Parental Perceptions of Child Life Specialists
Most children spend time in the hospital at some point during their childhood whether it is for a short visit or a long-term stay. Hospitalization can be a very stressful experience for children and their families. Child life specialists work in hospitals to help families navigate hospitalizations and cope with stress. This study aimed to investigate parental perceptions of child life specialists, including parents’ familiarity with their role, recognition of its importance, appreciation for help provided, and the formation of meaningful relationships with child life specialists. A survey was conducted among child life specialists who had worked with children and families in hospital settings, and ninety-one responses were collected. The findings indicate that parents were generally unfamiliar with the role of a child life specialist before entering the hospital. However, after interacting with a child life specialist, parents came to recognize the importance and value of the child life specialists’ role and appreciated the help that they provided for their child and family. Child life specialists also feel that they can form meaningful relationships with the parents they work with. Overall, the study emphasizes the crucial role that child life specialists play in supporting children and families throughout hospitalization

Epidemiology of surgery associated acute kidney injury (EPIS-AKI): a prospective international observational multi-center clinical study
Purpose: The incidence, patient features, risk factors and outcomes of surgery-associated postoperative acute kidney injury (PO-AKI) across different countries and health care systems is unclear. Methods: We conducted an international prospective, observational, multi-center study in 30 countries in patients undergoing major surgery (> 2-h duration and postoperative intensive care unit (ICU) or high dependency unit admission). The primary endpoint was the occurrence of PO-AKI within 72 h of surgery defined by the Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Secondary endpoints included PO-AKI severity and duration, use of renal replacement therapy (RRT), mortality, and ICU and hospital length of stay. Results: We studied 10,568 patients and 1945 (18.4%) developed PO-AKI (1236 (63.5%) KDIGO stage 1500 (25.7%) KDIGO stage 2209 (10.7%) KDIGO stage 3). In 33.8% PO-AKI was persistent, and 170/1945 (8.7%) of patients with PO-AKI received RRT in the ICU. Patients with PO-AKI had greater ICU (6.3% vs. 0.7%) and hospital (8.6% vs. 1.4%) mortality, and longer ICU (median 2 (Q1-Q3, 1-3) days vs. 3 (Q1-Q3, 1-6) days) and hospital length of stay (median 14 (Q1-Q3, 9-24) days vs. 10 (Q1-Q3, 7-17) days). Risk factors for PO-AKI included older age, comorbidities (hypertension, diabetes, chronic kidney disease), type, duration and urgency of surgery as well as intraoperative vasopressors, and aminoglycosides administration. Conclusion: In a comprehensive multinational study, approximately one in five patients develop PO-AKI after major surgery. Increasing severity of PO-AKI is associated with a progressive increase in adverse outcomes. Our findings indicate that PO-AKI represents a significant burden for health care worldwide

On-Column Partition Cleanup of Fatty Extracts for Organophosphate Pesticide Residue Determination
Abstract
A fast, single-step, and efficient partition between n-hexane and acetonitrile on ready-to-use, disposable mini-columns of Kieselghur-type material has been developed for the cleanup of fatty extracts for organophosphate (OP) pesticide residue determination by gas chromatography with flame photometric detection. Nine OP pesticides (diazinon, etrimfos, chlorpyrifos-methyl, pyrimiphos-methyl, chlorpyrifos, bromophos, bromophos-ethyl, malathion, fenitrothion) most commonly used for protection of stored cereals, oil seeds, and legumes were separated from up to 2.0 g lipidic material with recoveries between 80 and 107% at spiking levels ranging for the different compounds from 0.1 to 5.0 ppm.</jats:p

Optimization of Potent and Specific Trypanothione Reductase Inhibitors: A Structure-Based Drug Discovery Approach
Leishmania spp. are responsible for up to 1 million new cases each year. The current therapeutic arsenal against Leishmania is largely inadequate, and there is an urgent need for better drugs. Trypanothione reductase (TR) represents a druggable target since it is essential for the parasite and not shared by the human host. Here, we report the optimization of a novel class of potent and selective LiTR inhibitors realized through a concerted effort involving X-ray crystallography, synthesis, structure-activity relationship (SAR) investigation, molecular modeling, and in vitro phenotypic assays. 5-Nitrothiophene-2-carboxamides 3, 6e, and 8 were among the most potent and selective TR inhibitors identified in this study. 6e and 8 displayed leishmanicidal activity in the low micromolar range coupled to SI > 50. Our studies could pave the way for the use of TR inhibitors not only against leishmaniasis but also against other trypanosomatidae due to the structural similarity of TR enzymes
