    Special Operations Forces Culture and Implications for Interagency Collaboration

    The Horn of Africa exemplifies maritime instability due to regional climate, environmental, economic, food, and security issues. Future global challenges require collaborative approaches between U.S. government and military organizations to span organizational boundaries and leverage the strengths and insights of diverse organizations. The purpose of this research was to examine organizational culture and identity, as manifested in organizational literature, to identify opportunities and challenges to interagency networks and collaboration in the realm of confronting wicked problems around the globe. The research questions focus on the cultural and normative elements of organizational identity as manifest in the context of organizational literature. A qualitative organizational ethnographic approach provided a means to analyze the structure, cultures, themes, values, and interpretations of the environment present in the organizational literature and perceptions of those in the communities of interest. Interviews were conducted with 7 individuals who had served in professional capacities with organizations in the Horn of Africa. The study provided a composite description of the inter-organizational space and the results highlight key tensions and opportunities for collaboration and boundary spanning opportunities between U.S. Special Operations and the Department of State. The implications for social change include increased collaboration between organizations and the instruments of national power to better support current and emerging crises and vulnerable communities affected by instability around the world in ways that are more effective, efficient, and sustainable

    Retrospective histologic analyses of bone biopsies and of post mortem samples from normal persons of different age groups, and of bone biopsies of age- and sex-matched groups of patients with primary osteoporosis and aplastic anemia show characteristic age dependent as well as pathologic changes including atrophy of osseous trabeculae and of hematopoiesis, and changes in the sinusoidal and arterial capillary compartments. These results indicate the possible role of a microvascular defect in the pathogenesis of osteoporosis and aplastic anemia

The definitive version was published in Journal of the Acoustical Society of America 117 (2005): 1473-1485, doi:10.1121/1.1828501.The three-dimensional beam pattern of a sperm whale (Physeter macrocephalus) tagged in the Ligurian Sea was derived using data on regular clicks from the tag and from hydrophones towed behind a ship circling the tagged whale. The tag defined the orientation of the whale, while sightings and beamformer data were used to locate the whale with respect to the ship. The existence of a narrow, forward-directed P1 beam with source levels exceeding 210 dBpeak re: 1 µPa at 1 m is confirmed. A modeled forward-beam pattern, that matches clicks >20° off-axis, predicts a directivity index of 26.7 dB and source levels of up to 229 dBpeak re: 1 µPa at 1 m. A broader backward-directed beam is produced by the P0 pulse with source levels near 200 dBpeak re: 1 µPa at 1 m and a directivity index of 7.4 dB. A low-frequency component with source levels near 190 dBpeak re: 1 µPa at 1 m is generated at the onset of the P0 pulse by air resonance. The results support the bent-horn model of sound production in sperm whales. While the sperm whale nose appears primarily adapted to produce an intense forward-directed sonar signal, less-directional click components convey information to conspecifics, and give rise to echoes from the seafloor and the surface, which may be useful for orientation during dives.This work was funded by grants from the Office of Naval Research Grants N00014-99-1-0819 and N00014-01-1-0705, and the Packard Foundation

    Patients with artificial joints: do they need antibiotic cover for dental treatment?

An external link to the publisher’s copy is included.This study reviews whether patients with artificial joints need antibiotic cover for dental treatment. Generally in Australia the practice has developed of giving most patients with artificial joints antibiotic prophylaxis for a wide range of dental procedures. This is partly on anecdotal grounds, partly historical and partly for legal concerns. It has been encouraged by some guidelines. Scientifically, the risk and the benefit of each step in the process needs to be analysed. This review shows that the risk of an artificial joint becoming infected from a bacteraemia of oral origin is exceedingly low whereas the risk of an adverse reaction to the antibiotic prophylaxis is higher than the risk of infection. If all patients with artificial joints receive antibiotic prophylaxis then more will die from anaphylaxis than develop infections. Factors which balance the risk benefit are if the patient is seriously immunocompromised, if the joint prosthesis is failing or chronically inflamed and if the dental procedures, such as from extractions and deep periodontal scaling, produce high level bacteraemias.     Characterising neovascularisation in fracture healing with laser Doppler and micro-CT scanning

    Vascularity of the soft tissues around a bone fracture is critical for successful healing, particularly when the vessels in the medullary canal are ruptured. The objective of this work was to use laser Doppler and micro-computer tomography (micro-CT) scanning to characterise neovascularisation of the soft tissues surrounding the fracture during healing. Thirty-two Sprague–Dawley rats underwent mid-shaft osteotomy of the left femur, stabilised with a custom-designed external fixator. Five animals were killed at each of 2, 4 days, 1, 2, 4 and 6 weeks post-operatively. Femoral blood perfusion in the fractured and intact contralateral limbs was measured using laser Doppler scanning pre- and post-operatively and throughout the healing period. At sacrifice, the common iliac artery was cannulated and infused with silicone contrast agent. Micro-CT scans of the femur and adjacent soft tissues revealed vessel characteristics and distribution in relation to the fracture zone. Blood perfusion dropped immediately after surgery and then recovered to greater than the pre-operative level by proliferation of small vessels around the fracture zone. Multi-modal imaging allowed both longitudinal functional and detailed structural analysis of the neovascularisation process
