FOREIGN AID, GROWTH, POLICY AND REFORM
Whether good macroeconomic policy facilitates aid effectiveness in generating growth is a highly debated topic. In this paper we investigate if economic reform has a favorable effect on the aid-growth relation—specifically, if reform enhances the effect of aid on growth. In doing so, we also construct a new policy index and examine the robustness of the Burnside and Dollar (2000) ‘policy view'. The results indicate that although our new policy index and reform are both growth enhancing, they do not increase aid-effectiveness and aid seem to play no positive role in the picture.
La divulgation d'informations sur l'immatériel : Une étude empirique dans le contexte tunisien
Dans le cadre de ce travail de recherche exploratoire, nous étudions l'approche pratique du capital immatériel, à travers une enquête par questionnaire, pour voir l'étendue de la reconnaissance de ce concept dans notre contexte tunisien et notamment pour dégager l'avis des dirigeants d'entreprises sur les moyens d'identification et les règles de comptabilisation et de divulgation du capital immatériel. Les résultats de cette enquête montrent que le degré de connaissance du concept du capital immatériel est jugé satisfaisant. Expression of TIM3/VISTA checkpoints and the CD68 macrophage-associated marker correlates with anti-PD1/PDL1 resistance: implications of immunogram heterogeneity.
Although immunotherapies have achieved remarkable salutary effects among subgroups of advanced cancers, most patients do not respond. We comprehensively evaluated biomarkers associated with the "cancer-immunity cycle" in the pan-cancer setting in order to understand the immune landscape of metastatic malignancies as well as anti-PD-1/PD-L1 inhibitor resistance mechanisms. Interrogation of 51 markers of the cancer-immunity cycle was performed in 101 patients with diverse malignancies using a clinical-grade RNA sequencing assay. Overall, the immune phenotypes demonstrated overexpression of multiple checkpoints including VISTA (15.8% of 101 patients), PD-L2 (10.9%), TIM3 (9.9%), LAG3 (8.9%), PD-L1 (6.9%) and CTLA4 (3.0%). Additionally, aberrant expression of macrophage-associated markers (e.g. CD68 and CSF1R; 11-23%), metabolic immune escape markers (e.g. ADORA2A and IDO1; 9-16%) and T-cell priming markers (e.g. CD40, GITR, ICOS and OX40; 4-31%) were observed. Most tumors (87.1%, 88/101) expressed distinct immune portfolios, with a median of six theoretically actionable biomarkers (pharmacologically tractable by Food and Drug Administration approved agents [on- or off-label] or with agents in clinical development). Overexpression of TIM-3, VISTA and CD68 were significantly associated with shorter progression-free survival (PFS) after anti-PD-1/PD-L1-based therapies (among 39 treated patients) (all P < .01). In conclusion, cancer-immunity cycle biomarker evaluation was feasible in diverse solid tumors. High expression of alternative checkpoints TIM-3 and VISTA and of the macrophage-associated markers CD68 were associated with significantly worse PFS after anti-PD-1/PD-L1-based therapies. Most patients had distinct and complex immune expression profiles suggesting the need for customized combinations of immunotherapy
Factors Influencing the Incidence of Severe Complications in Head and Neck Free Flap Reconstructions.
Complications after head and neck free-flap reconstructions are detrimental and prolong hospital stay. In an effort to identify related variables in a tertiary regional head and neck unit, the microvascular reconstruction activity over the last 5 years was captured in a database along with patient-, provider-, and volume-outcome-related parameters.
Retrospective cohort study (level of evidence 3), a modified Clavien-Dindo classification, was used to assess severe complications.
A database of 217 patients was created with consecutively reconstructed patients from 2009 to 2014. In the univariate analysis of severe complications, we found significant associations (P < 0.05) between type of flap used, American Society of Anesthesiologists classification, T-stage, microscope use, surgeon, flap frequency, and surgeon volume. Within a binomial logistic regression model, less frequently versus frequently performed flap (odds ratio [OR] = 3.2; confidence interval [CI] = 2.9-3.5; P = 0.000), high-volume versus low-volume surgeon (OR = 0.52; CI = -0.22 to 0.82; P = 0.007), and ASA classification (OR = 2.9; CI = 2.4-3.4; P = 0.033) were retained as independent predictors of severe complications. In a Cox-regression model, surgeon (P = 0.011), site of reconstruction (P = 0.000), T-stage (P = 0.001), and presence of severe complications (P = 0.015) correlated with a prolonged hospitalization.
In this study, we identified a correlation of patient-related factors with severe complications (ASA score) and prolonged hospital stay (T-stage, site). More importantly, we identified several provider- (surgeon) and volume-related (frequency with which a flap was performed and high-volume surgeon) factors as predictors of severe complications. Our data indicate that provider- and volume-related parameters play an important role in the outcome of microvascular free-flap procedures in the head and neck region
Quality of life with cediranib in relapsed ovarian cancer: The ICON6 phase 3 randomized clinical trial
BACKGROUND: The ICON6 trial showed that cediranib, an oral inhibitor of vascular endothelial growth factor receptors 1, 2, and 3, improved clinical outcomes for patients with platinum-sensitive relapsed ovarian cancer when it was used with chemotherapy and was continued as maintenance therapy. This study describes health-related quality of life (QOL) during the first year of treatment. METHODS: Four hundred fifty-six women were randomly allocated to receive standard chemotherapy only, chemotherapy with concurrent cediranib, or chemotherapy with cediranib administered concurrently and continued as maintenance. Patients completed QOL questionnaires until disease progression every 3 weeks during chemotherapy and then every 6 weeks to 1 year. Patients alive with disease progression completed a QOL form 1 year after randomization. The primary QOL endpoint was the global score from the Quality of Life Questionnaire Core 30 (of the European Organization for Research and Treatment of Cancer) at 1 year, with the standard chemotherapy group compared with the concurrent-maintenance cediranib group.
RESULTS: The rate of questionnaire compliance was 90% at the baseline and 76% at 1 year and was similar across the 3 groups. The mean global QOL score at 1 year was 62.6 points for the standard chemotherapy group and 68.7 points for the concurrent-maintenance group (+4.5; 95% confidence interval, –2.0 to 11.0; P = .18). Sensitivity analyses suggested that this finding was robust to the effect of missing data, and the improvement became statistically significant after adjustments for self-reported diarrhea. CONCLUSIONS: The 6th study by the International Collaboration in Ovarian Neoplasm (ICON6) showed a significant improvement in progression-free survival with cediranib as concurrent and maintenance therapy. No QOL detriment with cediranib was found 1 year after treatment was commenced. The maintenance of QOL along with prolonged cancer control suggests that cediranib has a valuable role in the treatment of relapsed ovarian cancer

The growing demand of construction aggregates has raised concern about the availability of natural aggregates. Over two billion tons of natural aggregate are produced each year in the United States and that number is expected to increase to 2.5 billion tons by 2020. This has raised concern about the availability of natural aggregate. Discarding demolished concrete into landfills is a costly solution from an economical and environmental point of view. Many U.S. highway agencies are re-using Recycled Concrete Aggregates (RCA) as construction material. The use of fiber reinforcement in Portland Cement Concrete (PCC) has recently become a popular option in concrete construction because of its influence on preventing segregation, reducing early shrinkage cracks and increasing residual load capacity. Alkali-Silica Reaction (ASR) is a major problem in concrete, especially when using RCA, causing concrete expansion and cracks. Recently lithium has been found to reduce expansion due to ASR. This thesis will investigate the effect, of fibers soaked in lithium nitrate on the mechanical properties of RCA
