    Developing creative methodologies: using lyric writing to capture young peoples’ experiences of the youth offending services during the COVID-19 pandemic

    Purpose The COVID-19 lockdowns (2020–2021) disrupted all aspects of usual functioning of the criminal justice system, the outcomes and impact of which are largely still unknown. The pandemic has affected individuals across the wider society, this includes a negative impact on the social circumstances of children and young people involved within youth offending services (YOS) (Her Majesty’s Inspectorate of Probation, 2020; Criminal Justice Joint Inspectorates, 2021). This population frequently represents those from marginalised circumstances and are rarely given the opportunity to participate meaningfully in the services they are involved in. The purpose of this study was to explore the experiences of the young people serving orders with the YOS during Covid19 lockdowns and requirements. Design/methodology/approach This paper outlines a creative methodology and method used to uncover the experiences and perceptions of young people undergoing an order within a YOS during the COVID-19 lockdowns. The arts-based approach entailed a novel and creative method using a lyric artist to engage with young people through a virtual platform, supporting them to create lyrics about their experiences of the YOS during this time. Findings The artist developed a successful rapport with young people based on familiarity with, and passion for, music. He promoted their strengths, improving their confidence which was perceived to elicit more in-depth perspectives that might not have otherwise been obtained using more traditional methods. As such, the method and methodology outlined developed the young people’s social and communicative skills whilst producing meaningful feedback that can contribute to the YOS recovery plan and thus future of the service. Practical implications This paper reports on a novel arts-based research methodology, implemented to capture meaningful data from participants during the COVID-19 pandemic.     ‘Marked Out’: An exploration of the role of stigma in the marginalisation and racialisation of young people

    Stigma and the labelling process have been conceptualised and debated for decades (Link & Phelan, 1999). Stigma was first examined by Goffman (1963), whose work was valuable in explaining and drawing attention to stigmatisation and individual strategies of stigma management. This thesis moves beyond this individualising definition to consider stigma as a mechanism of power, examining the role of stigmatisers in conveying and (re)producing labels and stereotypes (Link & Phelan, 2001) through exploring the potential influences of institutional structures on stigmatisation. Through an examination of the lived realities of young people, this thesis seeks to understand the role and reproduction of stigma in their lives, tied to the determining contexts of race, gender and class, and created within constructs of power. This study provides an empirical example of stigma power in the lives of young people living in two Manchester neighbourhoods. Working with 28 young people aged 16-19 in two stigmatised areas of Manchester, this research was grounded in ethnographic principles, serving as a foundation for a participatory-informed research design developed to discover, interpret, and apply knowledge. In addition to conducting over 300 hours of participant observation of the young people’s lives, the research involved the use of participatory-informed, creative methods. In consultation with young people, this was facilitated through various creative approaches, including geographical mapping, creative writing, flashcards, and a blog. Based upon its findings, this thesis will argue that the lives of young people are layered through pre-existing faultlines that can be understood and determined within the structures of society. By revealing the experiences of young people through the institutional structures of public and media narratives, and police and educational encounters, this thesis illustrates how institutionalised narratives may contribute to the (re)production of stigmatised identities. By understanding how the lives of young people can become structured within the determining contexts of society, directly from the individuals to whom this applies, this thesis offers the potential to explore how the lives of some young people are (re)produced through cycles of stigma

This approach allowed participants to lead and voice their experiences in a more creative and participatory way than traditional methods. Through their lyrics, the young people articulated their perceptions of self which differed from how they felt others saw them. The value of relationships with practitioners and those outside the service were also explored. The benefits of the methods, to participants pro-social identity, and how they might be further employed across YJS are also discussed

    Novel scabies mite serpins inhibit the three pathways of the human complement system

    Scabies is a parasitic infestation of the skin by the mite Sarcoptes scabiei that causes significant morbidity worldwide, in particular within socially disadvantaged populations. In order to identify mechanisms that enable the scabies mite to evade human immune defenses, we have studied molecules associated with proteolytic systems in the mite, including two novel scabies mite serine protease inhibitors (SMSs) of the serpin superfamily. Immunohistochemical studies revealed that within mite-infected human skin SMSB4 (54 kDa) and SMSB3 (47 kDa) were both localized in the mite gut and feces. Recombinant purified SMSB3 and SMSB4 did not inhibit mite serine and cysteine proteases, but did inhibit mammalian serine proteases, such as chymotrypsin, albeit inefficiently. Detailed functional analysis revealed that both serpins interfered with all three pathways of the human complement system at different stages of their activation. SMSB4 inhibited mostly the initial and progressing steps of the cascades, while SMSB3 showed the strongest effects at the C9 level in the terminal pathway. Additive effects of both serpins were shown at the C9 level in the lectin pathway. Both SMSs were able to interfere with complement factors without protease function. A range of binding assays showed direct binding between SMSB4 and seven complement proteins (C1, properdin, MBL, C4, C3, C6 and C8), while significant binding of SMSB3 occurred exclusively to complement factors without protease function (C4, C3, C8). Direct binding was observed between SMSB4 and the complement proteases C1s and C1r. However no complex formation was observed between either mite serpin and the complement serine proteases C1r, C1s, MASP-1, MASP-2 and MASP-3. No catalytic inhibition by either serpin was observed for any of these enzymes. In summary, the SMSs were acting at several levels mediating overall inhibition of the complement system and thus we propose that they may protect scabies mites from complement-mediated gut damage

    Potassium Supplementation and Prevention of Atrial Fibrillation After Cardiac Surgery

    IMPORTANCESupplementing potassium in an effort to maintain high-normal serum concentrations is a widespread strategy used to prevent atrial fibrillation after cardiac surgery (AFACS), but is not evidence-based, carries risks, and is costly.OBJECTIVETo determine whether a lower serum potassium concentration trigger for supplementation is noninferior to a high-normal trigger.DESIGN, SETTING, AND PARTICIPANTSThis open-label, noninferiority, randomized clinical trial was conducted at 23 cardiac surgical centers in the United Kingdom and Germany. Between October 20, 2020, and November 16, 2023, patients with no history of atrial dysrhythmias scheduled for isolated coronary artery bypass grafting (CABG) surgery were enrolled. The last study patient was discharged from the hospital on December 11, 2023.INTERVENTIONSPatients were randomly assigned to a strategy of tight or relaxed potassium control (only supplementing if serum potassium concentration fell below 4.5 mEq/L or 3.6 mEq/L, respectively). Patients wore an ambulatory heart rhythm monitor, which was analyzed by a core laboratory masked to treatment assignment.MAIN OUTCOMES AND MEASURESThe prespecified primary end point was clinically detected and electrocardiographically confirmed new-onset AFACS in the first 120 hours after CABG surgery or until hospital discharge, whichever occurred first. All primary outcome events were validated by an event validation committee, which was masked to treatment assignment. Noninferiority of relaxed potassium control was defined as a risk difference for new-onset AFACS with associated upper bound of a 1-sided 97.5% CI of less than 10%. Secondary outcomes included other heart rhythm–related events, clinical outcomes, and cost related to the intervention.RESULTSA total of 1690 patients (mean age, 65 years; 256 [15%] females) were randomized. The primary end point occurred in 26.2% of patients (n = 219) in the tight group and 27.8% of patients (n = 231) in the relaxed group, which is a risk difference of 1.7% (95% CI, −2.6% to 5.9%). There was no difference between the groups in the incidence of at least 1 AFACS episode detected by any means or by ambulatory heart rhythm monitor alone, non-AFACS dysrhythmias, in-patient mortality, or length of stay.
