Osteolytic mystery: A rare case of pathologic fracture from a phosphaturic mesenchymal tumor in hip and femur.
Phosphaturic mesenchymal tumor (PMT) is a rare tumor causing bone complications and myopathy. Histologically, PMT displays a mix of spindled cells, osteoclast-like giant cells, basophilic matrix, and flocculent or grungy calcification. Here we describe a case of PMT in the right hip and proximal femur, initially suspected to be multiple myeloma, presenting with osteolytic lesions and elevated alkaline phosphatase. Tests for malignancy were negative, but a subsequent biopsy confirmed PMT. The patient underwent hip biopsy, femur resection, and hemiarthroplasty, with follow-up MRI recommended

A synchronous lesion: Papillary renal cell carcinoma mistaken as an adrenal gland mass.
In this case report, we describe a diagnosis of papillary renal cell carcinoma in a 76-year-old male patient who was incidentally found to have a left adrenal mass during routine aneurysm surveillance. Computed tomography demonstrated a left adrenal mass and left renal structure which was concerning for renal cell carcinoma. He underwent left adrenalectomy and initial histopathology demonstrated papillary renal cell carcinoma. He subsequently underwent left radical nephrectomy with lymph node dissection. Histopathological analysis of the removed left renal and nodal specimens revealed papillary renal cell carcinoma with lymph node metastasis. However, re-review of the adrenal pathology slides determined the specimen as represented by primary kidney tumor and not adrenal metastasis. First results from Faint Infrared Grism Survey (FIGS): first simultaneous detection of Lyman-alpha emission and Lyman break from a galaxy at z=7.51
Galaxies at high redshifts provide a valuable tool to study cosmic dawn, and
therefore it is crucial to reliably identify these galaxies. Here, we present
an unambiguous and first simultaneous detection of both the Lyman-alpha
emission and the Lyman break from a z = 7.512+/- 0.004 galaxy, observed in the
Faint Infrared Grism Survey (FIGS). These spectra, taken with G102 grism on
Hubble Space Telescope (HST), show a significant emission line detection (6
sigma) in multiple observational position angles (PA), with total integrated
Ly{\alpha} line flux of 1.06+/- 0.12 e10-17erg s-1cm-2. The line flux is nearly
a factor of four higher than the previous MOSFIRE spectroscopic observations of
faint Ly{\alpha} emission at {\lambda} = 1.0347{\mu}m, yielding z = 7.5078+/-
0.0004. This is consistent with other recent observations implying that
ground-based near-infrared spectroscopy underestimates total emission line
fluxes, and if confirmed, can have strong implications for reionization studies
that are based on ground-based Lyman-{\alpha} measurements. A 4-{\sigma}
detection of the NV line in one PA also suggests a weak Active Galactic Nucleus
(AGN), potentially making this source the highest-redshift AGN yet found. Thus,
this observation from the Hubble Space Telescope clearly demonstrates the
sensitivity of the FIGS survey, and the capability of grism spectroscopy to

Stellar Populations of Lyman Break Galaxies at z approx. to 1-3 in the HST/WFC3 Early Release Science Observations
We analyze the spectral energy distributions (SEDs) of Lyman break galaxies . (LBGs) at z approx = 1-3 selected using the Hubble Space Telescope (HST) Wide Field Camera 3 (WFC3) UVIS channel filters. These HST /WFC3 obse,rvations cover about 50 arcmin2 in the GOODS-South field as a part of the WFC3 Early Release Science program. These LBGs at z approx = 1-3 are selected using dropout selection criteria similar to high redshift LBGs. The deep multi-band photometry in this field is used to identify best-fit SED models, from which we infer the following results: (1) the photometric redshift estimate of these dropout selected LBGs is accurate to within few percent; (2) the UV spectral slope f3 is redder than at high redshift (z > 3), where LBGs are less dusty; (3) on average, LBGs at .z approx = 1-3 are massive, dustier and more highly star-forming, compared to LBGs at higher redshifts with similar luminosities, though their median values are similar within 1a uncertainties. This could imply that identical dropout selection technique, at all. redshifts, find physically similar galaxies; and (4) the stellar masses of these LBGs are directly proportional to their UV luminosities with a logarithmic slope of approx 0.46, and star-formation rates are proportional to their stellar masses with a logarithmic slope of approx 0.90. These relations hold true - within luminosities probed in this study - for LBGs from z approx = 1.5 to 5. The star-forming galaxies selected using other color-based techniques show similar correlations at z approx = 2, but to avoid any selection biases, and for direct comparison with LBGs at z > 3, a true Lyman break selection at z approx = 2 is essential. The future HST UV surveys,. both wider and deeper, covering a large luminosity range are important to better understand LBG properties, and their evolution

Incidental solitary fibrous tumor involving the seminal vesicle: A case report.
Solitary fibrous tumors are rare mesenchymal neoplasms that can range from slow-growing to aggressive tumors. This report presents a unique case of a young male patient with a solitary fibrous tumor involving the seminal vesicle, a rare location, and reinforces incidental discovery of these tumors on imaging and physical exams. Detection of these tumors is imperative to identify and treat malignancy. In our case, a 39-year-old previously healthy Asian male presents to the emergency department as a trauma admission post bicycle crash and is incidentally found to have a pelvic mass on computed tomography imaging of the pelvis. The patient underwent trans-anal biopsy which showed spindle epithelioid cells positive for CD34 and STAT6 markers, with a morphological and immunohistochemical profile consistent with a solitary fibrous tumor. The patient underwent surgery with a robotic-assisted laparoscopic pelvic mass resection and now follows up annually with imaging for observation

Undifferentiated carcinoma of the pancreas with osteoclast-like giant cells: a case report.
BACKGROUND: Undifferentiated carcinomas of the pancreas with osteoclast-like giant cells (UCPOGC) are rare pancreatic neoplasms that account for less than 1% of all pancreatic malignancies. This case report of a 54-year-old male with metastatic UCPOGC adds to the existing literature and further ascertains the clinical and imaging features, treatment options, and prognosis of this rare entity. CASE PRESENTATION: We present the detailed clinical course of a 54-year-old Asian male patient with UCPOGC, with focus on the relevant clinical features and imaging findings that are characteristic of this disease entity. CONCLUSIONS: UCPOGC is an extremely rare pancreatic tumor with a unique histopathology and clinical course. It is often difficult to distinguish UCPOGCs from other pancreatic tumors, such as traditional pancreatic ductal adenocarcinomas (PDAC), on imaging, and it therefore remains a pathological diagnosis. Surgery is generally regarded as the first-line treatment option, and the roles of chemotherapy and radiation are unclear. Due to the exceeding rarity of this tumor, large-scale clinical studies are not feasible. Therefore, it is important to share individual insights and experiences to improve our understanding and care for patients with this devastating disease
Short, Enantioselective Synthesis of Mevalonic Acid
A new enantioselective synthesis of mevalonic acid that is short, flexible, scalable to gram amounts, and uses readily available starting materials is reported. Enantioselective homoallylic epoxidation of isoprenol followed by enantiomeric enrichment gave the key epoxide intermediate. Further reactions led to either racemic mevalonic acid in 74% yield over two steps or the individual (R)- or (S)-enantiomer on a gram scale in 51% overall yield and ≥99% ee over five linear steps
Highlighting the Importance of a Comprehensive Workup for Altered Mental Status on the Detox Floor: A Case Report
miR-155 promotes T reg cell development by safeguarding medullary thymic epithelial cell maturation.
During thymocyte development, medullary thymic epithelial cells (mTECs) provide appropriate instructive cues in the thymic microenvironment for not only negative selection but also the generation of regulatory T (T reg) cells. Here, we identify that miR-155, a microRNA whose expression in T reg cells has previously been shown to be crucial for their development and homeostasis, also contributes to thymic T reg (tT reg) cell differentiation by promoting mTEC maturation. Mechanistically, we show that RANKL stimulation induces expression of miR-155 to safeguard the thymic medulla through targeting multiple known and previously uncharacterized molecules within the TGFβ signaling pathway, which is recognized for its role in restricting the maturation and expansion of mTECs. Our work uncovers a miR-155-TGFβ axis in the thymic medulla to determine mTEC maturity and, consequently, the quantity of tT reg cells and suggests that miR-155 ensures proper tT reg cell development in both cell-intrinsic and -extrinsic manners
