    Delayed Citrus Stimulation and Prevention of Salivary Glands Damage in Patients of Thyroid Carcinoma Treated by Radioiodine Therapy

    Objective: It is well established that citrus stimulation has a definite role in preventing salivary gland damage after radioiodine therapy. The aim of this study was to observe how effectively this stimulation can be used to prevent salivary gland damage and whether the timing of sucking citrus substances play any role in preventing this damage more effectively.&#x0D; Patients and Methods: A total of 128 differentiated thyroid cancer (DTC) patients who received radioiodine treatment for thyroid carcinoma were divided into two groups- group-A and group-B. Group-A patients started citrus stimulation 1 hour after radioiodine ingestion and group-B patients started 24 hours after RAIT. Patients were followed-up at 5th day, 3rd month, and 6th month after RAIT. Follow up procedure consisted of physical examination and history of pain and swelling in salivary gland regions, taste dysfunction and dryness of mouth.&#x0D; Result: The incidence of different types of salivary glands side effects and abnormal scan findings were significantly high in the group of patients who were advised to start citrus stimulation 1 h after radioiodine ingestion. On the other hand, patients who started citrus stimulation 24 h after RAIT suffered less from salivary gland side effects.  Conclusion: This study showed that delayed initiation of citrus stimulation was more effective in reducing salivary gland damage after RAIT.&#x0D; Bangladesh J.     Visual Inspection of Cervix wiih Acetic Acid as A Method of Cervical Cancer Screening in Tertiary Hospital

    Background: Cervical cancer belongs to a glorious natural history which favors well implemented secondary prevention. Considering the various constraints in our country, our study aimed to evaluate Visual Inspection of Cervix with Acetic Acid (VIA) as an alternative or adjunct to cervical cytology (Pap smear) in early detection of cervical cancer and also to assess its effectiveness as a screening method in a tertiary center outside of low resource settings.&#x0D; Materials and methods: This cross-sectional descriptive study was carried out at Gynae OPD of Chittagong Medical College Hospital (CMCH) from January to December 2012. A total of 1600 sexually active women with 21 to 60 years old were enrolled in this study by purposive sampling technique. The women underwent a complete clinical evaluation, including Pap smear and VIA. Any positive test was referred to colposcopy and directed biopsy. Finally, data were analyzed with the help of SPSS 16 software.&#x0D; Results: Out of 1600, VIA was positive in 135(8.4%) and cytology was positive in 74(4.6%) cases of the true positive 48 cases. The sensitivity of VIA (75%) was much higher than that of Pap smear (48%). The sensitivity ratio 1.54 (p&lt;0.001). The positive predictive value for detection of CIN2 and above was 17% for VIA and 16% for pap (p=0.5). Moreover, patients of lost to follow-up before colposcopy and biopsy were only 4.44% with VIA positive cases while 28.38% with positive Pap smear (p&lt;0.0001).&#x0D; Conclusions: The high sensitivity of VIA indicates that it may be a feasible alternative test for detection of precancerous lesions of the cervix not only in limited-resource settings but also in a well-equipped tertiary hospital.&#x0D; IAHS Medical Journal Vol 4(2), June 2021; 22-26</jats:p

    Effect of Spirulina (Arthrospira platensis) on blood uric acid, hemoglobin and kidney histo-texture of mice treated with acetaminophen

    Background: Acetaminophen is a medication used to treat pain and fever. It is typically used for mild to moderate pain relief. Spirulina is a biomass of cyanobacteria that cultivated worldwide and used as a dietary supplement or whole food. It is also used as a feed supplement in the aquaculture, aquarium, and poultry industries. The present study was undertaken to determine the effect of Spirulina on blood and kidney in mice. Methods: Twenty Swiss Albino mice (Mus musculus) were randomly divided into 4 equal groups’ viz., A, B, C, and D. Group A was kept as control and fed only normal diet. Group B was supplemented with acetaminophen where as group C was supplemented with acetaminophen and Spirulina, and group D was treated with Spirulina. Results: The application of acetaminophen did not have significant effect on hemoglobin and uric acid content. But the addition of Spirulina to the diet increased significantly (P&lt;0.05) hemoglobin and uric acid level. Histopathological examination revealed that no significant changes were found in the kidney of all of treated groups in comparison with the mice of control group. Conclusions: The research work suggests that long-term ingestion of acetaminophen does not exert a significant change in hemoglobin or uric acid level but spirulina supplemented diet may be associated with the rise in hemoglobin and uric acid level without affecting renal tissue texture in a significant manner.</jats:p

    A Systematic Review and Meta-Analysis of Fecal Contamination and Inadequate Treatment of Packaged Water

    <div><p>Background</p><p>Packaged water products provide an increasingly important source of water for consumption. However, recent studies raise concerns over their safety.</p><p>Objectives</p><p>To assess the microbial safety of packaged water, examine differences between regions, country incomes, packaged water types, and compare packaged water with other water sources.</p><p>Methods</p><p>We performed a systematic review and meta-analysis. Articles published in English, French, Portuguese, Spanish and Turkish, with no date restrictions were identified from online databases and two previous reviews. Studies published before April 2014 that assessed packaged water for the presence of <i>Escherichia coli</i>, thermotolerant or total coliforms were included provided they tested at least ten samples or brands.</p><p>Results</p><p>A total of 170 studies were included in the review. The majority of studies did not detect fecal indicator bacteria in packaged water (78/141). Compared to packaged water from upper-middle and high-income countries, packaged water from low and lower-middle-income countries was 4.6 (95% CI: 2.6–8.1) and 13.6 (95% CI: 6.9–26.7) times more likely to contain fecal indicator bacteria and total coliforms, respectively. Compared to all other packaged water types, water from small bottles was less likely to be contaminated with fecal indicator bacteria (OR = 0.32, 95%CI: 0.17–0.58) and total coliforms (OR = 0.10, 95%CI: 0.05, 0.22). Packaged water was less likely to contain fecal indicator bacteria (OR = 0.35, 95%CI: 0.20, 0.62) compared to other water sources used for consumption.</p><p>Conclusions</p><p>Policymakers and regulators should recognize the potential benefits of packaged water in providing safer water for consumption at and away from home, especially for those who are otherwise unlikely to gain access to a reliable, safe water supply in the near future. To improve the quality of packaged water products they should be integrated into regulatory and monitoring frameworks.</p></div
