Using serum CA125 to assess the activity of potential cytostatic agents in ovarian cancer

Objective: New strategies are required to rapidly identify novel cytostatic agents before embarking on large randomized trials. This study investigates whether a change in rate of rise (slope) of serum CA125 from before to after starting a novel agent could be used to identify cytostatic agents. Tamoxifen was used to validate this hypothesis. Methods: Asymptomatic patients with relapsed ovarian cancer who had responded to chemotherapy were enrolled and had CA125 measurements taken every 4 weeks, then more frequently when rising. Once levels reached 4 times the upper limit of normal or nadir, they started continuous tamoxifen 20 mg daily, as well as fortnightly CA125 measurements until symptomatic progression. Because of the potentially nonlinear relationship of CA125 over time, it was felt that to enable normal approximations to be utilized a natural logarithmic standard transformation [ln(CA125)] was the most suitable to improve linearity above the common logarithmic transformation to base 10. Results: From 235 recruited patients, 81 started tamoxifen and had at least 4 CA125 measurements taken before and 4 CA125 measurements taken after starting tamoxifen, respectively. The mean regression slopes from using at least 4 1n(CA125) measurements immediately before and after starting tamoxifen were 0I0149 and 0I0093 [ln(CA125)/d], respectively. This difference is statistically significant, P = 0I001. Therefore, in a future trial with a novel agent, at least as effective as tamoxifen, using this effect size, the number of evaluable patients needed, at significance level of 5% and power of 80%, is 56. Conclusions: Further validation of this methodology is required, but there is potential to use comparison of mean regression slopes of ln(CA125) as an interim analysis measure of efficacy for novel cytostatic agents in relapsed ovarian cancer.Peer reviewedFinal Accepted Versio

Addisonian crisis complicated by rhabdomyolysis and acute kidney injury

Addisonian Crisis is an endocrine emergency requiring hospitalization and urgent management. The diagnosis is usually complicated by nonspecific sign and symptoms of primary adrenal insufficiency. A high degree of suspicion Is required in such cases. Rhabdomyolysis caused by addisonian crisis is a rare complication which was seen in our patient

Combinatorial extensions of Popoviciu\u27s inequality via Abel-Gontscharoff polynomial with applications in information theory

    Get PDF
We establish new refinements and improvements of Popoviciu’s inequality for n-convex functions using Abel-Gontscharoff interpolating polynomial along with the aid of new Green functions. We construct new inequalities for n-convex functions and compute new upper bounds for Ostrowski and Grüss type inequalities. As an application of our work in information theory, we give new estimations for Shannon, Relative and Zipf-Mandelbrot entropies using generalized Popoviciu’s inequality

Language learning in the 21st century develops in tandem with technology. The growth of social media as the platform for communication is a positive element that can influence students’ learning especially in languages. One clear advantage of having such media is that students can be autonomous in their learning because they are already familiar with the platform and they use these sites in their daily lives. VELLP or Virtual English Language Learning Programme is one such agenda that has managed to take advantage of the popular usage of social media amongst students today. This programme is created to cater to thirty students of SMK Bakar Arang, Sungai Petani who have the potential to become better English language users. All thirty students are invited to join the online learning community in Facebook, moderated by four English language lecturers. Diverse topics and interesting issues were raised in the online forum, enabling the students to communicate with one another using correct language. A pretest and a posttest were given; and data from the tests were tabulated to show the effectiveness of the programme. All in all, VELLP has managed to prove that an online chatroom or an online community is able to facilitate second language learners to practice their writing skills in the target language in a fun and relaxed manner

Composite split cord malformation associated with a dermal sinus tract, dermoid cyst, and epidural abscess: A case report and review of literature

    Get PDF
Background: Split cord malformation (SCM) is typically present at a single level but rarely, may be present at multiple levels in the spinal cord and can be associated with a wide array of lesions such as myelomeningoceles, lipomas, teratomas, and dermal sinus tracts (DSTs).Case description: We describe a case of a 15-month-old female child who presented with high-grade fever and progressive motor weakness in the lower limbs. Magnetic resonance imaging revealed the presence of SCM along with an epidural abscess, DST, and dermoid cyst. The child underwent surgery for excision of DST along with removal of the dermoid cyst and drainage of epidural abscess. The postoperative course was uneventful. Elective repair of the SCM was performed 4 weeks later. The postoperative course was uneventful again. Conclusion: To the best of our knowledge, the combination of a composite SCM with a DST and dermoid cyst with associated epidural abscess has rarely been reported in literature

State estimation technique for a planetary robotic rover

    Get PDF
Given the long traverse times and severe environmental constraints on a planet like Mars, the only option feasible now is to observe and explore the planet through more sophisticated planetary rovers. To achieve increasingly ambitious mission objectives under such extreme conditions, the rovers must have autonomy. Increased autonomy, obviously, relies on the quality of estimates of rover's state i.e. its position and orientation relative to some starting frame of reference. This research presents a state estimation approach based on Extended Kalman Filter (EKF) to fuse distance from odometry and attitude from an Inertial Measurement Unit (IMU), thus mitigating the errors generated by the use of either system alone. To simulate a Sun-sensor based approach for absolute corrections, a magnetic compass was used to give absolute heading updates. The technique was implemented on MotherBot, a custom-designed skid steered rover. Experimental results validate the application of the presented estimation strategy. It showed an error range within 3% of the distance travelled as compared to about 8% error obtained from direct fusion
