Indicators and instruments to assess components of disability in community-dwelling older adults: a systematic review
The epidemiological demands of aging point to the need for characterizing older adults regarding health and disability. This systematic review aims to summarize the indicators (instruments) identifying different components of disability as a result of aging exposition in community-dwelling older adults, considering the International Classification of Functioning, Disability, and Health framework. Taking the PRISMA 2020 recommendations as a reference, studies with community-dwelling older adults, reporting the development and/or age disability modifications were included. Two reviewers analyzed the observational studies searched in the MEDLINE, CINAHL, Web of Science, Scopus, and Embase databases. Of the 137 potentially eligible studies, 49 were included in this review. Several indicators (instruments) demonstrated older adults’ disabilities according to the different domains of the ICF. Objective measures assessed Body Structures, Body Functions, and Environmental Factors and included handgrip strength (dynamometry, n = 8), cognitive function (Mini-Mental State examination, n = 7), gait speed (walk test, n = 6), and endurance (Chair stand-test, n = 4). Self-reported measures assessed Activities and Participation, but not the Body Structures, and included the basic and instrumental activities of daily living (ADL) (the Katz Index of ADL, n = 4 studies, the Lawton and Brody Instrumental ADL, n = 4 studies). The summary of the measures gathered can guide researchers and health professionals to select indicators (instruments) to assess and monitor older adults’ disabilities resulting from aging exposition, to support the development of new wearables, and to provide improvements to the existing ones, allowing the tailored assessment of different health and disability dimensions.info:eu-repo/semantics/publishedVersio
Life-cycle analysis of last-mile parcel delivery using autonomous delivery robots
The acceleration of global e-commerce brings an increasing environmental burden to urban last-mile logistics. Autonomous delivery robots (ADRs) have often been considered as an attractive solution to this challenge but, to date, their environmental impact had not been fully assessed. To fill this gap, a life-cycle analysis of two-echelon and business-as-usual distribution strategies is proposed in this paper. To model ADR production, primary data from an actual prototype is used. The mathematical formulation of the use stage is done using the continuous approximation methodology. Finally, some managerial insights are obtained. Two-echelon operations would generate between 60 and 130 gCO2-eq per parcel delivery depending on the considered operation scenario. The ADR fleet production and renewal are the biggest contributors to this total global warming potential (GWP). As a consequence, the three main leverages to decrease the GWP of an ADR-based two-echelon delivery scheme are an improvement of the ADR production processes, the maximization of the robot lifespan (both for mechanical parts and battery), and the optimization of delivery operations to minimize the robot fleet size.The first author would like to personally acknowledge CARNET for the funding of this research article, developed in the framework of his PhD thesis. Sustainability Assessment and Engineering of Emerging Aircraft Technologies: Challenges, Methods and Tools
Driven by concerns regarding the sustainability of aviation and the continued growth of air traffic, increasing interest is given to emerging aircraft technologies. Although new technologies, such as battery-electric propulsion systems, have the potential to minimise in-flight emissions and noise, environmental burdens are possibly shifted to other stages of the aircraft’s life cycle, and new socio-economic challenges may arise. Therefore, a life-cycle-oriented sustainability assessment is required to identify these hotspots and problem shifts and to derive recommendations for action for aircraft development at an early stage. This paper proposes a framework for the modelling and assessment of future aircraft technologies and provides an overview of the challenges and available methods and tools in this field. A structured search and screening process is used to determine which aspects of the proposed framework are already addressed in the scientific literature and in which areas research is still needed. For this purpose, a total of 66 related articles are identified and systematically analysed. Firstly, an overview of statistics of papers dealing with life-cycle-oriented analysis of conventional and emerging aircraft propulsion systems is given, classifying them according to the technologies considered, the sustainability dimensions and indicators investigated, and the assessment methods applied. Secondly, a detailed analysis of the articles is conducted to derive answers to the defined research questions. It illustrates that the assessment of environmental aspects of alternative fuels is a dominating research theme, while novel approaches that integrate socio-economic aspects and broaden the scope to battery-powered, fuel-cell-based, or hybrid-electric aircraft are emerging. It also provides insights by what extent future aviation technologies can contribute to more sustainable and energy-efficient aviation. The findings underline the need to harmonise existing methods into an integrated modelling and assessment approach that considers the specifics of upcoming technological developments in aviation
Characterization and genomic analysis of a new phage infecting Helicobacter pylori
Helicobacter pylori, a significant human gastric pathogen, has been demonstrating increased antibiotic resistance, causing difficulties in infection treatment. It is therefore important to develop alternatives or complementary approaches to antibiotics to tackle H. pylori infections, and (bacterio)phages have proven to be effective antibacterial agents. In this work, prophage isolation was attempted using H. pylori strains and UV radiation. One phage was isolated and further characterized to assess potential phage-inspired therapeutic alternatives to H. pylori infections. HPy1R is a new podovirus prophage with a genome length of 31,162 bp, 37.1% GC, encoding 36 predicted proteins, of which 17 were identified as structural. Phage particles remained stable at 37 °C, from pH 3 to 11, for 24 h in standard assays. Moreover, when submitted to an in vitro gastric digestion model, only a small decrease was observed in the gastric phase, suggesting that it is adapted to the gastric tract environment. Together with its other characteristics, its capability to suppress H. pylori population levels for up to 24 h post-infection at multiplicities of infection of 0.01, 0.1, and 1 suggests that this newly isolated phage is a potential candidate for phage therapy in the absence of strictly lytic phages.This study was supported by the Portuguese Foundation for Science and Technology (FCT), under the scope of the strategic funding of the UIDB/04469/2020 unit, and Project Helicophage PTDC/SAU-PUB/29182/2017 (POCI-01-0145-FEDER-029182). Green batteries for clean skies: Sustainability assessment of lithium‐sulfur all‐solid‐state batteries for electric aircraft
The use of novel battery technologies in short-haul electric aircraft can support the aviation sector in achieving its goals for a sustainable development. However, the production of the batteries is often associated with adverse environmental and socio-economic impacts, potentially leading to burden shifting. Therefore, this paper investigates alternative technologies for lithium–sulfur all-solid-state batteries (LiS-ASSBs) in terms of their contribution to the sustainable development goals (SDGs). We propose a new approach that builds on life cycle sustainability assessment and links the relevant impact categories to the related SDGs. The approach is applied to analyze four LiS-ASSB configurations with different solid electrolytes, designed for maximum specific energy using an electrochemical model. They are compared to a lithium–sulfur battery with a liquid electrolyte as a benchmark. The results of our cradle-to-gate analysis reveal that the new LiS-ASSB technologies generally have a positive contribution to SDG achievement. However, the battery configuration with the best technical characteristics is not the most promising in terms of SDG achievement. Especially variations from the technically optimal cathode thickness can improve the SDG contribution. A sensitivity analysis shows that the results are rather robust against the weighting factors within the SDG quantification method
H. pylori phages: from genome release to hope for use as therapy
The increasing antibiotic-resistant Helicobacter pylori infections worldwide and the ineffectiveness of treatments led the World Health Organization to designate clarithromycin-resistant H. pylori as a high-priority bacterium for antibiotic research and development. (Bacterio)phages, viruses that infect bacteria, showing effectiveness in the treatment of pathogenic bacteria, could be a promising alternative strategy in the fight against H. pylori infections.
Material and methods
In this work, a collection of 74 Portuguese H. pylori-clinical strains was used to screen for the presence of phage genes, using a new PCR-based method. Selected strains were subsequently sequenced and prophage isolation was attempted using UV radiation. Three phages were isolated, one of which was further characterized genetically and biologically.
Results
PCR-based detection indicated the presence of target phage sequences in 14 strains, and the induction strategies resulted in the release of a new phage. It presents a genome length of 31,162 bp with a G+C content of 37.1 %. This podovirus showed capability to form phage plaques in five strains, was stable under an in vitro gastric digestion model, and was able to maintain a H. pylori population at low levels for up to 24h post-infection.
Conclusion
The new PCR screening method proved to be very effective in the selection of strains carrying prophages, resulting in the isolation of a new H. pylori phage. This phage presented very promising characteristics in terms of stability and efficacy, being therefore a small step towards the future use of phage therapy in the fight against H. pylori infections.info:eu-repo/semantics/publishedVersio

Predictors of cardiac involvement in idiopathic inflammatory myopathies

This work aimed to find predictors of cardiac involvement in IIM.
Methods: Multicenter, open cohort study, including patients registered in the IIM module of the Rheumatic Diseases Portuguese Register (Reuma.pt/Myositis) until January 2022. Patients without cardiac involvement information were excluded. Myo(peri)carditis, dilated cardiomyopathy, conduction abnormalities, and/or premature coronary artery disease were considered.
Results: 230 patients were included, 163 (70.9%) of whom were females. Thirteen patients (5.7%) had cardiac involvement. Compared with IIM patients without cardiac involvement, these patients had a lower bilateral manual muscle testing score (MMT) at the peak of muscle weakness [108.0 ± 55.0 vs 147.5 ± 22.0, p=0.008] and more frequently had oesophageal [6/12 (50.0%) vs 33/207 (15.9%), p=0.009] and lung [10/13 (76.9%) vs 68/216 (31.5%), p=0.001] involvements. Anti-SRP antibodies were more commonly identified in patients with cardiac involvement [3/11 (27.3%) vs 9/174 (5.2%), p=0.026]. In the multivariate analysis, positivity for anti-SRP antibodies (OR 104.3, 95% CI: 2.5-4277.8, p=0.014) was a predictor of cardiac involvement, regardless of sex, ethnicity, age at diagnosis, and lung involvement. Sensitivity analysis confirmed these results.
Conclusion: Anti-SRP antibodies were predictors of cardiac involvement in our cohort of IIM patients, irrespective of demographical characteristics and lung involvement. We suggest considering frequent screening for heart involvement in anti-SRP-positive IIM patients.info:eu-repo/semantics/publishedVersio
Trauma Associado às Touradas à Corda nos Açores: Um Estudo Transversal
Introdução: O objetivo deste estudo foi caracterizar as lesões traumáticas tauromáquicas ocorridas nas touradas à corda nos Açores no que diz respeito à causa do incidente, mecanismo de trauma, área anatómica mais afetada e gravidade das lesões.
Métodos: Estudo unicêntrico, transversal, com a colheita prospetiva de dados realizada durante dois anos. Foram incluídos os doentes que consecutivamente recorreram ao serviço de urgência do hospital local por lesões traumáticas ocorridas por trauma direto com o animal ou quedas aquando da fuga ou manuseio da corda. Foram colhidos dados demográficos gerais, características da lesão, tratamentos efetuados, necessidade de internamento hospitalar e mortalidade. Foi realizada uma análise estatística descritiva com recurso ao software estatístico SPSS.
Resultados: Registaram-se 56 admissões hospitalares e 80 lesões traumáticas. A principal causa de traumatismo foi o trauma direto com o animal (37; 66,07%) e o mecanismo de lesão foi o trauma fechado (56; 100%). As áreas anatómicas mais afetadas foram a cabeça e pescoço (27; 33,75%). A mediana de Injury Severity Score foi de 4 à admissão hospitalar. Cinco doentes (8,92%) apresentaram trauma major. Dez doentes (17,85%) necessitaram de internamento hospitalar com uma mediana de dias de internamento de sete (IIQ 4,5 dias). Três (30%) dos doentes internados necessitaram de internamento em unidade de cuidados intensivos. Seis doentes (10,71%) foram submetidos a cirurgia.
Conclusão: A principal causa de traumatismo foi o trauma direto com o animal e o mecanismo de lesão foi o trauma fechado. As áreas anatómicas mais afetadas foram a cabeça e pescoço. Estes dados constituem um alerta para o impacto destes eventos no que diz respeito aos custos económicos que acarretam, aos custos para a saúde da população local, às medidas de segurança atualmente implementadas e à disponibilidade dos meios necessários para tratar estes doentes
