    Defined approach solutions for problems of the modern civilized market of intellectual property as one of the key conditions for innovative development and comprehensive modernization of the national economy. The integration of the domestic sector research and development in the global innovation system by harmonizing the legal framework in the field of science and innovation with the appropriate legal framework for Europe and the world will move to online monitoring system of innovation development of economy in accordance with the objectives of the National Strategy on Intellectual Property. The aim – to reveal the impact of intellectual property on the innovation way of development of regional economy. The study used scientific methods: a comparative analysis – for comparison of relevant policies, regulatory framework and regulatory agencies in the field of intellectual property in post-Soviet countries; expert analysis – to determine the contribution of the labor efficiency index "copyright industry" in GDP and the share of people employed in this sector for the countries studied; economic and statistical method – to evaluate the effectiveness implementation of state policy in the field of intellectual property; scientific approbation – to determine the main directions of international cooperation in the field of intellectual property; system approach and method of analogue – to determine an effective system of intellectual property protection. The offers on improvement of state regulation in this sphere to create a common economic space and the innovation infrastructure. Details based on a comparative analysis with the best world practices investigated performance indicators assess the impact of research and development and their implications for the formation of intellectual property as a condition of the economy of innovation development.     ДЛИТЕЛЬНЬНОЕ ПРИМЕНЕНИЕ БЕВАЦИЗУМАБА С ИНТЕРФЕРОНОМ-АЛЬФА В КАЧЕСТВЕ ПЕРВОЙ ЛИНИИ ТАРГЕТНОЙ ТЕРАПИИ РАСПРОСТРАНЕННОГО РАКА ПОЧКИ

    This article presents the results of our observations of the treatment of patients with metastatic kidney cancer. Bevacizumab is used in combination with interferon-alpha (IFN-a) as a first-line treatment. The medicament belongs to the new generation group of drugs that block angiogenesis.Aim: to determine the effectiveness and possible adverse effects of the treatment of renal cell carcinoma with the combination of bevacizumab and interferon-alpha.Materials and methods: treatment was performed in 22 patients with distant metastases of renal cell carcinoma. Patients received therapy in the mode of bevacizumab 10 mg/kg intravenously every 14 days, and IFN-a-2A 3 million units subcutaneously 3 times a week. Blood test, serum biochemistry, coagulogram and analysis of urine were monitored every 2 weeks. Computed tomography of the chest, abdomen and pelvis was performed every 3 months.Results: progression-free survival was 12 months, overall survival was 21 months (12-24 months). We identified adverse events during the treatment, in relation to which some patients had either temporarily cancel current treatment or to modify the dose (grade 3 (G3) was detected in 18% of patients).Conclusion: after 24 months of treatment 14 patients (63%) had disease progression, 5 (22%) patients had stabilization of the disease and 3 patients (15%) had partial response to treatment. Our results (overall survival, progression-free survival, rate of side effects) are consistent with results of the treatment described in world literature. Side effects of the treatment are associated primarily with the use of low doses of IFN-alpha and there were practically no side effects of high severity, requiring special treatment or cancel of treatment. It allows to recommend bevacizumab and interferon-alpha in low doses as first-line combination therapy. Prevention of adverse effects of treatment allows to continue the ongoing targeted therapy.В данной статье приводятся результаты собственных наблюдений лечения больных метастатическим  раком почки. В качестве первой линии терапии применяется бевацизумаб  в комбинации с интерфероном-альфа (ИНФ-а). Препарат относится к группе препаратов  нового поколения, блокирующих ангиогенез.Цель: определить эффективность и возможные нежелательные явления при проведении лечения почечно-клеточного рака комбинацией бевацизумаба  с интерфероном-а.Материалы и методы: лечение проведено 22 пациентам, имеющим отдаленные метастазы почечно-клеточного рака. Больным проводилась терапия в режиме бевацизумаб 10 мг/кг внутривенно капельно каждые 14 дней и ИНФ-а-2А по 3 млн ед подкожно 3 раза в неделю.  Каждые 2 недели проводился контроль общего анализа, биохимического анализа крови, коагулограмма и общего анализа мочи. Каждые 3 месяца проводилaсь компьютерная томография грудной клетки, брюшной полости и таза.Результаты: в ходе  лечения  выживаемость  без прогрессирования  составила 12 месяцев,  общая выживаемость – 21 месяц (12–24).  На фоне проводимого лечения нами были выявлены нежелательные явления, в связи с которыми части пациентов пришлось либо временно  отменить проводимое  лечение,  либо модифицировать дозу (3 степень (G3) выявлена у 18% больных).Заключение: через 24 месяца  у 14 (63%) больных в ходе лечения отмечено прогрессирование заболевания, стабилизация  у 5 (22%) и у 3 (15%) частичный ответ, наши результаты лечения соответствуют международным результатам лечения (цифры общей выживаемости и выживаемости без прогрессирования,  частоту побочных эффектов). Побочные эффекты проводимого лечения связаны в основном с применением ИФН-а в низких дозах и практически не вызывает побочных эффектов высоких степеней тяжести, требующих проведения специального лечения или отмены проводимого лечения, что позволяет рекомендовать проведение лечения в качестве первой линии терапии комбинацией бевацизумаба с интерфероном–а в малых дозах, профилактика нежелательных явлений лечения позволяет продолжать проводимую таргетную терапию

    Остеопороз и его осложнения:приверженность лечению и возможности ее повышения

    Objective. To estimate patient adherence to treatment with antiosteoporotic drugs within the first year after experienced low-trauma osteoporotic fractures (LOF). Material and methods. The data of a questionnaire survey were analyzed in 172 women aged 50 years or older (mean age 66+8 years) who had sustained fractures at different sites after a fall from standing height, were analyzed. Therapy and its compliance were assessed 12 months after LOF. Results and discussion. Among the 172 respondents, 55% received therapy, including 73% took only calcium preparations and vitamin D; 8% calcitonin; 14% bisphosphonates; 5% strontium ranelate. At the same time, 45% of those asked were given no antiosteoporotic drugs within the first year of life after fracture. Only 42%> continued to receive treatment following 12 months; 18% interrupted it within the first 4 months after fracture. Conclusion. In real clinical practice, osteoporosis was not treated in the majority of patients within the first year after LOF. One of the major reasons why it is not treated is the high cost of pathogenetic drugs. This reason can be successfully eliminated using generic drugs.Цель исследования - оценить приверженность пациентов лечению противоостеопоротическими препаратами в течение первого года после перенесенного малотравматичного остеопоротического перелома (МОП). Материал и методы. Проанализированы данные анкетирования 172 женщин 50 лет и старше (средний возраст - 66+8 лет), перенесших переломы различных локализаций в результате падения с высоты собственного роста. Оценка назначения терапии и приверженности лечению осуществлялась через 12 мес после МОП. Результаты и их обсуждение. Среди 172 анкетированных 55% пациенток получали терапию, в том числе 73% - только препараты кальция и витамина D, 8% - кальцитонин, 14% - бисфосфонаты и 5% - стронция ранелат. В то же время 45% опрошенных не получали никаких противоостеопоротических препаратов в течение первого года после перелома. Через 12 мес продолжали получать лечение только 42%> опрошенных, а прервали его в течение первых 4 мес после перелома - 18%. Заключение. В реальной клинической практике в течение первого года после МОП у большинства пациенток лечение остеопороза не проводилось. Одна из основных причин отсутствия терапии - высокая стоимость патогенетических препаратов. Данная причина может быть успешно устранена в случае применения дженерических лекарственных средств

    Sensitive issue of modern gynecology can be considered widespread and substantial "rejuvenation" of inflammatory diseases of the pelvic organs in women of reproductive age.Ascending path of infection prevails in the pathogenesis of inflammatory diseases of the internal genital organs. Invasion of microbes in the internal genital organs may occur during the various manipulations, different pelvic operations and  in the postpartum period. The degree of colonization of microorganisms of the vagina and cervix plays an important   role  in the development of the infectious process. In obstetrics and gynecology inflammatory diseases can be caused by pathogenic and non- pathogenic (opportunistic) microorganisms. Among the pathogens causing the defeat of the female genital organs, most often found N. gonorrhea, C. trachomatis, T. vaginalis. Opportunistic pathogens, part of the normal flora of the genital tract, in certain circumstances, can become agents of post-partum, post-abortion, post-operative complications and inflammatory diseases of the female genital organs. Among the opportunistic pathogens that are part of the normal microflora of the female genital organs, found hemolytic and non-hemolytic streptococci (the most important are streptococci groups A, B, D), coagulase-negative staphylococci and micrococci (allocated 60% and 35% of healthy women, respectively). They can cause secondary infectious processes of the urinary system, inflammatory diseases of the genital organs of pregnant women and mothers with immunosuppression. These microorganisms are often the agents of inflammatory diseases in the newborn, especially with low weight and malnutrition children. Gram-negative opportunistic bacteria that are isolated from the genital tract, can also be agents of inflammatory processes of various localization. Escherichia coli is the most frequently obtain and cause urinary tract infection in pregnant and postpartum women. It is also causative agent of postpartum sepsis and postoperative peritonitis. Among other gram-negative bacteria should be noted genus Klebsiella. Therefore, therapy of inflammatory diseases of the pelvic organs high demands due to the risk that represents this state to the reproductive function of patients. The mixed nature of the infection, the increasing rates of resistance of pathogens to antibiotics creates difficulties in selecting antimicrobial therapy, which remains the empirical  till now. The inclusion of specific bacteriophages and plant antiseptics in therapeutic regimens for local treatment of inflammatory diseases of the vagina associated with an excessive colonization of the vaginal mucosa by aerobic opportunistic bacteria justified and can serve as an alternative to the traditional antibiotic therapy.  All this determines the necessity for further studying   the etiology and pathogenesis of infectious and inflammatory complications, the development of new highly effective methods of prevention and treatment
