. Participants’ stroke type and severity varied. Only 1 study found significant between-group differences for systems functioning and activity (P≤.02). The 11 included studies in this review had small sample sizes ranging from 5 to 99 participants at an average (mean) age of 57 years.
Conclusions: This review has highlighted a number of reasons for insignificant findings in this area including low sample sizes and the appropriateness of the methodology for complex interventions. However, technology has the potential to measure outcomes, provide feedback, and engage users outside of clinical sessions. This could provide a platform for motivating stroke survivors to carry out more rehabilitation in the absence of a therapist, which could maximize recovery
Closed-loop stimulation for upper limb rehabilitation following spinal cord injury and stroke
PhD ThesisInnovation is required to improve upper limb rehabilitation for neurological conditions such as
stroke and spinal cord injury (SCI). There is growing appreciation of the importance of neural
plasticity in recovery, and how this can be facilitated by synchronous activity in peripheral
neural circuits and central brain areas. However, despite increasing scientific evidence,
technological solutions that exploit associative plasticity have not yet been widely evaluated in
clinical practice.
In this thesis, I report the development and initial evaluation of a novel device which enabled
a reaching and grasping motion in the affected limb by combining assistive functional electrical
stimulation (FES) with inferred voluntary brain activity. The device was designed to enable
translation from laboratory-to-clinic by overcoming common practical barriers to translational
research, such as adaptability and ease of use.
The device was demonstrated to be usable by individuals with either chronic stroke or SCI, and
received positive qualitative feedback. Some participants showed modest improvements on
assessments of upper limb function following a short intervention period.
A study with healthy able-bodied volunteers indicated that after using the device, corticospinal
pathways to the antagonist (flexor) muscle may be facilitated, and this facilitation might be
increased by adjusting the relative timing of stimulation and inferred brain activity.
The device could also deliver alternative stimulation techniques, and an exploratory study into
transcutaneous spinal cord stimulation (tSCS) was conducted with healthy able-bodied
volunteers. It was found that tSCS may activate peripheral and spinal pathways within
acceptable comfort levels, but the parameters used in this study did not to generate functional
contractions. An unexpected oscillatory motor response provided insights into how tSCS acts
upon the motor system.
Prior to a large scale evaluation of clinical effectiveness, further research is required to: further
develop a theoretical basis for the intervention; demonstrate the mechanisms of action; and to

FES-UPP: A Flexible Functional Electrical Stimulation System to Support Upper Limb Functional Activity Practice
There is good evidence supporting highly intensive, repetitive, activity-focused, voluntary-initiated practice as a key to driving recovery of upper limb function following stroke. Functional electrical stimulation (FES) offers a potential mechanism to efficiently deliver this type of therapy, but current commercial devices are too inflexible and/or insufficiently automated, in some cases requiring engineering support. In this paper, we report a new, flexible upper limb FES system, FES-UPP, which addresses the issues above. The FES-UPP system consists of a 5-channel stimulator running a flexible FES finite state machine (FSM) controller, the associated setup software that guides therapists through the setup of FSM controllers via five setup stages, and finally the Session Manager used to guide the patient in repeated attempts at the activities(s) and provide feedback on their performance. The FSM controller represents a functional activity as a sequence of movement phases. The output for each phase implements the stimulations to one or more muscles. Progression between movement phases is governed by user-defined rules. As part of a clinical investigation of the system, nine therapists used the FES-UPP system to set up FES-supported activities with twenty two patient participants with impaired upper-limbs. Therapists with little or no FES experience and without any programming skills could use the system in their usual clinical settings, without engineering support. Different functional activities, tailored to suit the upper limb impairment levels of each participant were used, in up to 8 sessions of FES-supported therapy per participant. The efficiency of delivery of the therapy using FES-UPP was promising when compared with published data on traditional face-face therapy. The FES-UPP system described in this paper has been shown to allow therapists with little or no FES experience and without any programming skills to set up state-machine FES controllers bespoke to the patient’s impairment patterns and activity requirements, without engineering support. The clinical results demonstrated that the system can be used to efficiently deliver high intensity, activity-focused therapy. Nevertheless, further work to reduce setup time is still required

New Technologies for On-Demand Hand Rehabilitation in the Living Environment after Neurologic Injury
High-dosage rehabilitation therapy enhances neuroplasticity and motor recovery after neurologic injuries such as stroke and spinal cord injury. The optimal exercise dosage necessary to promote upper extremity (UE) recovery is unknown. However, occupational and physical therapy sessions are currently orders of magnitude too low to optimally drive recovery. Taking therapy outside of the clinic and into the living environment using sensing and computer technologies is attractive because it could result in a more cost efficient and effective way to extend therapy dosage. This dissertation developed innovative wearable sensing algorithms and a novel robotic system to enhance hand rehabilitation. We used these technologies to provide on-demand exercise in the living environment in ways not previously achieved, as well as to gain new insights into UE use and recovery after neurologic injuries.Currently, the standard-of-practice for wearable sensing of UE movement after stroke is bimanual wrist accelerometry. While this approach has been validated as a way to monitor amount of UE activity, and has been shown to be correlated with clinical assessments, it is unclear what new information can be obtained with it. We developed two new kinematic metrics of movement quality obtainable from bimanual wrist accelerometry. Using data from stroke survivors, we applied principal component analysis to show that these metrics encode unique information compared to that typically carried by conventional clinical assessments. We presented these results in a new graphical format that facilitates the identification of limb use asymmetries.Wrist accelerometry has the limitation that it cannot isolate functional use of the hand. Previously, we had developed a sensing system, the Manumeter, that quantifies finger movement by sensing magnetic field changes induced by movement of a ring worn on the finger, using a magnetometer array worn at the wrist. We developed, optimized, and validated a calibration-free algorithm, the “HAND” algorithm, for real-time counting of isolated, functional hand movements with the Manumeter. Using data from a robotic wrist simulator, unimpaired volunteers and stroke survivors, we showed that HAND counted movements with ~85% accuracy, missing mainly smaller, slower movements. We also showed that HAND counts correlated strongly with clinical assessments of hand function, indicating validity across a range of hand impairment levels.To date, there have been few attempts to increase hand use and recovery of individuals with a stroke by providing real-time feedback from wearable sensors. We used HAND and the Manumeter to perform a first-of-its-kind randomized controlled trial of the effect of real-time hand movement feedback on hand use and recovery after chronic stroke. We found that real-time feedback on hand movement was ineffective in increasing hand use intensity and improving hand function. We also showed for the first time the non-linear relationship between hand capacity, measured in the laboratory, and actual hand use, measured at-home. Even people with a moderate level of clinical hand function exhibit very low hand use at home. Finally, the challenge of improving hand function for people with moderate to severe injuries highlights the need for novel approaches to rehabilitation. One emerging technique is regenerative rehabilitation, in which regenerative therapies, such as stem cell engraftment, are coupled with intensive rehabilitation. In collaboration with the Department of Veteran Affairs Gordon Mansfield Spinal Cord Injury Translational Collaborative Consortium, we developed a robot for promoting on-demand, hand rehabilitation in a non-human primate model of hemiparetic spinal cord injury that is being used to synergize hand rehabilitation with novel regenerative therapies. Using an innovative bimanual manipulation paradigm, we show that subjects engaged with the device at a similar rate before and after injury across a range of hand impairment severity. We also demonstrate that we could shape relative use of the arm and increase the number of exercise repetitions per reward by changing parameters of the robot. We then evaluated how the peak grip force that the subjects applied to the robot decreased after SCI, demonstrating that it can serve as a potential marker of recovery.These developments provide a foundation for future work in technologies for therapeutic movement rehabilitation in the living environment by establishing: 1) new metrics of upper extremity movement quality; 2) a validated algorithm for achieving a “pedometer for the hand” using wearable magnetometry; 3) a negative clinical trial result on the therapeutic effect of real-time hand feedback after stroke, which begs the question of what can be improved in future trials; 4) the nonlinear relationship between hand movement ability and at-home use, supporting the concept of learned non-use; and 5) the first example of robotic regenerative rehabilitation
Advances and perspectives of mechanomyography
INTRODUCTION: The evaluation of muscular tissue condition can be accomplished with mechanomyography (MMG), a technique that registers intramuscular mechanical waves produced during a fiber's contraction and stretching that are sensed or interfaced on the skin surface.
OBJECTIVE: Considering the scope of MMG measurements and recent advances involving the technique, the goal of this paper is to discuss mechanomyography updates and discuss its applications and potential future applications.
METHODS: Forty-three MMG studies were published between the years of 1987 and 2013.
RESULTS: MMG sensors are developed with different technologies such as condenser microphones, accelerometers, laser-based instruments, etc. Experimental protocols that are described in scientific publications typically investigated the condition of the vastus lateralis muscle and used sensors built with accelerometers, third and fourth order Butterworth filters, 5-100Hz frequency bandpass, signal analysis using Root Mean Square (RMS) (temporal), Median Frequency (MDF) and Mean Power Frequency (MPF) (spectral) features, with epochs of 1 s.
CONCLUSION: Mechanomyographic responses obtained in isometric contractions differ from those observed during dynamic contractions in both passive and functional electrical stimulation evoked movements. In the near future, MMG features applied to biofeedback closed-loop systems will help people with disabilities, such as spinal cord injury or limb amputation because they may improve both neural and myoelectric prosthetic control. Muscular tissue assessment is a new application area enabled by MMG; it can be useful in evaluating the muscular tonus in anesthetic blockade or in pathologies such as myotonic dystrophy, chronic obstructive pulmonary disease, and disorders including dysphagia, myalgia and spastic hypertonia.
