Diagnosis of Helicobacter pylori by carbon-13 urea breath test using a portable mass spectrometer
CONTEXT: In the non-invasive detection of markers of disease, mass spectrometry is able to detect small quantities of volatile markers in exhaled air. However, the problem of size, expense and immobility of conventional mass spectrometry equipment has restricted its use. Now, a smaller, less expensive, portable quadrupole mass spectrometer system has been developed. Helicobacter pylori has been implicated in the development of chronic gastritis, gastric and duodenal ulcers and gastric cancer. OBJECTIVES: To compare the results obtained from the presence of H. pylori by a carbon-13 urea test using a portable quadrupole mass spectrometer system with those from a fixed mass spectrometer in a hospital-based clinical trial. METHODS: Following ethical approval, 45 patients attending a gastroenterology clinic at the Royal Liverpool University Hospital exhaled a breath sample into a Tedlar gas sampling bag. They then drank an orange juice containing urea radiolabelled with carbon and 30âmin later gave a second breath sample. The carbon-13 content of both samples was measured using both quadrupole mass spectrometer systems. If the post-drink level exceeded the pre-drink level by 3% or more, a positive diagnosis for the presence of H. pylori was made. RESULTS: The findings were compared to the results using conventional isotope ratio mass spectrometry using a laboratory-based magnetic sector instrument off-site. The results showed agreement in 39 of the 45 patients. CONCLUSIONS: This study suggests that a portable quadrupole mass spectrometer is a potential alternative to the conventional centralised testing equipment. Future development of the portable quadrupole mass spectrometer to reduce further its size and cost is indicated, together with further work to validate this new equipment and to enhance its use in mass spectrometry diagnosis of other medical conditions
âThereâs no way that you get paid to do the artsâ: unpaid labour across the cultural and creative life course
Unpaid labour is an important element of how precarity has been theorised. It is also an issue that is often seen as endemic to cultural and creative work. Questions as to the role of unpaid work, including but not limited to unpaid internships, have become central to understanding how the social exclusiveness of many cultural and creative jobs is reinforced through their precarity. This article uses survey and interview data to outline the differing experiences of unpaid labour in cultural jobs. It contrasts the meaning of âfreeâ work over the life courses of a range of creative workers, showing how it is stratified by social class, age, and career stage. By exploring the stratification of unpaid work as a form of precariousness in cultural jobs, and of who describes their experiences of unpaid work as benign, the article offers new empirical evidence for those challenging the negative impacts of precarious working conditions
Atmospheric CO2 over the last 1000 years: A high-resolution record from the West Antarctic Ice Sheet (WAIS) Divide ice core
We report a decadally resolved record of atmospheric CO2 concentration for the last 1000 years, obtained from the West Antarctic Ice Sheet (WAIS) Divide shallow ice core. The most prominent feature of the preâindustrial period is a rapid âź7 ppm decrease of CO2 in a span of âź20â50 years at âź1600 A.D. This observation confirms the timing of an abrupt atmospheric CO2 decrease of âź10 ppm observed for that time period in the Law Dome ice core CO2 records, but the true magnitude of the decrease remains unclear. Atmospheric CO2 variations over the time period 1000â1800 A.D. are statistically correlated with northern hemispheric climate and tropical IndoâPacific sea surface temperature. However, the exact relationship between CO2 and climate remains elusive due to regional climate variations and/or uneven geographical data density of paleoclimate records. We observe small differences of 0 âź 2% (0 âź 6 ppm) among the highâprecision CO2 records from the Law Dome, EPICA Dronning Maud Land and WAIS Divide Antarctic ice cores. However, those records share common trends of CO2 change on centennial to multicentennial time scales, and clearly show that atmospheric CO2 has been increasing above preindustrial levels since âź1850 A.D
Antenatal atazanavir: a retrospective analysis of pregnancies exposed to atazanavir.
INTRODUCTION: There are few data regarding the tolerability, safety, or efficacy of antenatal atazanavir. We report our clinical experience of atazanavir use in pregnancy.
METHODS: A retrospective medical records review of atazanavir-exposed pregnancies in 12 London centres between 2004 and 2010.
RESULTS: There were 145 pregnancies in 135 women: 89 conceived whilst taking atazanavir-based combination antiretroviral therapy (cART), "preconception" atazanavir exposure; 27 started atazanavir-based cART as "first-line" during the pregnancy; and 29 "switched" to an atazanavir-based regimen from another cART regimen during pregnancy. Gastrointestinal intolerance requiring atazanavir cessation occurred in five pregnancies. Self-limiting, new-onset transaminitis was most common in first-line use, occurring in 11.0%. Atazanavir was commenced in five switch pregnancies in the presence of transaminitis, two of which discontinued atazanavir with persistent transaminitis. HIV-VL < 50 copies/mL was achieved in 89.3% preconception, 56.5% first-line, and 72.0% switch exposures. Singleton preterm delivery (<37 weeks) occurred in 11.7% preconception, 9.1% first-line, and 7.7% switch exposures. Four infants required phototherapy. There was one mother-to-child transmission in a poorly adherent woman.
CONCLUSIONS: These data suggest that atazanavir is well tolerated and can be safely prescribed as a component of combination antiretroviral therapy in pregnancy
Relationships between child development at school entry and adolescent health - a participatory systematic review
The relationship between child development and adolescent health, and how this may be modified by socio-economic conditions, is poorly understood. This limits cross-sector interventions to address adolescent health inequality. This review summarises evidence on the associations between child development at school starting age and subsequent health in adolescence and identifies factors affecting associations. We undertook a participatory systematic review, searching electronic databases (MEDLINE, PsycINFO, ASSIA and ERIC) for articles published between November 1990 and November 2020. Observational, intervention and review studies reporting a measure of child development and subsequent health outcomes, specifically weight and mental health, were included. Studies were individually and collectively assessed for quality using a comparative rating system of stronger, weaker, inconsistent or limited evidence. Associations between child development and adolescent health outcomes were assessed and reported by four domains of child development (socio-emotional, cognitive, language and communication, and physical development). A conceptual diagram, produced with stakeholders at the outset of the study, acted as a framework for narrative synthesis of factors that modify or mediate associations. Thirty-four studies were included. Analysis indicated stronger evidence of associations between measures of socio-emotional development and subsequent mental health and weight outcomes; in particular, positive associations between early externalising behaviours and later internalising and externalising, and negative associations between emotional wellbeing and later internalising and unhealthy weight. For all other domains of child development, although associations with subsequent health were positive, the evidence was either weaker, inconsistent or limited. There was limited evidence on factors that altered associations. Positive socio-emotional development at school starting age appears particularly important for subsequent mental health and weight in adolescence. More collaborative research across health and education is needed on other domains of development and on the mechanisms that link development and later health, and on how any relationship is modified by socio-economic context
A Hot Uranus Orbiting the Super Metal-rich Star HD77338 and the Metallicity - Mass Connection
We announce the discovery of a low-mass planet orbiting the super metal-rich
K0V star HD77338 as part of our on-going Calan-Hertfordshire Extrasolar Planet
Search. The best fit planet solution has an orbital period of 5.7361\pm0.0015
days and with a radial velocity semi-amplitude of only 5.96\pm1.74 m/s, we find
a minimum mass of 15.9+4.7-5.3 Me. The best fit eccentricity from this solution
is 0.09+0.25-0.09, and we find agreement for this data set using a Bayesian
analysis and a periodogram analysis. We measure a metallicity for the star of
+0.35\pm0.06 dex, whereas another recent work (Trevisan et al. 2011) finds
+0.47\pm0.05 dex. Thus HD77338b is one of the most metal-rich planet host stars
known and the most metal-rich star hosting a sub-Neptune mass planet. We
searched for a transit signature of HD77338b but none was detected. We also
highlight an emerging trend where metallicity and mass seem to correlate at
very low masses, a discovery that would be in agreement with the core accretion
model of planet formation. The trend appears to show that for Neptune-mass
planets and below, higher masses are preferred when the host star is more
metal-rich. Also a lower boundary is apparent in the super metal-rich regime
where there are no very low-mass planets yet discovered in comparison to the
sub-solar metallicity regime. A Monte Carlo analysis shows that this, low-mass
planet desert, is statistically significant with the current sample of 36
planets at around the 4.5\sigma\ level. In addition, results from Kepler
strengthen the claim for this paucity of the lowest-mass planets in super
metal-rich systems. Finally, this discovery adds to the growing population of
low-mass planets around low-mass and metal-rich stars and shows that very
low-mass planets can now be discovered with a relatively small number of data

Morbid Obesity as a Risk Factor for Hospitalization and Death Due to 2009 Pandemic Influenza A(H1N1) Disease
BACKGROUND: Severe illness due to 2009 pandemic A(H1N1) infection has been reported among persons who are obese or morbidly obese. We assessed whether obesity is a risk factor for hospitalization and death due to 2009 pandemic influenza A(H1N1), independent of chronic medical conditions considered by the Advisory Committee on Immunization Practices (ACIP) to increase the risk of influenza-related complications. METHODOLOGY/PRINCIPAL FINDINGS: We used a case-cohort design to compare cases of hospitalizations and deaths from 2009 pandemic A(H1N1) influenza occurring between April-July, 2009, with a cohort of the U.S. population estimated from the 2003-2006 National Health and Nutrition Examination Survey (NHANES); pregnant women and children <2 years old were excluded. For hospitalizations, we defined categories of relative weight by body mass index (BMI, kg/m(2)); for deaths, obesity or morbid obesity was recorded on medical charts, and death certificates. Odds ratio (OR) of being in each BMI category was determined; normal weight was the reference category. Overall, 361 hospitalizations and 233 deaths included information to determine BMI category and presence of ACIP-recognized medical conditions. Among >or=20 year olds, hospitalization was associated with being morbidly obese (BMI>or=40) for individuals with ACIP-recognized chronic conditions (OR = 4.9, 95% CI 2.4-9.9) and without ACIP-recognized chronic conditions (OR = 4.7, 95%CI 1.3-17.2). Among 2-19 year olds, hospitalization was associated with being underweight (BMI<or=5(th) percentile) among those with (OR = 12.5, 95%CI 3.4-45.5) and without (OR = 5.5, 95%CI 1.3-22.5) ACIP-recognized chronic conditions. Death was not associated with BMI category among individuals 2-19 years old. Among individuals aged >or=20 years without ACIP-recognized chronic medical conditions death was associated with obesity (OR = 3.1, 95%CI: 1.5-6.6) and morbid obesity (OR = 7.6, 95%CI 2.1-27.9). CONCLUSIONS/SIGNIFICANCE: Our findings support observations that morbid obesity may be associated with hospitalization and possibly death due to 2009 pandemic H1N1 infection. These complications could be prevented by early antiviral therapy and vaccination
The NANOGrav 11-Year Data Set: Limits on Gravitational Waves from Individual Supermassive Black Hole Binaries
Observations indicate that nearly all galaxies contain supermassive black
holes (SMBHs) at their centers. When galaxies merge, their component black
holes form SMBH binaries (SMBHBs), which emit low-frequency gravitational waves
(GWs) that can be detected by pulsar timing arrays (PTAs). We have searched the
recently-released North American Nanohertz Observatory for Gravitational Waves
(NANOGrav) 11-year data set for GWs from individual SMBHBs in circular orbits.
As we did not find strong evidence for GWs in our data, we placed 95\% upper
limits on the strength of GWs from such sources as a function of GW frequency
and sky location. We placed a sky-averaged upper limit on the GW strain of at nHz. We also developed a
technique to determine the significance of a particular signal in each pulsar
using ``dropout' parameters as a way of identifying spurious signals in
measurements from individual pulsars. We used our upper limits on the GW strain
to place lower limits on the distances to individual SMBHBs. At the
most-sensitive sky location, we ruled out SMBHBs emitting GWs with
nHz within 120 Mpc for , and
within 5.5 Gpc for . We also determined that
there are no SMBHBs with emitting
GWs in the Virgo Cluster. Finally, we estimated the number of potentially
detectable sources given our current strain upper limits based on galaxies in
Two Micron All-Sky Survey (2MASS) and merger rates from the Illustris
cosmological simulation project. Only 34 out of 75,000 realizations of the
local Universe contained a detectable source, from which we concluded it was
unsurprising that we did not detect any individual sources given our current

The Star Formation History in The Far Outer Disc of M33
The outer regions of disc galaxies are becoming increasingly recognized as
key testing sites for models of disc assembly and evolution. Important issues
are the epoch at which the bulk of the stars in these regions formed and how
discs grow radially over time. To address these issues, we use Hubble Space
Telescope Advanced Camera for Surveys imaging to study the star formation
history (SFH) of two fields at 9.1 and 11.6 kpc along M33's northern major
axis. These fields lie at ~ 4 and 5 V-band disc scale-lengths and straddle the
break in M33's surface brightness profile. The colour-magnitude diagrams (CMDs)
reach the ancient main sequence turnoff with a signal-to-noise ratio of ~ 5.
From detailed modelling of the CMDs, we find that the majority of stars in both
fields combined formed at z < 1. The mean age in the inner field, S1, is ~ 3
+/- 1 Gyr and the mean metallicity is [M/H] ~ -0.5 +/- 0.2 dex. The star
formation history of S1 unambiguously reveals how the inside-out growth
previously measured for M33's inner disc out to ~ 6 kpc extends out to the disc
edge at ~ 9 kpc. In comparison, the outer field, S2, is older (mean age ~ 7 +/-
2 Gyr), more metal-poor (mean [M/H] ~ -0.8 +/- 0.3 dex), and contains ~ 30
times less stellar mass. These results provide the most compelling evidence yet
that M33's age gradient reverses at large radii near the disc break and that
this reversal is accompanied by a break in stellar mass surface density. We
discuss several possible interpretations of this behaviour including radial
stellar mixing, warping of the gaseous disc, a change in star formation
efficiency, and a transition to another structural component. These results
offer one of the most detailed views yet of the peripheral regions of any disc
galaxy and provide a much-needed observational constraint on the last major
