The value of hydrogen sulphide as marker of severity and prognosis of in acute pancreatitis
AIM OF THE STUDY: The present study aimed to evaluate the role of H2S as marker of severity and prognosis of in acute pancreatitis.
MATERIALS AND METHODS: All patients of acute pancreatitis admitted between June 2012
and september 2013 were included. Atlanta criteria was used to define the severity of pancreatitis. Plasma H2S was measured (spectrophotometry) at admission, at 48 hours and at discharge. A sample size of 50 was calculated including both mild and severe cases. Results was expressed as mean and median with appropriate measures of dispersion. Data does not satisfy the assumption of normality, so a non-parametric approach will be used. To calculate paired differences Wilcoxon signed rank test will be used and Mann-Whitney u-test will be used to compare groups.
RESULTS: 55 patients (male: 43; age: 41.6, 18 -80 years; mean, range) were included. Alcohol (25) and biliary (15) were common etiology of pancreatitis. 23(41.8%) had severe pancreatitis; 35% had acute lung injury, 9 % had acute kidney injury and 40.7 % developed sepsis. Admission H2S levels was lower in mild pancreatitis as compare to the patients with severe pancreatitis, but difference was not statistically significant (16.6, 7.14-81.43 v/s 35.7, 6.07-74.43 渭mol/L, median, range, p value 0.3). H2S levels at 48 hours after admission were also not significantly different between mild and severe cases. Patients who presented within 4 days of onset of pain had higher values of H2S at admission (n=42, median 27.67 Vs n=13, 15 渭mol/l, p-value 0.032) compared to those who presented late. There was no significant difference between H2S levels in patients who developed acute lung injury, kidney injury, sepsis and those who did not. Plasma H2S level at admission in the two patients who expired of multiorgan failure was also not significantly different from the patients who survived (median 28.6 v/s 20.7渭mol/L). The values of H2S were not correlated with established biomarkers of severity such as Creactive protein.
CONCLUSION: Plasma H2S levels at admission and at 48 hours did not predict the severity of acute pancreatitis and their levels did not correlate with the systemic complications or mortality

Sistem Informasi Reservasi Ruangan Rawat Inap Berbasis Web

Abstract: The hospital is an integral part of the entire health care system developed through Indonesia's Health development plan. In organizing requires an accurate and reliable information system and adequate enough to improve the service to the patient as well as other related environment. At this time, the development of technology that continues to increase demands us to continue to improve the service so that every process of the server can run well and effectively. If we can take advantage of this, it can be useful to resolve the problem in the hospital one of the problems that often occur is when the patient wants hospitalization, inaccuracies recording of the staff about the room availability information So that the staff should contact each existing inpatient room to find out the available or absence of an empty inpatient room so that the room booking process will require a long process. Therefore, one solution to solve the problem is to provide an application information system reservation room based inpatient website, with the application provides the ease of hospital personnel in Make a room reservation process for patients who will be hospitalized. So the administration process can be well documented and structured.

Viktoria: A new Paradigm for Hash Functions
Viktoria hash is a compression function that generates a set of 512 bits from an arbitrary size input (limit of 2^480-1 bytes). This hash function contains some internal routines clearly inspired by AES and RC4 symmetric algorithms [14]. The new paradigm presents two major innovations: a fast preprocessing that initiates an internal state of 256!^2 permutations and a post-processing that guarantees a minimum number of executed rounds of 2^13. The pre-processing allows to differentiate very similar messages in the first runs of the algorithm. In the post-processing we have a safety barrier provided by a large number of rounds through a different structure of the main processing. The Viktoria algorithm seems to inaugurate a new design model in the construction of robust hash functions for some reasons, among them we highlight: the customization of the internal state according to each message, the elegance and efficiency of its main function and also a supposed high margin of safety provided by its post-processing function. Viktoria hash can also process bit oriented messages (whose last byte size is not complete) and generate larger hashes (1024, 1536, 2048 or larger) always as multiples of 512
