Community deworming alleviates geohelminth-induced immune hyporesponsiveness
In cross-sectional studies, chronic helminth infections have been associated with immunological hyporesponsiveness that can affect responses to unrelated antigens. To study the immunological effects of deworming, we conducted a cluster-randomized, double-blind, placebo-controlled trial in Indonesia and assigned 954 households to receive albendazole or placebo once every 3 mo for 2 y. Helminth-specific and nonspecific whole-blood cytokine responses were assessed in 1,059 subjects of all ages, whereas phenotyping of regulatory molecules was undertaken in 121 school-aged children. All measurements were performed before and at 9 and 21 mo after initiation of treatment. Anthelmintic treatment resulted in significant increases in proinflammatory cytokine responses to Plasmodium falciparum-infected red blood cells (PfRBCs) and mitogen, with the largest effect on TNF responses to PfRBCs at 9 mo—estimate [95% confidence interval], 0.37 [0.21–0.53], P value over time (Ptime) < 0.0001. Although the frequency of regulatory T cells did not change after treatment, there was a significant decline in the expression of the inhibitory molecule cytotoxic T lymphocyte-associated antigen 4 (CTLA-4) on CD4+ T cells of albendazole-treated individuals, –0.060 [–0.107 to –0.013] and –0.057 [–0.105 to –0.008] at 9 and 21 mo, respectively; Ptime = 0.017. This trial shows the capacity of helminths to up-regulate inhibitory molecules and to suppress proinflammatory immune responses in humans. This could help to explain the inferior immunological responses to vaccines and lower prevalence of inflammatory diseases in low- compared with high-income countries
Quaternary evolution of the northern North Sea margin through glacigenic debris-flow and contourite deposition
The Norwegian Channel Ice Stream of the Scandinavian Ice Sheet extended across the northern North Sea margin during the mid- to late Quaternary, eroding older sediment from the continental shelf. Consequently, little is known about early Quaternary sedimentation on this margin. We use two- and three-dimensional seismic-reflection data to investigate changing sediment volumes and sources in the northern North Sea through the Quaternary. The northern North Sea Basin was infilled during the early Quaternary by intercalated glacigenic debris-flows and contourites, which provide a record of the delivery of glacigenic sediment to the slope and the intensity of North Atlantic thermohaline circulation during early Quaternary glacial-interglacial cycles. The infilling of the basin reduced accommodation and led to the deflection of mid- to late Quaternary sediments into the Norwegian Sea, forming the North Sea Fan. Close to the onset of the mid-Quaternary, the south-western Scandinavian Ice Sheet margin was drained by an ice stream located beneath Måløy Plateau, 60 km east of the Last Glacial Maximum Norwegian Channel Ice Stream. The southward-flowing Norwegian Sea Bottom Water current was directed into the partially filled northern North Sea Basin during the early Quaternary, and deflected progressively northwards as the basin became infilled.During this work, C. Acute and Chronic Musculoskeletal Injury in Para Sport: A Critical Review
Seated Para athletes sustain upper extremity injuries more commonly, whereas ambulant Para athletes frequently sustain lower extremity injuries. The upper extremity is the most commonly injured area in all Para athletes, unlike ablebodied athletes for whom lower extremity injuries predominate. Minor soft tissue injuries are the most common injuries among Para athletes, similar to injury patterns observed among able-bodied athletes. Football 5-a-side, powerlifting, Goalball, Wheelchair fencing, and Wheelchair rugby are the highest risk summer sports; ice hockey, alpine skiing, and snowboarding are the highest-risk winter Paralympic sports. Compared with elite Para athletes, recreational and youth Para athletes remain understudied in the literatur
Down-regulation of specific antigen-driven cytokine production in a population with endemic Schistosoma japonicum infection.
Schistosome antigen-driven cytokine responses and antischistosome antibody levels of residents of a Schistosoma japonicum endemic island in Poyang Lake, Jiangxi Province were studied before and 45 days after treatment with praziquantel. IL-4, IL-5, IL-10 and INF-gamma were all detected in the supernatants of whole-blood cultures after stimulation with schistosome soluble egg antigen (SEA) and soluble worm antigen preparation (SWAP). The percentages of subjects producing detectable amounts of each cytokine assayed were higher in the group who were negative by stool examination at the start of the study than in those who were initially stool positive. After praziquantel treatment the percentages of subjects producing both type I and type II cytokines increased. This suggests that the production of both types of cytokine was down-regulated in the presence of live, egg-laying S. japonicum adult worms but that this was reversible by treatment. In contrast, the antibody studies showed higher levels of SWAP and SEA-specific antibodies (IgE, total IgG, IgG4, IgM) in subjects who were originally stool-positive than in those who were stool-negative. After treatment specific IgE responses were elevated, but total IgG and IgG4 anti-SEA and IgM anti-SWAP antibody levels all fell significantly
Regulatory T Cells in Human Lymphatic Filariasis: Stronger Functional Activity in Microfilaremics
Infection with filarial parasites is associated with T cell hyporesponsiveness, which is thought to be partly mediated by their ability to induce regulatory T cells (Tregs) during human infections. This study investigates the functional capacity of Tregs from different groups of filarial patients to suppress filaria-specific immune responses during human filariasis. Microfilaremic (MF), chronic pathology (CP) and uninfected endemic normal (EN) individuals were selected in an area endemic for Brugia timori in Flores island, Indonesia. PBMC were isolated, CD4CD25hi cells were magnetically depleted and in vitro cytokine production and proliferation in response to B. malayi adult worm antigen (BmA) were determined in total and Treg-depleted PBMC. In MF subjects BmA-specific T and B lymphocyte proliferation as well as IFN-gamma, IL-13 and IL-17 responses were lower compared to EN and CP groups. Depletion of Tregs restored T cell as well as B cell proliferation in MF-positives, while proliferative responses in the other groups were not enhanced. BmA-induced IL-13 production was increased after Treg removal in MF-positives only. Thus, filaria-associated Tregs were demonstrated to be functional in suppressing proliferation and possibly Th2 cytokine responses to BmA. These suppressive effects were only observed in the MF group and not in EN or CP. These findings may be important when considering strategies for filarial treatment and the targeted prevention of filaria-induced lymphedema
Streptomyces from traditional medicine: sources of new innovations in antibiotic discovery
Given the increased reporting of multi-resistant bacteria and the shortage of newly approved medicines, researchers have been looking towards extreme and unusual environments as a new source of antibiotics. Streptomyces currently provides many of the world’s clinical antibiotics, so it comes as no surprise that these bacteria have recently been isolated from traditional medicine. Given the wide array of traditional medicines, it is hoped that these discoveries can provide the much sought after core structure diversity that will be required of a new generation of antibiotics. This review discusses the contribution of Streptomyces to antibiotics and the potential of newly discovered species in traditional medicine. We also explore how knowledge of traditional medicines can aid current initiatives in sourcing new and chemically diverse antibiotics
