    Intranasal fentanyl versus fentanyl pectin nasal spray for the management of breakthrough cancer pain in doses proportional to basal opioid regimen.

    Abstract The aim of this randomized, crossover, comparison study was to assess the analgesic and adverse effects of 2 nasal preparations, intranasal fentanyl (INFS) and fentanyl pectin nasal spray (FPNS), for breakthrough pain, given in doses proportional to opioid basal regimen. Each patient randomly received INFS or FPNS in doses proportional to opioid dosages used for background analgesia for 2 pairs of episodes. For each episode of breakthrough pain, pain intensity and adverse effects intensity were recorded just before starting the INFS or FPNS (T0) and 5 minutes (T5), 10 minutes (T10), and 20 minutes (T20) after the administration of the nasal drugs. Sixty-nine patients were studied. The mean age was 63.4 years, and 37 patients were males. For the present analysis, 188 episodes were considered. A statistical decrease in pain intensity was observed with both nasal drugs after 5, 10, and 20 minutes. A decrease in pain intensity of >33% was observed in 16, 102, and 159 treated episodes at T5, T10, and T20, respectively. Adverse effects were of mild nature in most cases or were preexistent because of basal opioid therapy. No differences were found in summed pain intensity difference 20 minutes after dosing. Most of patients did not find substantial preferences. INFS and FPNS were effective and well-tolerated treatments for breakthrough pain management. Both delivery systems, in doses proportional to the basal opioid regimen, provided significant analgesia within 10 minutes, without producing relevant adverse effects. PERSPECTIVE: This article showed that INFS and FPNS in doses proportional to basal opioid regimen are equally safe and effective for the management of breakthrough pain in cancer patients.     Using b-tagging to enhance the SUSY reach of the CERN Large Hadron Collider

    Assuming that supersymmetry is realized with parameters in the hyperbolic branch/focus point (HB/FP) region of the minimal supergravity (mSUGRA) model, we show that by searching for multijet + missing E_T events with tagged b jets the reach of experiments at the LHC may be extended by as much as 20% from current projections. The reason for this is that gluino decays to third generation quarks are enhanced because the lightest neutralino has substantial higgsino components. Although we were motivated to perform this analysis because the HB/FP region is compatible with the recent determination of the relic density of cold dark matter, our considerations may well have a wider applicability since decays of gluinos to third generation quarks are favoured in a wide variety of models.Comment: 16 pages, 1 figur

    First Order Extended Gravity and the Dark Side of the Universe: the General Theory

    General Relativity is not the definitive theory of Gravitation due to several shortcomings which are coming out both from theoretical and experimental viewpoints. At large scales (astrophysical and cosmological scales) the attempts to match it with the today observational data lead to invoke Dark Energy and Dark Matter as the bulk components of the cosmic fluid. Since no final evidence, at fundamental level, exists for such ingredients, it is clear that General Relativity presents shortcomings at infrared scales. On the other hand, the attempts to formulate theories more general than the Einstein one give rise to mathematical difficulties that need workarounds which, in turn, generate problems from the interpretative viewpoint. We present here a completely new approach to the mathematical objects in terms of which a theory of Gravitation may be written in a first-order `a la Palatini formalism, and introduce the concept of Dark Metric which could completely bypass the introduction of disturbing concepts as Dark Energy and Dark Matter.Comment: Proceedings of the Conference "The Invisible Universe" Paris, June 29-July 3, 2009 10 page

    Calculable Sparticle Masses with Radiatively Driven Inverted Mass Hierarchy

    Supersymmetric models with an inverted mass hierarchy (IMH: multi-TeV first and second generation matter scalars, and sub-TeV third generation scalars) can ameliorate problems arising from flavor changing neutral currents, CPCP violating phases and electric dipole moments, while at the same time satisfying conditions on naturalness. It has recently been shown that such an IMH can be generated radiatively, making use of infra-red fixed point properties of renormalization group equations given Yukawa coupling unification and suitable GUTGUT scale boundary conditions on soft SUSY breaking masses. In these models, explicit spectra cannot be obtained due to problems implementing radiative electroweak symmetry breaking (REWSB). We show that use of SO(10) DD-term contributions to scalar masses can allow REWSB to occur, while maintaining much of the desired IMH.
