The Moral Obligation to Prioritize Research Into Deep Brain Stimulation Over Brain Lesioning Procedures for Severe Enduring Anorexia Nervosa
Deep Brain Stimulation is currently being investigated as an experimental treatment for patients suffering from treatment-refractory AN, with an increasing number of case reports and small-scale trials published. Although still at an exploratory and experimental stage, initial results have been promising. Despite the risks associated with an invasive neurosurgical procedure and the long-term implantation of a foreign body, DBS has a number of advantageous features for patients with SE-AN. Stimulation can be fine-tuned to the specific needs of the particular patient, is relatively reversible, and the technique also allows for the crucial issue of investigating and comparing the effects of different neural targets. However, at a time when DBS is emerging as a promising investigational treatment modality for AN, lesioning procedures in psychiatry are having a renaissance. Of concern it has been argued that the two kinds of interventions should instead be understood as rivaling, yet “mutually enriching paradigms” despite the fact that lesioning the brain is irreversible and there is no evidence base for an effective target in AN. We argue that lesioning procedures in AN are unethical at this stage of knowledge and seriously problematic for this patient group, for whom self-control is particularly central to wellbeing. They pose a greater risk of major harms that cannot justify ethical equipoise, despite the apparent superiority in reduced short term surgical harms and lower cost
Island Movements: Thinking with the Archipelago
Whether in Homer or Plato, Shakespeare or Huxley, throughout history, thinking about islands has shaped how we think about human nature and our place in the world. However, to date archipelagos have received far less attention. This is problematic because we live, increasingly, in a world of island-island movements and not static forms. Not only in the more obvious cases of the Caribbean, Hawaii or the Philippines but, as Stratford et al (2011) say, many ‘continental forms’ like Canada and Australia are in fact archipelagos composed of thousands of island movements. To this list we can add more manufactured archipelagos: wind turbine arrays, industrial oil and military constellations. The key question therefore arises: what does it mean to think with the archipelago? This paper argues firstly that archipelagic thinking denaturalizes the conceptual basis of space and place, and therefore engages ‘the spatial turn’ presently sweeping the social sciences and humanities. Secondly, such thinking highlights the trope of what I call ‘metamorphosis’, of the adaptation and transformation of material, cultural and political practices through island movements. In both cases, I argue that thinking with the archipelago requires an important shift in how we frame analysis and engagement
The Ethics of Deep Brain Stimulation for the Treatment of Anorexia Nervosa
There is preliminary evidence, from case reports and investigational studies, to suggest that Deep Brain Stimulation (DBS) could be used to treat some patients with Anorexia Nervosa (AN). Although this research is at an early stage, the invasive nature of the intervention and the vulnerability of the potential patients are such that anticipatory ethical analysis is warranted. In this paper, we first show how different treatment mechanisms raise different philosophical and ethical questions. We distinguish three potential mechanisms alluded to in the neuroscientific literature, relating to desire, control, and emotion, respectively. We explain why the precise nature of the mechanism has important implications for the patient’s autonomy and personal identity. In the second part of the paper, we consider practical dimensions of offering DBS to patients with AN in certain cases. We first discuss some limited circumstances where the mere offering of the intervention might be perceived as exerting a degree of coercive pressure that could serve to undermine the validity of the patient’s consent. Finally, we consider the implications of potential effects of DBS for the authenticity of the patient’s choice to continue using stimulation to ameliorate their condition
Comparing and Combining Lexicase Selection and Novelty Search
Lexicase selection and novelty search, two parent selection methods used in
evolutionary computation, emphasize exploring widely in the search space more
than traditional methods such as tournament selection. However, lexicase
selection is not explicitly driven to select for novelty in the population, and
novelty search suffers from lack of direction toward a goal, especially in
unconstrained, highly-dimensional spaces. We combine the strengths of lexicase
selection and novelty search by creating a novelty score for each test case,
and adding those novelty scores to the normal error values used in lexicase
selection. We use this new novelty-lexicase selection to solve automatic
program synthesis problems, and find it significantly outperforms both novelty
search and lexicase selection. Additionally, we find that novelty search has
very little success in the problem domain of program synthesis. We explore the
effects of each of these methods on population diversity and long-term problem
solving performance, and give evidence to support the hypothesis that
novelty-lexicase selection resists converging to local optima better than
lexicase selection
Neuro-stimulation, doping, and the spirit of sport
There is increasing interest in using neuro-stimulation devices to achieve an ergogenic effect in elite athletes. Although the World Anti-Doping Authority (WADA) does not currently prohibit neuro-stimulation techniques, a number of researchers have called on WADA to consider its position on this issue. Focusing on trans-cranial direct current stimulation (tDCS) as a case study of an imminent so-called ‘neuro-doping’ intervention, we argue that the emerging evidence suggests that tDCS may meet WADA’s own criteria (pertaining to safety, performance-enhancing effect, and incompatibility with the ‘spirit of sport’) for a method’s inclusion on its list of prohibited substances and methods. We begin by surveying WADA’s general approach to doping, and highlight important limitations to the current evidence base regarding the performance-enhancing effect of pharmacological doping substances. We then review the current evidence base for the safety and efficacy of tDCS, and argue that despite significant shortcomings, there may be sufficient evidence for WADA to consider prohibiting tDCS, in light of the comparable flaws in the evidence base for pharmacological doping substances. In the second half of the paper, we argue that the question of whether WADA ought to ban tDCS turns significantly on the question of whether it is compatible with the ‘spirit of sport’ criterion. We critique some of the previously published positions on this, and advocate our own sport-specific and application-specific approach. Despite these arguments, we finally conclude by suggesting that tDCS ought to be monitored rather than prohibited due to compelling non-ideal considerations

The child’s right to bodily integrity and autonomy: a conceptual analysis
It is widely accepted that children enjoy some form of a right to bodily integrity. However, there is little agreement about the precise nature and scope of this right. This paper offers a conceptual analysis of the child's right to bodily integrity, in order to further elucidate the relationship between the child's right to bodily integrity and considerations of autonomy. Following a discussion of Leif Wenar's work on the structure and justification of rights, I first explain how the adult's right to bodily integrity can be distilled into separate elements that may plausibly be justified by different moral considerations. In particular, I claim that this analysis suggests that whilst the adult's right to bodily integrity is not wholly reducible to bodily autonomy, autonomy nonetheless remains entwined with our understanding of this right in a number of ways. On the basis of this discussion, I go on to outline three important complexities that arise when we consider the child's right to bodily integrity, before particularly focusing on the question of how third parties should determine whether or not to perform a physical interference upon a child who lacks decision-making capacity. Here, I raise some objections to Earp and Mazor's recent attempts to answer this question, before briefly defending an ‘autonomy-based interests’ account of permissible interference, an account that shares in what I take to be the spirit, if not the precise letter, of these earlier views
