A scoping review of outcome measures for people living with dementia and family supporters to evaluate Recovery College dementia courses
IntroductionRecovery Colleges (RC/RCs) aim to promote personal recovery through co-produced courses, grounded in the CHIME (Connectedness, Hope, Identity, Meaning, Empowerment) framework. The DiSCOVERY research programme noted that RC dementia courses may offer a person-centred approach to post-diagnostic dementia care. However, the lack of validated outcome measures for this context presents a challenge in evaluating RCs’ effectiveness. This scoping review examines the potential outcome measures for evaluating the impact of RC dementia courses.MethodsThe review followed the Arksey and O’Malley framework, searching for eligible papers across six databases related to dementia and the CHIME strengths-based approach. Instruments relating to personal recovery and positive psychology for people with dementia or their family supporters were included. Measures of cognition, clinical symptoms, or ‘negative constructs’ (e.g., burden) were excluded. DiSCOVERY stakeholder groups (people with dementia and clinicians) met to collaboratively identify meaningful domains and relevant measures.ResultsFourteen instruments relating to hope, resilience, self-efficacy, empowerment, and coping were identified. Stakeholders of people living with dementia endorsed domains of empowerment, resilience, and hope. No single instrument captured the range of outcomes that underlie the concepts of the RC dementia course.DiscussionThis study contributes to the limited literature on instruments for the evaluation of concepts underlying RC dementia courses. Findings suggest a need for adaptation and further validation of existing measures, to address responsiveness, interpretability, and the inclusion of domains related to recovery. Future research on recovery in the context of dementia should involve developing or adapting new measures, conducting feasibility studies, and exploring cultural sensitivity for diverse populations.</p

BackgroundDelivering care to growing numbers of patients with increasingly ‘complex’ needs is currently compromised by a system designed to treat patients within organizational clinical specialties, making this difficult to reconfigure to fit care to needs. Problematic experiences of people with cognitive impairment(s) admitted to hospitals with a hip fracture, exemplify the complex challenges that result if their care is not tailored. This study explored whether a flexible, multicomponent intervention, adapting services to the needs of this patient group, could be implemented in acute hospital settings.MethodsWe used action research with case study design to introduce the intervention using a Plan-Do-Study-Act (PDSA) model to three different hospital sites (cases) across England. The qualitative data for this paper was researcher-generated (notes from observations and teleconference meetings) and change agent-generated (action plans and weekly reflective reports of change agents’ activities). Normalization Process Theory (NPT) was used to analyze and explain the work of interacting actors in implementing and then normalizing (embedding) the intervention across contexts and times. Data analysis was abductive, generating inductive codes then identified with NPT constructs. Across the three cases, change agents had to work through numerous implementation challenges: needing to make sense of the intervention package, the PDSA model as implementation method, and their own role as change agents and to orientate these within their action context (coherence). They had to work to encourage colleagues to invest in these changes (cognitive participation) and find ways to implement the intervention by mobilising changes (collective action). Finally, they created strategies for clinical routines to continue to self-review, reconfiguring actions and future plans to enable the intervention to be sustained (reflexive monitoring).ConclusionsSuccessful implementation of the (PERFECT-ER) intervention requires change agents to recognize and engage with local values, and then to enable its fit with practice and wider contextual goals. A context of constant change fragments normalization. Thus, sustaining practice change over time is fragile and requires change agents to continue a recursive two-way sense-making process. This enables implementation and normalization to re-energize and overcome barriers to change.</div

Operationalising the Recovery College model with people living with dementia: a realist review
ObjectivesPost-diagnostic support is a significant factor in facilitating personal recovery following a diagnosis of dementia, but access is often inconsistent and insufficient. Recovery Colleges offer peer-led, co-produced courses that can support people to have meaningful lives and have been adapted for use in the context of dementia. A realist review was conducted to understand the application and sustainability of Recovery College dementia courses.MethodAn iterative, five-step process combined literature published to 2023 with knowledge from stakeholders with lived and professional experience of dementia involved with Recovery College dementia courses (PROSPERO registration CRD42021293687).ResultsThirty-five documents and discussions with 19 stakeholders were used to build the initial programme theory comprising of 24 context-mechanism-outcome configurations. Reoccurring factors included: attending to aspects of co-production and course delivery to ensure they promoted inclusion and were not compromised by organisational pressures; how stigma impacted access to course opportunities; and embedding personal recovery principles throughout course development to be relevant for people living with dementia and those who support them.ConclusionPeople struggling to reconcile their future alongside dementia need practical and emotional support to access and benefit from Recovery College dementia courses, ways to achieve this will be explored through a realist evaluation
