Hypoglossal and recurrent laryngeal nerve lesions in their unilateral or bilateral extracranial pathways cause a motor deficit in the tongue with transient, partial or total loss of speech, which may be associated with mild or severe dysphagia, characterizing Tapia’s syndrome. This unusual condition is usually related to airway manipulation during the anesthetic-surgical procedure, and few cases in the literature are correlated with maxillofacial surgery, which explains, in part, the lack of knowledge of this complication by a significant number of professionals. We describe a case report of bilateral Tapia syndrome established after a procedure for maxillomandibular advancement. This is a 55-year-old male patient diagnosed with retrognathism and retromaxilism. The patient underwent maxillomandibular advancement with nasotracheal intubation. He evolved with bilateral Tapia syndrome, recovering from the signs and symptoms of this complication, being reoperated on to correct aesthetic results obtained in the first intervention, which were below acceptable, not evolving after this second surgery with the syndrome. We conclude that preventive measures must be taken, such as avoiding sudden movements in the head region and prolonged hyperextension of the neck during surgery, minimizing trauma to the tracheal tube on the tissues that cover the hypoglossal and recurrent laryngeal nerves.Hypoglossal and recurrent laryngeal nerve lesions in their unilateral or bilateral extracranial pathways cause a motor deficit in the tongue with transient, partial or total loss of speech, which may be associated with mild or severe dysphagia, characterizing Tapia’s syndrome. This article problematizes some imagetic meanings articulated to the consumption of serialized narratives. In order to undertake this analysis, we start from a theoretical contextualization about serialization of culture and decentralization of mass culture, analyzing the imagery of consumption articulated in serialization. We turn, then, in a historical perspective, to model lifestyle images, by massively disseminating mass communication in an American television series produced in the 1960s, Bewitched. In parallel with the normative action of the diffusion of this model, contradictions and contextual factors intended the pattern, evidencing involuntary intentional actions that dialogued with distinct portions of the population that were not conniving or contemplated by the status quo. Thus, it is possible to understand that there is the dissemination of ambivalent imaginaries by the series and in the process of their consumption

Pancreatic Necrosis and Gas in the Retroperitoneum: Treatment with Antibiotics Alone

    Get PDF
OBJECTIVE: To present our experience in the management of patients with infected pancreatic necrosis without drainage. METHODS: The records of patients with pancreatic necrosis admitted to our facility from 2011 to 2015 were retrospectively reviewed. RESULTS: We identified 61 patients with pancreatic necrosis. Six patients with pancreatic necrosis and gas in the retroperitoneum were treated exclusively with clinical support without any type of drainage. Only 2 patients had an APACHE II score >;8. The first computed tomography scan revealed the presence of gas in 5 patients. The Balthazar computed tomography severity index score was >;9 in 5 of the 6 patients. All patients were treated with antibiotics for at least 3 weeks. Blood cultures were positive in only 2 patients. Parenteral nutrition was not used in these patients. The length of hospital stay exceeded three weeks for 5 patients; 3 patients had to be readmitted. A cholecystectomy was performed after necrosis was completely resolved; pancreatitis recurred in 2 patients before the operation. No patients died. CONCLUSIONS: In selected patients, infected pancreatic necrosis (gas in the retroperitoneum) can be treated without percutaneous drainage or any additional surgical intervention. Intervention procedures should be performed for patients who exhibit clinical and laboratory deterioration

CLIMATE INDICATORS FOR A WATERSHED IN THE EASTERN AMAZON

Abstract: The present study compares the climatological normals for periods of 36 and 34 years (interpolated data) and observed precipitation and air temperature data (2015 and 2016) for four weather stations located within the boundaries of the Itacaiúnas River Watershed (IRW), with the historical data from the Marabá station in the eastern Amazon used as control data. Both the interpolated and observed data were validated with the control data. The quantile technique was used to identify annual precipitation intensity thresholds and seasonal periods. A comparison was made with the precipitation and air temperature data to determine the effects of ENSO events during the period from 1980 to 2016. Our results regarding annual variability showed that the Onça Puma station was the rainiest among the subareas studied. The month of March was characterized as the rainiest month, and July was the least rainy for the watershed area. The influence of ENSO events on air temperature occurred in the early years, from 1980 to 2003, and the stronger events had a greater influence on precipitation in the IRW area. The 2015/16 El Niño may have had a more pronounced effect on the rainfall behavior at the Fazenda Abadia and Sossego stations. In the Salobo station (forest area), there was a delay in precipitation curves related to the occurrence of ENSO events, which may be associated with temporal variations in the dynamics of soil-vegetation-atmosphere interactions

Salmonella typhimurium and Escherichia coli dissimilarity: closely related bacteria with distinct metabolic profiles

Live attenuated strains of Salmonella typhimurium have been extensively investigated as vaccines for a number of infectious diseases. However, there is still little information available concerning aspects of their metabolism. S. typhimurium and Escherichia coli show a high degree of similarity in terms of their genome contents and metabolic networks. However, this work presents experimental evidence showing that significant differences exist in their abilities to direct carbon fluxes to biomass and energy production. It is important to study the metabolism of Salmonella in order to elucidate the formation of acetate and other metabolites involved in optimizing the production of biomass, essential for the development of recombinant vaccines. The metabolism of Salmonella under aerobic conditions was assessed using continuous cultures performed at dilution rates ranging from 0.1 to 0.67 h1, with glucose as main substrate. Acetate assimilation and glucose metabolism under anaerobic conditions were also investigated using batch cultures. Chemostat cultivations showed deviation of carbon towards acetate formation, starting at dilution rates above 0.1 h1. This differed from previous findings for E. coli, where acetate accumulation was only detected at dilution rates exceeding 0.4 h1, and was due to the lower rate of acetate assimilation by S. typhimurium under aerobic conditions. Under anaerobic conditions, both microorganisms mainly produced ethanol, acetate, and formate. A genome-scale metabolic model, reconstructed for Salmonella based on an E. coli model, provided a poor description of the mixed fermentation pattern observed during Salmonella cultures, reinforcing the different patterns of carbon utilization exhibited by these closely related bacteria.
