    Chory, lat 43 u kt贸rego na podstawie kryteri贸w klinicznych rozpoznano nerwiakow艂贸kniakowato艣膰 typu 1 (NF1, choroba van Recklinghausena), zosta艂 skierowany na oddzia艂 chor贸b p艂uc w celu przeprowadzenia diagnostyki przyczyn pogorszenia tolerancji wysi艂ku fizycznego oraz etiologii torbielowatych zmian w p艂ucach uwidocznionych w tomografii komputerowej wysokiej rozdzielczo艣ci (HRCT). Od dzieci艅stwa pacjent by艂 leczony z powodu padaczki, rozpoznano tak偶e niedomykalno艣膰 zastawki tr贸jdzielnej III stopnia, nie stwierdzaj膮c nadci艣nienia p艂ucnego w trakcie cewnikowania prawego serca. Ostatecznie przyj臋to, 偶e w omawianym przypadku zmiany 艣r贸dmi膮偶szowe w p艂ucach s膮 zwi膮zane z chorob膮 zasadnicz膮 i wymagaj膮 dalszej obserwacji klinicznej. W doniesieniu om贸wiono kryteria rozpoznania NF1, a tak偶e kontrowersje dotycz膮ce wsp贸艂istnienia 艣r贸dmi膮偶szowych zmian p艂ucnych w obrazie choroby.A case of a 43-year-old man with clinically diagnosed neurofibromatosis type I (NF-1, von Recklinghausen disease), was referred to a lung disease unit in order to diagnosis of worsening tolerance to physical effort, and aetiology of radiological cystic lesions in the lungs, seen in the high-resolution computed tomography (HRCT). Since childhood the patient has been treated for epilepsy, and a 3rd degree tricuspid valve incompetence, without pulmonary hypertension was detected during right heart catheterization. Finally, the interstitial pulmonary lesions were attributed to the primary disease, and it was said they need further clinical observation in order to determine their dynamics. The observed deterioration in patient’s tolerance to physical effort was connected to the accompanying infection of the respiratory system with Klebsiella oxytoca and Staphylococcus aureus, with cystic lesions in lungs and tricuspid valve incompetence. The report describes the criteria for NF-1 diagnosis, as well as points out the controversies of coexistence of interstitial pulmonary lesions in the clinical picture of the disease

    Nursing care for a patient after an ischemic stroke

    Introduction and aim of the study. More than half of ischemic strokes occur above the age of 65, so a stroke is above all a disease of an old age. The problem of a stroke is crucial, because apart from a high mortality rate it also entails a disability. The aim of the study was to determine the scope of nursing care for the patient after an ischemic stroke. Methods and materials. The study was based on the case study method with the use of the following research techniques: documentation analysis, an interview, measurement and observation. Moreover, the study tools applied comprised: an individualized nursing care plan, Barthel Scale, Glasgow Scale, Dutch Scale, the authors鈥 own test examining the patient鈥檚 knowledge. Criteria for the care categories. Findings. After the interview, the biopsychosocial status of the patient was assessed. Nursing diagnoses were made using the empirical data for this purpose. Conclusions. The sudden occurrence of the disease, hospitalization and lack of support from relatives is a difficult and critical situation for the patient, and it disturbs normal functioning in all spheres: biological, psychological and a social one

W wyniku przeprowadzonych bada艅 u obojga chorych stwierdzono brak wydzielania interleukiny 12 (IL-12) i interferonu γ (IFN-γ) przez stymulowane limfocyty. Opisywany wcze艣niej defekt genetyczny warunkuje wyst臋powanie wra偶liwo艣ci typu Mendla na zaka偶enie pr膮tkami (MSMD). Pacjenci spe艂niali kryteria kliniczno-radiologiczne i bakteriologiczne rozpoznania mykobakteriozy opracowane przez American Thoracic Society w 2007 roku. Po 13 miesi膮cach typowego leczenia uzyskano ust膮pienie dolegliwo艣ci, remisj臋 radiologiczn膮 oraz utrzymuj膮ce si臋 przez 12 miesi臋cy odpr膮tkowanie. Rokowanie pacjent贸w pozostaje niepewne. Genetycznie uwarunkowana sk艂onno艣膰 do zaka偶e艅 pr膮tkami mo偶e spowodowa膰 wznow臋 choroby lub by膰 przyczyn膮 nowego zachorowania na choroby wywo艂ane przez pr膮tki. Pneumonol. Alergol. Pol. 2011; 79, 6: 428–436Two previously healthy siblings were diagnosed with pulmonary mycobacteriosis caused by M. kansasii. During examination both patients were diagnosed with an interleukin 12 (IL-12) and interferon γ (IFN-γ) production disorder of the stimulated lymphocytes. The given genetic defect conditions the occurrence of the Mendelian susceptibility to mycobacterial infection (MSMD). The patients fulfilled clinical, radiological, and bacteriological criteria for diagnosis of mycobacteriosis laid out by American Thoracic Society in 2007. After 13 months of standard treatment the ailments receded, and radiological remission, as well as a 12-month-lasting sputum negativity was achieved. The prognosis for the patients remains uncertain. The genetic conditioning to mycobacterial infections may cause disease recurrences or other mycobacterial illnesses. The patients will need to be checked systematically by pulmonologist. It is not known whether the offspring of the patients are exposed to general Baccillus Calmette-Gu茅rin infection due to the compulsory vaccinations against tuberculosis, and whether the risk of complications is higher than the potential risk of coming down with hematogenous TB in childhood. The patient received antibiotics, however, due to recurrent pericardial effusion a pericardial drainage was required.     Dedicated control and measurement system for bioreactors to study the composting process

    During the composting process, waste biomass with high moisture content undergoes various transformation in the presence of oxygen. The composting process is analyzed in dedicated bioreactors which are air-tight facilities with external air supply. Subject to the type of composted plant material, biomass should be periodically turned to promote even aeration. The following information is required to build a model of the composting process: oxygen (air) uptake, moisture content of exhaust gas, production of carbon dioxide, ammonia and other gases in the composting process, and temperature distribution inside the bioreactor. A temperature monitoring system for a bioreactor is difficult to build due to challenging operating conditions including the airtight structure of a bioreactor, high moisture content, the operation of temperature sensors in a highly aggressive environment, problems with uninterrupted power supply for the monitoring system in a bioreactor. This article presents a patented temperature monitoring system for a bioreactor. The system鈥檚 design and structure are discussed, and recommendations for functional improvements are made

    Evaluation of selected APIs for emotion recognition from facial expressions

    Facial expressions convey the vast majority of the emotional information contained in social utterances. From the point of view of affective intelligent systems, it is therefore important to develop appropriate emotion recognition models based on facial images. As a result of the high interest of the research and industrial community in this problem, many ready-to-use tools are being developed, which can be used via suitable web APIs. In this paper, two of the most popular APIs were tested: Microsoft Face API and Kairos Emotion Analysis API. The evaluation was performed on images representing 8 emotions鈥攁nger, contempt, disgust, fear, joy, sadness, surprise and neutral鈥攄istributed in 4 benchmark datasets: Cohn-Kanade (CK), Extended Cohn-Kanade (CK+), Amsterdam Dynamic Facial Expression Set (ADFES) and Radboud Faces Database (RaFD). The results indicated a significant advantage of the Microsoft API in the accuracy of emotion recognition both in photos taken en face and at a 45鈭 angle. Microsoft鈥檚 API also has an advantage in the larger number of recognised emotions: contempt and neutral are also included
