    Attitudes and characteristics of health professionals working in Aboriginal health

    There is an unacceptable gap in health status between Aboriginal and non-Aboriginal people in Australia. Linked to social inequalities in health and political and historical marginalisation, this health gap must be urgently addressed. It is important that health professionals, the majority of whom in Australia are non-Aboriginal, are confident and equipped to work in Aboriginal health in order to contribute towards closing the health gap. The purpose of this study was to explore the attitudes and characteristics of non-Aboriginal health professionals working in Aboriginal health. Methods: The research was guided and informed by a social constructionist epistemology and a critical theoretical approach. It was set within a larger healthy eating and physical activity program delivered in one rural and one metropolitan community in South Australia from 2005 to 2010. Non-Aboriginal staff working in the health services where the program was delivered and who had some experience or an interest working in Aboriginal health were invited to participate in a semi-structured interview. Dietitians working across South Australia (rural and metropolitan locations) were also invited to participate in an interview. Data were coded into themes that recurred throughout the interview and this process was guided by critical social research. Results: Thirty-five non-Aboriginal health professionals participated in a semi-structured interview about their experiences working in Aboriginal health. The general attitudes and characteristics of non-Aboriginal health professionals were classified using four main groupings, ranging from a lack of practical knowledge (‘don’t know how’), a fear of practice (‘too scared’), the area of Aboriginal health perceived as too difficult (‘too hard’) and learning to practice regardless (‘barrier breaker’). Workers in each group had different characteristics including various levels of willingness to work in the area; various understandings of Australia’s historical relationship with Aboriginal peoples; varying awareness of their own cultural identity and influence on working with Aboriginal people; and different levels of (dis)comfort expressed in discussions about social, political and intercultural issues that impact on the healthcare encounter. Conclusions: These groupings can be used to assist non-Aboriginal health professionals to reflect on their own levels of confidence, attitudes, characteristics, experiences, approaches and assumptions to Aboriginal health, as an important precursor to further practice and development in Aboriginal health. By encouraging self-reflection of non-Aboriginal health professionals about where their experiences, characteristics and confidence lie, the groupings presented in this paper can be used to encourage non-Aboriginal health professionals, rather than Aboriginal clients or workers, to be the focus for change and deliver health care that is more acceptable to patients and clients, hence influencing health service delivery. The groupings presented can also begin to enable discussions between all health professionals about working together in Aboriginal health

    Correlator Convolutional Neural Networks: An Interpretable Architecture for Image-like Quantum Matter Data

    Machine learning models are a powerful theoretical tool for analyzing data from quantum simulators, in which results of experiments are sets of snapshots of many-body states. Recently, they have been successfully applied to distinguish between snapshots that can not be identified using traditional one and two point correlation functions. Thus far, the complexity of these models has inhibited new physical insights from this approach. Here, using a novel set of nonlinearities we develop a network architecture that discovers features in the data which are directly interpretable in terms of physical observables. In particular, our network can be understood as uncovering high-order correlators which significantly differ between the data studied. We demonstrate this new architecture on sets of simulated snapshots produced by two candidate theories approximating the doped Fermi-Hubbard model, which is realized in state-of-the art quantum gas microscopy experiments. From the trained networks, we uncover that the key distinguishing features are fourth-order spin-charge correlators, providing a means to compare experimental data to theoretical predictions. Our approach lends itself well to the construction of simple, end-to-end interpretable architectures and is applicable to arbitrary lattice data, thus paving the way for new physical insights from machine learning studies of experimental as well as numerical data.Comment: 7 pages, 4 figures + 13 pages of supplemental materia

    Development of Technical Specifications and Process System Requirements for the World’s Largest LH2 Refueling Station

    Hydrogen is emerging as one of the premising energy sources to achieve carbon neutral society. To efficiently store and make use of the produced hydrogen by various methods, liquid hydrogen and liquid hydrogen refueling station (LHRS) are spotlighted as solutions for storage and utilization sector respectively. However, technical limitations of cryogenic pumps, heat exchangers and unconsolidated standards for LHRS led to hinder the wide spread of the stations. This paper reviews general technical specifications and safety standards of commercialized small-scale LHRS and suggests technical specifications and safety standards for large-scale LHRS. Especially, an application of separated cryogenic pumps and heat integration for the vaporizer specializes the designed station charging 1000kg of hydrogen per day in this paper from small-scale LHRS and other large-scale LHRS. The authors expect commercializing large-scale LHRS with the proposed technical specification and process system requirements to be able and the safety standards set for the designed station can contribute to the unification of world LHRS standards

    Can adverse childhood experiences predict chronic health conditions? Development of trauma-informed, explainable machine learning models

    IntroductionDecades of research have established the association between adverse childhood experiences (ACEs) and adult onset of chronic diseases, influenced by health behaviors and social determinants of health (SDoH). Machine Learning (ML) is a powerful tool for computing these complex associations and accurately predicting chronic health conditions.MethodsUsing the 2021 Behavioral Risk Factor Surveillance Survey, we developed several ML models—random forest, logistic regression, support vector machine, Naïve Bayes, and K-Nearest Neighbor—over data from a sample of 52,268 respondents. We predicted 13 chronic health conditions based on ACE history, health behaviors, SDoH, and demographics. We further assessed each variable’s importance in outcome prediction for model interpretability. We evaluated model performance via the Area Under the Curve (AUC) score.ResultsWith the inclusion of data on ACEs, our models outperformed or demonstrated similar accuracies to existing models in the literature that used SDoH to predict health outcomes. The most accurate models predicted diabetes, pulmonary diseases, and heart attacks. The random forest model was the most effective for diabetes (AUC = 0.784) and heart attacks (AUC = 0.732), and the logistic regression model most accurately predicted pulmonary diseases (AUC = 0.753). The strongest predictors across models were age, ever monitored blood sugar or blood pressure, count of the monitoring behaviors for blood sugar or blood pressure, BMI, time of last cholesterol check, employment status, income, count of vaccines received, health insurance status, and total ACEs. A cumulative measure of ACEs was a stronger predictor than individual ACEs.DiscussionOur models can provide an interpretable, trauma-informed framework to identify and intervene with at-risk individuals early to prevent chronic health conditions and address their inequalities in the U.S

    Different routes for conifer- and sinapaldehyde and higher saccharification upon deficiency in the dehydrogenase CAD1

    In the search for renewable energy sources, genetic engineering is a promising strategy to improve plant cell wall composition for biofuel and bioproducts generation. Lignin is a major factor determining saccharification efficiency and, therefore, is a prime target to engineer. Here, lignin content and composition were modified in poplar (Populus tremula 3 Populus alba) by specifically down-regulating CINNAMYL ALCOHOL DEHYDROGENASE1 (CAD1) by a hairpin-RNA-mediated silencing approach, which resulted in only 5% residual CAD1 transcript abundance. These transgenic lines showed no biomass penalty despite a 10% reduction in Klason lignin content and severe shifts in lignin composition. Nuclear magnetic resonance spectroscopy and thioacidolysis revealed a strong increase (up to 20-fold) in sinapaldehyde incorporation into lignin, whereas coniferaldehyde was not increased markedly. Accordingly, ultra-high-performance liquid chromatography-mass spectrometry-based phenolic profiling revealed a more than 24,000-fold accumulation of a newly identified compound made from 8-8 coupling of two sinapaldehyde radicals. However, no additional cinnamaldehyde coupling products could be detected in the CAD1-deficient poplars. Instead, the transgenic lines accumulated a range of hydroxycinnamate-derived metabolites, of which the most prominent accumulation (over 8,500-fold) was observed for a compound that was identified by purification and nuclear magnetic resonance as syringyl lactic acid hexoside. Our data suggest that, upon down-regulation of CAD1, coniferaldehyde is converted into ferulic acid and derivatives, whereas sinapaldehyde is either oxidatively coupled into S'(8-8) S' and lignin or converted to sinapic acid and derivatives. The most prominent sink of the increased flux to hydroxycinnamates is syringyl lactic acid hexoside. Furthermore, low-extent saccharification assays, under different pretreatment conditions, showed strongly increased glucose (up to +81%) and xylose (up to +153%) release, suggesting that down-regulating CAD1 is a promising strategy for improving lignocellulosic biomass for the sugar platform industry

    Protocols for an Aboriginal-led, Multi-methods Study of the Role of Aboriginal and Torres Strait Islander Health Workers, Practitioners and Liaison Officers in Quality Acute Health Care

    ObjectivesAboriginal and Torres Strait Islander Health Workers/Practitioners and Liaison Officers play an important, often critical role providing advocacy and cultural and emotional support for Aboriginal and Torres Strait Islander patients. The main goals of this research are to explore i) how Aboriginal and Torres Strait Islander Health Workers/Practitioners and Liaison Officers are integrated in the routine delivery of care for Aboriginal and Torres Strait Islander peoples in hospital, and ii) how the role of Aboriginal and Torres Strait Islander Health Workers/Practitioners and Liaison Officers facilitates quality health outcomes. MethodsThis study is being conducted in three different hospitals using a multi-method approach including: yarning and Dadirri, patient journey mapping, survey and semi-structured interviews. Ethics approval has been provided from four ethics committees covering the three project sites in Australia (Adelaide, South Australia; Sydney, New South Wales and Alice Springs, Northern Territory). SignificanceThis study uses innovative methodology founded on the privileging of Aboriginal and Torres Strait Islander knowledges to collect Aboriginal and Torres Strait Islander perspectives and understand patient journeys within acute health care systems. This project is led by Aboriginal and Torres Strait Islander researchers and guided by the Project Steering Committee comprised of stakeholders. ImplicationsThere is limited research that explores quality acute care processes and the integration of Aboriginal and Torres Strait Islander Health Workers/Practitioners work within health care teams. This research will make a valuable contribution to understanding how hospital services can achieve quality acute health care experiences for Aboriginal and Torres Strait Islander People
