COMMUNICATION IN OBSTETRIC PRACTICE AS AN ELEMENT OF SAFE PATIENT CARE
Communication is an art, a complex of methods of speaking, transmitting, perceiving information and intentions. The elements of communication have an impact on the subjects and the processes taking place. Obstetric care has its specifics and is sensitive to the content of information, the participants in the process of communication and the way of perception. The aim of this study is to differentiate the communication relationships of healthcare professionals in obstetric structures with patients, improving the safety of obstetric care. The subject of the study are medical specialists and patients from three obstetric structures in Sofia. Varna. Sociological methods of survey and observation are used. The data is processed using both urgencies and urgencies. Results and discussion: a survey was conducted among 206 respondents-healthcare professionals and patients. 38.09% of healthcare professionals rated communication as very good and 36.07% of patients as good. The problems in communication for the medics are: large administrative activity; workload; incompleteness of the information received; lack of time for communication. In patients, problems refer to: misunderstanding of the information received, insufficient time to communicate with doctors, lack of empathy, poor attitude. According to pregnant women, the leading factors for good obstetric care are personal attention, good communication with specialists, detailed information and support. In conclusion, healthcare professionals must be able to conduct proper and patient-centered communication to provide them with the care they want and need. Using the right communication strategies can help patients feel heard, encourage them to provide accurate and relevant information so they can assist during the healing process

Процесът на адаптация към условията на болничната трудова среда е от важно значение за успешното усвояване на дейностите по оказване на болнични акушерски грижи, за приемане на отговорностите при практикуване на професията. Проблемът с адаптацията на правоспособните здравни специалисти – акушерки за практикуване в болнични условия, е изключително актуален, предвид оскъдността на трудовите ресурси, текучеството на персонала и голямото натоварване. Особено осезаем е проблемът с адаптацията на новозавършилите акушерки и неспособността им за успешна адаптация в болничните структури.The medical institutions for hospital care and the management of health care in them are critically dependent on the number, skills, continuous adaptation and motivation of people working in them. The process of adapting to the conditions of the hospital working environment is of great importance for successfully mastering of the skills related to the provision of hospital obstetric care, and for accepting the responsibilities in practicing the profession. The problem with the adaptation of qualified health care professionals - midwives to hospital practice is extremely topical, given the scarcity of labor resources, staff turnover and heavy workload. Especially significant is the problem of the adaptation of newly graduated midwives and their inability to adapt to hospital structures

The process of adaptation to the conditions of the university education is important for the successful acquisition of a knowledge and skills for the future profession. Organizing Activities to Prevent Abandonment in Maternity Homes
It is the social responsibility of the state to protect and prevent children from being abandoned. When a newborn is abandoned in a hospital maternity ward upon birth, the state is liable for providing foster care or placing the infant in a social institution. One of the most important activities for the implementation of state policy in the field of public child care is preventing child abandonment and placement outside the family, with the goal of reducing the number of children raised outside their normal family environment and ensuring their right to grow up in their biological or extended family. Abandonment prevention activities are organized by the Social Assistance Directorate as part of the Child Protection Department (CPD). This is the state’s structure for social assistance for children.A significant part of abandonment prevention activities should begin with the birth of a child at risk of being abandoned and placed in an institution. The role and function of the medical institution in recognizing the risk of abandonment and advising the appropriate protection authorities to take action against the infant's family are of great importance. Although medical specialists are working hard to prevent child abandonment at maternity homes, there are still many infants who end up in social institutions.The aim of the study is to infer the role of the medical institution in identifying the risk of abandonment and the follow-up measures for referral of such children to special institutions to ensure the well-being of the child. For survey purposes, documents pertaining to 49 abandonment cases in SHOGAT Varna from January 2020 to May 2022 were examined
THE ROLE OF PRACTICAL TRAINING IN A REAL WORKING ENVIRONMENT FOR MIDWIFERY STUDENTS
The Clinical bases enable the students from the regulated specialties in the field of healthcare (medical students, health care and professional Bachelor) to form their professional skills by working in a real hospital environment. Involving work in departments and sectors builds in students clinical thinking, forms communication skills, develops skills and models the organization of work tasks. This is an important process for forming professional skills in future healthcare professionals and preparing them for the challenges in the work process.The purpose of this article is to present a study among students in the specialty "midwife", regarding their motivation for training in clinical bases and also to reveal opportunities to increase their engagement and readiness for realization, through direct involvement in the work process. A survey was carried out among 136 students from the „midwife“ in Varna, Sliven and Pleven in Bulgaria. The methods used are documentary, sociological (direct anonymous survey) and statistical (descriptive statistics and correlation analysis).The results show that 71% of respondents rated communication with mentors in clinical practice as very good. As main problems of practical training, they point to vague tasks (90.4%), lack of feedback with healthcare professionals (86%), insufficient teamwork in the work (79.4%), tense situation due to the new way of Organization of the work process in e-health (36.8%). The desire of the students is to have more opportunity to consolidate the learned (10%), greater autonomy in the performance of activities (58%) and real participation in obstetric work (30%). Only 36.8% of essay students take advantage of the opportunity to work on an employment contract during their studies in clinical bases.In conclusion, the possibility of real involvement in work tasks, in working relations with the bases, allows for good preparation and absorption of a burning volume of knowledge and skills in a real working environment. This enables students to acquire practical skills that will give them the necessary confidence and professional self-esteem
Training In The Workplace - A Prerequisite For Good Adaptation To Practice The Profession In Hospital Conditions

Тъй като целите на обучението определят желания краен резултат, водейки се от предварително зададеното изискване към образователната подготовка на учащите се, те служат за формулиране на компонентите на професионалните компетенции като комплекс от знания, умения, отношения и личностни качества, които трябва всеки учащ се да придобие и усвои до края на обучението си. Обучението на работното място като форма на продължаващо развитие на медицинските работници е важен фактор за повишаване на качеството на здравните грижи и предлаганата медицинска помощ. Обучението продължава и по време на професионалната реализация и способства адаптацията и израстването в професията.Students from medical universities acquire knowledge and skills during the period of their training, as set out in the curricula, according to the state requirements for acquiring the profession. Since the learning objectives determine the desired end result, using the pre-set requirement for learners' education as a basis, they serve to formulate the components of professional competences as a complex of knowledge, skills, relationships and personal qualities that each learner needs to acquire and master until the end of training. On-the-job training, as a form of continuing development of medical workers, is an important factor in improving the quality of health care and medical assistance. Основните задачи на акушерката се отнасят до участие в решаването на проблеми, свързани с възпроизводството на населението, медико-социални въпроси на брака и семейството, социално-правни проблеми на бременната, майката и детето. Акушерките се реализират професионално в специализирани болници по акушерство и гинекология, акушеро-гинекологични отделения на други болници, акушеро-гинекологични кабинети в извънболничната помощ, кабинети по стерилитет, кабинети по медико-генетични консултации, кабинети за семейно планиране, кабинети за социално-правна помощ на майчинството и детството, научни изследователски институти. Midwives are professionally employed in specialized hospitals of obstetrics and gynecology, obstetric gynecology departments of other hospitals, obstetric and gynecological offices in outpatient care, sterility consultation offices, consultation offices for medical genetic counseling, family planning offices, maternity and childhood social care offices, scientific research institutes. The midwife can organize and manage health structures to promote health, health prophylaxis, health education and training for pregnant women, nurses and their families, parents' schools and nursing schools
Infection Prevention Associated with Medical Care in Cesarean Section
Public health encompasses a wide range of opportunities, including the possibility to influence and manage infectious agents, to influence the infectious process, and to implement anti-epidemic measures at the local and/or general structural levels. With particular emphasis are the activities related to the protection of maternal and child health at the maternity home level, as well as the care related to them until their discharge. In the conditions of a global pandemic, a number of events related to the protection of personal health, as well as activities at the level of medical institutions for strict disinfection and behavior protocols, were imposed. The volume of invasive treatment and diagnostic manipulations has increased, as has the diversity of patients and pathologies in the process of hospitalization and subsequent specific medical care. The subject of the study is the maternity ward at SHOGAT "Prof. Dr. D. Stamatov," Varna.The aim of the study is to define the prevention of infections associated with medical care in the context of an increased epidemiological risk. Results: In the period 2020–2021, 6,599 patients passed through the maternity ward at SHOGAT Varna, accounting for 54.76% of all patients at the hospital. In the years analyzed, there were no registered hospital infections (RHI) or infections related to medical care (IRMC), as well as incidents when dealing with sharp objects. A total of 18,201 microbiological studies were conducted on patients and the working environment in the hospital. All positive results for improved IRMC/RHI registration were operationally discussed at clinical-epidemiological and laboratory meetings. Infections related to medical care (IRMC) play a leading role in the organization of activities in the maternity ward and related structures (maternity and operating rooms, intensive care units, postpartum sectors, etc.), as well as with the accompanying human factor problems and in conducting hygiene and disinfection events

The development of professional competencies is a prerequisite for the successful implementation of current and future specialists. With its help, mentors have the opportunity to acquire knowledge, skills and experience, receive advice and support, analyze their thoughts, feelings and experiences of patients in order to face the challenges of the professional environment and achieve changes and development in relation to themselves and professional responsibility. Significant in this direction is the attitude of medical specialists who provide obstetric care to conducting supervision in the real working environment of a hospital institution.
