    Title: Assessment of Student Perception Regarding Clinical Training in Undergraduate Dental Programs Using Clin Ed IQ.

    Purpose. Clinical training is essential part of the dental curriculum. Both clinical environment and Teachers are essential factors which affect the acquired clinical skills. The objective was to assess the perceptions of dental students regarding their clinical training. Methods. This mixed method study using Clin Ed IQ questionnaire was conducted on Dental students graduating from both Private and Public sector institutes of Karachi. The questionnaire had four sections, the first three based on various aspect of clinical training, with responses based on 5-point Likert scale. Section four had open-ended items about dental curriculum. Results. For responses of 220 participants related to the first three sections, mean and standard deviation was calculated. Section two about Involvement in specific learning activities, had the lowest composite mean score (8.4). Results for all responses of agreement were tabulated as percentages. Overall, the scores were positive, with Section three regarding interaction with clinical instructors having highest positive responses. Dental graduates of Private sector institutes were more satisfied by their clinical training. Thematic analysis of open-ended questions yielded themes related to trained faculty, communication skills, patient safety skills, diversity, and integrated curriculum. Conclusion. The students of both types of institutes reported lack of diversity in clinical cases and deficient feedback from instructors as their major concern. However, the results showed a positive perception to their clinical experiences. Institutes and regulatory authorities should address areas of concern, to ensure provision of a holistic clinical learning environment to dental graduates

    Extreme returns and the investor’s expectation for future volatility: Evidence from the Finnish stock market

    We examine the significance of extreme positive returns of the previous month (MAX) as a return predictor in the Finnish stock market. We show that high fear months, i.e., months associated with the investor’s high expectation for future volatility, are accompanying with low MAX effect implying that investors reluctant to gamble in high MAX stocks when they have high expectation for future volatility.</p

    Affine Invariant Contour Descriptors Using Independent Component Analysis and Dyadic Wavelet Transform

    The paper presents a novel technique for affine invariant feature extraction with the view of object recognition based on parameterized contour. The proposed technique first normalizes an input image by removing the affine deformations using independent component analysis which also reduces the noise introduced during contour parameterization. Then four invariant functionals are constructed using the restored object contour, dyadic wavelet transform and conics in the context of wavelets. Experimental results are conducted using three different standard datasets to confirm the validity of the proposed technique. Beside this the error rates obtained in terms of invariant stability are significantly lower when compared to other wavelet based invariants. Also the proposed invariants exhibit higher feature disparity than the method of Fourier descriptors

    Risk factors for mortality of patients with ceftriaxone resistant E. coli bacteremia receiving carbapenem versus beta lactam/beta lactamase inhibitor therapy

    Objective: Extended spectrum β-lactamases (ESBL) producing Enterobacteriaceae predominantly E. coli and K. pneumoniae bacteremia have limited treatment options and high mortality. The objective was to determine the risk factors for in-hospital mortality particularly treatment with carbapenem versus beta lactam/beta lactamase combination (BL/BLI) in patients with ceftriaxone resistant E. coli bacteremia. A retrospective cohort study was conducted at the Aga Khan University, Karachi, Pakistan. Adult patients with sepsis and monomicrobial ceftriaxone resistant E. coli bacteremia were enrolled. Factors associated with mortality in patients were determined using logistic regression analysis. Results: Mortality rate was 37% in those empirically treated with carbapenem compared to 20% treated with BL/BLI combination therapy (p-value: 0.012) and was 21% in those treated with a carbapenem compared to 13% in patients definitively treated with BL/BLI combination therapy (p-value: 0.152). In multivariable logistic regression analysis, only Pitt bacteremia score of ≥ four was significantly associated with mortality (OR: 7.7 CI 2.6-22.8) while a urinary source of bacteremia was protective (OR: 0.26 CI 0.11-0.58). In-hospital mortality in patients with Ceftriaxone resistant E. coli bacteremia did not differ in patients treated with either a carbapenem or BL/BLI combination. However, Pitt bacteremia score of ≥ 4 was strongly associated with mortality

    India being a developing economy is one of the emerging markets in the world. But an obstacle that still stands in its way of development is the scarcity of financial/monetary resources required for sustainable investments. The giants of Microfinance in India are known to be charging high rates and the smaller players follow in their footsteps. For such regions, Interest-free microfinance is what is needed to uplift the minority and take them out of the fangs of poverty. Interest-free microfinance and its implementation can prove to be very useful in eradicating poverty and providing sustainable development of micro-enterprises and the expansion of financial inclusion. Thus, the aim of this study is to examine the prospects of Islamic microfinance in India. The quantitative method is employed, and primary data is collected through a questionnaire. The probability sampling method is used. Out of 500 questionnaires disbursed, 303 are received. The result of the questionnaire shows that majority of the public believes Islamic Microfinance Institions (IMFIs) will aid the poor financially, poverty will be alleviated, it will be beneficial to the society, and will help micro SMEs become more developed. Similarly, majority of the respondents also agreed to IMFIs being ethical, meant for all religions, catering to the needs of the society and help to extend loans without collaterals. This reflects that Islamic Microfinance holds great potential and prospects and will be widely accepted by the public and also the Indian market. Policymakers should consider IMFIs, establish guidelines and promulgate them as this can pave way to a financially more stable India. The limitation of the study is that the outreach was among people with access to the internet. Therefore, perception of those in rural areas, people with no internet and education could not be included. Hence, future research will fulfill this lacun

    The coronary steal phenomenon refers to myocardial ischemia caused by the diversion of blood away from normal myocardial circulation. A coronary artery fistula (CAF) is an abnormality of the coronary anatomy characterized by the aberrant termination of a coronary artery or its branches into cardiac chambers or great vessels. Although CAFs are often thought to be asymptomatic, fistulas that diverge a significant amount of blood flow and decrease the normal perfusion of myocardial tissue can cause ischemia and can present with acute coronary syndrome. We describe a unique case of a 70-year-old woman with no coronary artery disease (CAD) undergoing ventricular fibrillation and sudden cardiac arrest from myocardial ischemia secondary to coronary steal brought on by multivessel CAFs. This case was unique in that fistulas originating from the left anterior descending and from the circumflex artery draining into the left heart chambers are the least frequently observed. To our knowledge, only two other reports in the literature, demonstrating sudden cardiac arrest in patients with left anterior descending to left ventricle fistulas with no CAD, exist. The case presented, along with the literature reviewed, demonstrates that CAFs may be an important part of the differential diagnosis of symptoms of chest pain and myocardial ischemia, particularly in middle-aged adults with no history of coronary artery disease or related comorbidities

    ABSTRACT The butanol fraction of the leaves of Castanospermum australe Cunn. &amp; Fraser., was tested on adult Callosobruchus analis (pulse beetle) for its insecticidal activity by impregnation method at concentration of 3, 9, 48, 240 and 354 µg / cm 2 . The result showed that the extract is toxic to Callosobruchus analis and the LD 50 was found to be 38 g/cm 2

Fractal technique for extraction of topographicfractal dimension (Dtopo) and vertical dissection (Rtopo-surf) were used to assess the Neotectonics deformation.Geomorphological regions can be demarcated by using above mentioned procedures as they reveal the topographicevolutionary stages as an evaluation of topographic textures. The idea is to recognize the zones that are stronglyinfluenced by Neotectonics. Astore-Deosai-Skardu (ADS) region in Gilgit-Baltistan was selected for this purpose as itlies between MKT and MMT which is experiencing surface topographic deformation (STD) caused by anti-clock-wiseprogression and subduction of Indian plate beneath Eurasia. The results obtained illustrates that Dtopo and Rtopo-surf showanomalies in the ADS region that clearly represent a robust control of nearby MMT, MKT and KkF and highlights theirsignificance to describe regions vulnerable to Neotectonics and related deadly events threatening precious human livesand infrastructure damages
