Natural History of the Peregrine Falcon in the Keewatin District of the Northwest Territories
A dense, productive population of peregrine falcons (Falco peregrinus) was studied for five years on a 450 sq km study area located along the northwest coast of Hudson Bay. The mean internest distance of 3.3 km represents the highest nesting density on record for the species in the Arctic. Morphometric and plumage characteristics of adults in the population suggest they are intermediate between F.p. tundrius from farther north and F.p. anatum from boreal regions to the south and west. The migratory pathway used by this population of birds is similar to that used by peregrines from Alaska, northern Canada, and Greenland. Both timing of arrival on the breeding grounds and clutch initiation are influenced strongly by spring weather patterns. Nineteen species of birds and three species of mammals were used as prey; however, in most years the bulk of the diet consisted of six species of birds and one mammal. A dramatic increase in the density of territorial peregrines in a year of high microtine rodent abundance, coupled with changes in various measures of reproductive performance, suggested that use of lemmings by falcons can be significant in some years. Suggestions that peregrines lay fewer eggs in the arctic part of their range were not supported. Broods hatched asynchronously, with the last-hatched young dying in about one-half of all broods of four. Surviving last-hatched young in broods of four grew at rates similar to older nest mates. Calibration strategies for use of the nanoDot OSLD in CT applications.
Aluminum oxide based optically stimulated luminescent dosimeters (OSLD) have been recognized as a useful dosimeter for measuring CT dose, particularly for patient dose measurements. Despite the increasing use of this dosimeter, appropriate dosimeter calibration techniques have not been established in the literature; while the manufacturer offers a calibration procedure, it is known to have relatively large uncertainties. The purpose of this work was to evaluate two clinical approaches for calibrating these dosimeters for CT applications, and to determine the uncertainty associated with measurements using these techniques. Three unique calibration procedures were used to calculate dose for a range of CT conditions using a commercially available OSLD and reader. The three calibration procedures included calibration (a) using the vendor-provided method, (b) relative to a 120 kVp CT spectrum in air, and (c) relative to a megavoltage beam (implemented with 60 Co). The dose measured using each of these approaches was compared to dose measured using a calibrated farmer-type ion chamber. Finally, the uncertainty in the dose measured using each approach was determined. For the CT and megavoltage calibration methods, the dose measured using the OSLD nanoDot was within 5% of the dose measured using an ion chamber for a wide range of different CT scan parameters (80-140 kVp, and with measurements at a range of positions). When calibrated using the vendor-recommended protocol, the OSLD measured doses were on average 15.5% lower than ion chamber doses. Two clinical calibration techniques have been evaluated and are presented in this work as alternatives to the vendor-provided calibration approach. These techniques provide high precision for OSLD-based measurements in a CT environment
Primary hemiarthroplasty for treatment of proximal humeral fractures
Background: Primary hemiarthroplasty of the shoulder is used to treat complex proximal humeral fractures, although the reported functional results following this method of treatment have varied widely. The aim of this study was to prospectively assess the prosthetic survival and functional outcomes in a large series of patients treated with shoulder hemiarthroplasty for a proximal humeral fracture. By determining the factors that affected the outcome, we also aimed to produce models that could be used clinically to estimate the functional outcome at one year following surgery.Methods: A thirteen-year observational cohort study of 163 consecutive patients treated with hemiarthroplasty for a proximal humeral fracture was performed. Twenty-five patients died or were lost to follow-up in the first year after treatment, leaving 138 patients who had assessment of shoulder function with use of the modified Constant score at one year postinjury.Results: The overall rate of prosthetic survival was 96.9% at one year, 95.3% at five years, and 93.9% at ten years. The overall median modified Constant score was 64 points at one year, with a typically good score for pain relief (median, 15 points) and poorer scores, with a greater scatter of values, for function (median, 12 points), range of motion (median, 24 points), and muscle power (median, 14 points). Of the factors that were assessed immediately after the injury, only patient age, the presence of a neurological deficit, tobacco usage, and alcohol consumption were significantly predictive of the one-year Constant score (p < 0.05). Of the factors that were assessed at six weeks postinjury, those that predicted the one-year Constant score included the age of the patient, the presence of a persistent neurological deficit, the need for an early reoperation, the degree of displacement of the prosthetic head from the central axis of the glenoid seen radiographically, and the degree of displacement of the tuberosities seen radiographically.Conclusions: Primary shoulder hemiarthroplasty performed for the treatment of a proximal humeral fracture in medically fit and cooperative adults is associated with satisfactory prosthetic survival at an average of 6.3 years. Although the shoulder is usually free of pain following this procedure, the overall functional result, in terms of range of motion, function, and power, at one year varies. A good functional outcome can be anticipated for a younger individual who has no preoperative neurological deficit, no postoperative complications, and a satisfactory radiographic appearance of the shoulder at six weeks. The results are poorer in the larger group of elderly patients who undergo this procedure, especially if they have a neurological deficit, a postoperative complication requiring a reoperation, or an eccentrically located prosthesis with retracted tuberosities.<br /

A Discrete Event Simulation model to evaluate the treatment pathways of patients with Cataract in the United Kingdom
Background The number of people affected by cataract in the United Kingdom (UK) is growing rapidly due to ageing population. As the only way to treat cataract is through surgery, there is a high demand for this type of surgery and figures indicate that it is the most performed type of surgery in the UK. The National Health Service (NHS), which provides free of charge care in the UK, is under huge financial pressure due to budget austerity in the last decade. As the number of people affected by the disease is expected to grow significantly in coming years, the aim of this study is to evaluate whether the introduction of new processes and medical technologies will enable cataract services to cope with the demand within the NHS funding constraints. Methods We developed a Discrete Event Simulation model representing the cataract services pathways at Leicester Royal Infirmary Hospital. The model was inputted with data from national and local sources as well as from a surgery demand forecasting model developed in the study. The model was verified and validated with the participation of the cataract services clinical and management teams. Results Four scenarios involving increased number of surgeries per half-day surgery theatre slot were simulated. Results indicate that the total number of surgeries per year could be increased by 40% at no extra cost. However, the rate of improvement decreases for increased number of surgeries per half-day surgery theatre slot due to a higher number of cancelled surgeries. Productivity is expected to improve as the total number of doctors and nurses hours will increase by 5 and 12% respectively. However, non-human resources such as pre-surgery rooms and post-surgery recovery chairs are under-utilized across all scenarios. Conclusions Using new processes and medical technologies for cataract surgery is a promising way to deal with the expected higher demand especially as this could be achieved with limited impact on costs. Non-human resources capacity need to be evenly levelled across the surgery pathway to improve their utilisation. The performance of cataract services could be improved by better communication with and proactive management of patients.Peer reviewedFinal Published versio
Nuclear Polarization of Molecular Hydrogen Recombined on a Non-metallic Surface

Measurement of Angular Distributions and R= sigma_L/sigma_T in Diffractive Electroproduction of rho^0 Mesons
Production and decay angular distributions were extracted from measurements
of exclusive electroproduction of the rho^0(770) meson over a range in the
virtual photon negative four-momentum squared 0.5< Q^2 <4 GeV^2 and the
photon-nucleon invariant mass range 3.8< W <6.5 GeV. The experiment was
performed with the HERMES spectrometer, using a longitudinally polarized
positron beam and a ^3He gas target internal to the HERA e^{+-} storage ring.
The event sample combines rho^0 mesons produced incoherently off individual
nucleons and coherently off the nucleus as a whole. The distributions in one
production angle and two angles describing the rho^0 -> pi+ pi- decay yielded
measurements of eight elements of the spin-density matrix, including one that
had not been measured before. The results are consistent with the dominance of
helicity-conserving amplitudes and natural parity exchange. The improved
precision achieved at 47 GeV,
reveals evidence for an energy dependence in the ratio R of the longitudinal to

Observation of a Coherence Length Effect in Exclusive Rho^0 Electroproduction
Exclusive incoherent electroproduction of the rho^0(770) meson from 1H, 2H,
3He, and 14N targets has been studied by the HERMES experiment at squared
four-momentum transfer Q**2>0.4 GeV**2 and positron energy loss nu from 9 to 20
GeV. The ratio of the 14N to 1H cross sections per nucleon, known as the
nuclear transparency, was found to decrease with increasing coherence length of
quark-antiquark fluctuations of the virtual photon. The data provide clear
evidence of the interaction of the quark- antiquark fluctuations with the
