Effects of Grazing on the Behaviour, Oxidative and Immune Status, and Production of Organic Dairy Cows
Abstract: This study compared the eects of a short daily grazing time with those of permanent free-stall housing on the behaviour, oxidative status, immune response, and milk production of organically reared cows. During a 63-day period, two homogeneous groups of eight lactating Brown cows were allocated to either housing (H) in a free-stall building for 24 h/day. Feeding was based on a total mixed ration or grazing (G) on barley grass for 5 h/day, and housing in a free-stall structure with feeding was based on the same total mixed ration oered to the H group. With regard to behaviour, H cows spent more time idling, walking, drinking, and self-grooming, whereas G cows showed a greater intent to eat and interact socially. Moreover, G cows exhibited slightly higher reactive oxygen metabolites and similar biological antioxidant potential concentrations than the H group, which indicates that short grazing resulted in an almost negligible increase in oxidative stress and an unchanged antioxidant capacity. Skin tests, performed by injecting phytohemoagglutinin intradermally, indicated that G cows had thicker skin than H cows at the end of the trial, an index of a better cell-mediated immune response. Grazing did not aect milk yield but improved milk quality in terms of an increase in fat and a reduction in urea content, somatic cell count, and total microbial count. Milk from G cows was richer in saturated fatty acids, likely because of the contribution of palmitic acid present in the grazed barley grass, and also showed higher contents of some healthy fatty acids, such as rumenic acid and -linolenic acid, and a lower omega-6/omega-3 ratio. These results show that including a short grazing time in the diets of organic dairy cows does not have negative consequences for milk production and contributes to improved milk quality as well as to a more ecient immune response in the cow
ChatGPT-4 Consistency in Interpreting Laryngeal Clinical Images of Common Lesions and Disorders.
peer reviewed[en] OBJECTIVE: To investigate the consistency of Chatbot Generative Pretrained Transformer (ChatGPT)-4 in the analysis of clinical pictures of common laryngological conditions.
STUDY DESIGN: Prospective uncontrolled study.
SETTING: Multicenter study.
METHODS: Patient history and clinical videolaryngostroboscopic images were presented to ChatGPT-4 for differential diagnoses, management, and treatment(s). ChatGPT-4 responses were assessed by 3 blinded laryngologists with the artificial intelligence performance instrument (AIPI). The complexity of cases and the consistency between practitioners and ChatGPT-4 for interpreting clinical images were evaluated with a 5-point Likert Scale. The intraclass correlation coefficient (ICC) was used to measure the strength of interrater agreement.
RESULTS: Forty patients with a mean complexity score of 2.60 ± 1.15. were included. The mean consistency score for ChatGPT-4 image interpretation was 2.46 ± 1.42. ChatGPT-4 perfectly analyzed the clinical images in 6 cases (15%; 5/5), while the consistency between GPT-4 and judges was high in 5 cases (12.5%; 4/5). Judges reported an ICC of 0.965 for the consistency score (P = .001). ChatGPT-4 erroneously documented vocal fold irregularity (mass or lesion), glottic insufficiency, and vocal cord paralysis in 21 (52.5%), 2 (0.05%), and 5 (12.5%) cases, respectively. ChatGPT-4 and practitioners indicated 153 and 63 additional examinations, respectively (P = .001). The ChatGPT-4 primary diagnosis was correct in 20.0% to 25.0% of cases. The clinical image consistency score was significantly associated with the AIPI score (rs = 0.830; P = .001).
CONCLUSION: The ChatGPT-4 is more efficient in primary diagnosis, rather than in the image analysis, selecting the most adequate additional examinations and treatments
Presentation, radiologic features, and treatment options of congenital tongue tumors: a comprehensive review
AIM: Congenital tumors of the tongue are rare in pediatric patients but encompass a diverse range of entities. Each tumor type exhibits distinct clinical behaviors, necessitating a precise approach to differentiating the tumor types and a tailored, tumor-specific treatment regimen. Advanced imaging techniques, such as diffusion-weighted imaging and perfusion studies, play a vital role in differentiating benign and malignant tongue tumors. This review summarizes current knowledge regarding the presentation, imaging features, and treatment of congenital tongue tumors. METHODS: A literature review was conducted by searching studies on congenital tongue tumors in databases such as PubMed, Embase, Web of Science, and Scopus. Relevant data, such as clinical features, radiologic characteristics, treatment modalities, and outcomes for different tumor types, were extracted from the selected articles. RESULTS: Our literature review reveals the various entities of congenital tongue tumors, which can be categorized in terms of hereditary pattern, phenotype, and rarity. Congenital tongue tumors include a range of vascular malformations, such as hemangiomas, lymphatic malformations, arteriovenous malformations, and venous malformations. Another entity is represented by cystic lesions, including dermoid cysts, epidermoid cysts, ranulas, and mucous retention cysts. Rare malignant neoplasms include teratomas and rhabdomyosarcomas. These tumor types vary in terms of swelling, respiratory distress, or impaired oral function, depending on size and location. The detection of these tumors can be carried out using imaging modalities, such as ultrasound, magnetic resonance imaging, and computed tomography, which are utilized to facilitate diagnosis and differentiation. At present, surgical excision remains the cornerstone of treatment, while other modalities may be adopted, depending on tumor type and extent. The prognosis of congenital tongue tumors can be affected by tumor's site, size, involvement of vital structures, and malignancy. CONCLUSIONS: Given their diversity and complexity, congenital tongue tumors, albeit uncommon, require specialized clinical treatments tailored to each tumor type's characteristics. Understanding the variable presentations and imaging features enables accurate diagnosis, while customized treatment strategies are key to optimizing outcomes and minimizing morbidity in pediatric tongue tumors. We read the paper of Kim et al. entitled “Persistent subjective voice symptoms for two years after thyroidectomy” [1]. Authors observed that some voice quality parameters improved from pre-to post-treatment, while others deteriorated such as the Thyroidectomy-related Voice and Symptoms' Questionnaire scores. Voice abuse history in professional users was associated with worse vocal quality after thyroidectomy. We congratulate authors to have investigated this controversial topic through a prospective study. Post-thyroidectomy voice disorders are prevalent in otolaryngological consultations, but this topic is poorly investigated in the current literature [2]. In this letter, we would like to draw attention to an important condition that was not considered in the study, which is the impairment of the upper esophageal sphincter (UES) during the surgery
