    EFFECTS OF FUNCTIONAL KNEE BRACE ON LOWER EXTREMITY MUSCLE ACTIVATIONS AFTER ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTION

    The purpose of this study was to analyze effects of functional knee brace on muscular activation of lower extremity for the ACLR athletes during single leg drop landing. Marker trajectories, ground reaction forces and EMG signals were collected and synchronized by Vicon Nexus software. Mann-Whitney U test was used to compare muscle activations of ACLR athletes when wearing knee brace verses no knee brace, and the significance level was set at a = .05. The results showed that the GM, RF and BF muscle activation levels were significant increased when wearing knee brace than no knee brace at singleleg drop jump. Wearing knee brace for ACLR athletes can help to increase activation of knee flexors and help to stabilize knee joint during dynamic situation. We suggest that knee brace could help ACLR athlete to maintain better knee stability

    The A54T polymorphism at the intestinal fatty acid binding protein 2 is associated with insulin resistance in glucose tolerant Caucasians

    BACKGROUND: An A54T polymorphism at the fatty acid binding protein 2 (FABP2) locus was found to be associated with insulin resistance in non-diabetic Pima Indians. To see whether this association is present in other populations, we performed a cross sectional study to examine the role of this polymorphism on insulin resistance in 55 healthy and normotensive Caucasian subjects with normal glucose tolerance. Insulin sensitivity (%S) and beta cell function (%B) were assessed using the Homeostasis Model Assessment (HOMA). Their genotypes were determined using a polymerase chain reaction-restriction fragment length polymorphism assay. The relationship between the genotypes and the phenotypes was examined. RESULTS: After genotyping, we identified 24 AA, 27 AT and 4 TT subjects. The TT subjects were combined with the AT subjects during the analysis due to its small sample size. No differences were noted in gender distribution, clinical features, and fasting lipid profile between the two genotypic groups (AA vs. AT/TT). The AT/TT group had a higher fasting plasma insulin concentration and a lower %S than the AA group (p = 0.0444 and p = 0.0461, respectively). However, no differences were noted in plasma glucose concentrations and %B. Univariate analysis revealed that this polymorphism explained 7.3% of the variation in %S. Multivariate analysis revealed that the polymorphism was an independent determinant for %S (p = 0.0434) and with body mass index accounted for 28.7% of the variation in %S. In contrast, this polymorphism had no impact on %B. CONCLUSIONS: The A54T polymorphism at the FABP2 locus is a risk factor for insulin resistance in a Caucasian population

    Treatment of Type 2 Diabetes Mellitus in a Primary Care Setting in Taiwan: Comparison with Secondary/Tertiary Care

    BackgroundThis study investigated the status of diabetes control and management in patients treated in a primary healthcare setting and compared the results with data previously obtained for secondary/tertiary care patients in Taiwan.MethodsThis study was conducted at 51 primary healthcare stations randomly selected island-wide in Taiwan in 2001. A total of 1302 type 2 diabetes patients who had been followed-up for more than 1 year were included. Blood was collected for centralized HbA1c assay. The remaining data and information were collected by review of medical records and patient interview.ResultsCompared with the results of a previous study on patients treated in a secondary/tertiary care setting, a significantly smaller percentage of primary care patients were receiving insulin therapy. Primary care patients also had a shorter duration of diabetes, a higher HbA1c level, better blood pressure control and a lower prevalence of complications. The proportion of patients achieving optimal control of glycemia and blood pressure was low. Patients aged < 65 years had a significantly shorter duration of diabetes, poorer diabetes control and better blood pressure control than elderly patients aged ≥ 65 years. Primary care patients aged ≥ 65 years had a significantly higher frequency of stroke than those aged < 65 years. The elderly group of secondary/tertiary care patients had a significantly higher frequency of coronary heart disease and stroke. Duration of diabetes and hypertension were the leading risk factors for complications in diabetes patients treated in both primary and secondary/tertiary care settings.ConclusionDiabetes control was poorer in primary care than in secondary/tertiary care patients, but control of blood pressure was better in primary care patients. The shorter duration of diabetes and better control of blood pressure in primary care patients and in patients aged < 65 years compared with their elderly counterparts might be related to a lower prevalence of complications

    Inferring genetic interactions via a nonlinear model and an optimization algorithm

    <p>Abstract</p> <p>Background</p> <p>Biochemical pathways are gradually becoming recognized as central to complex human diseases and recently genetic/transcriptional interactions have been shown to be able to predict partial pathways. With the abundant information made available by microarray gene expression data (MGED), nonlinear modeling of these interactions is now feasible. Two of the latest advances in nonlinear modeling used sigmoid models to depict transcriptional interaction of a transcription factor (TF) for a target gene, but do not model cooperative or competitive interactions of several TFs for a target.</p> <p>Results</p> <p>An S-shape model and an optimization algorithm (GASA) were developed to infer genetic interactions/transcriptional regulation of several genes simultaneously using MGED. GASA consists of a genetic algorithm (GA) and a simulated annealing (SA) algorithm, which is enhanced by a steepest gradient descent algorithm to avoid being trapped in local minimum. Using simulated data with various degrees of noise, we studied how GASA with two model selection criteria and two search spaces performed. Furthermore, GASA was shown to outperform network component analysis, the time series network inference algorithm (TSNI), GA with regular GA (GAGA) and GA with regular SA. Two applications are demonstrated. First, GASA is applied to infer a subnetwork of human T-cell apoptosis. Several of the predicted interactions are supported by the literature. Second, GASA was applied to infer the transcriptional factors of 34 cell cycle regulated targets in <it>S. cerevisiae</it>, and GASA performed better than one of the latest advances in nonlinear modeling, GAGA and TSNI. Moreover, GASA is able to predict multiple transcription factors for certain targets, and these results coincide with experiments confirmed data in YEASTRACT.</p> <p>Conclusions</p> <p>GASA is shown to infer both genetic interactions and transcriptional regulatory interactions well. In particular, GASA seems able to characterize the nonlinear mechanism of transcriptional regulatory interactions (TIs) in yeast, and may be applied to infer TIs in other organisms. The predicted genetic interactions of a subnetwork of human T-cell apoptosis coincide with existing partial pathways, suggesting the potential of GASA on inferring biochemical pathways.</p

    Hepatic glucokinase promoter polymorphism is associated with hepatic insulin resistance in Asian Indians.

    BACKGROUND: The role of glucokinase (GCK) in the pathogenesis of maturity-onset diabetes of the young is well established. However, its role in the common form of type 2 diabetes is far from convincing. We investigated the role of the G-to-A polymorphism in the hepatic GCK promoter on insulin sensitivity and beta cell function in 63 normotensive Asian Indians with normal glucose tolerance. As proposed by Matsuda and DeFronzo, hepatic insulin sensitivity (ISI(H)) and total body insulin sensitivity (ISI(M)) were estimated from the oral glucose tolerance test. Beta cell function was estimated using %B from the Homeostasis Model Assessment and insulingenic index (dI/dG). RESULT: We identified 38 GG, 24 GA, and one AA subjects. The AA subject was pooled with the GA subjects during the analysis. No difference was noted in the demographic features between the two genotypic groups (GG vs. GA/AA). Compared to the GG group, the GA/AA group had a lower ISI(H) (p=0.002), a lower ISI(M) (p=0.009), a higher %B (p=0.014), and a higher dI/dG (p=0.030). Multivariate analysis revealed that this polymorphism is an independent determinant for ISI(H) (p=0.019) and along with age, waist-hip ratio, gender, and diastolic blood pressure accounted for 51.5% of the variation of ISI(H). However, this polymorphism was a weak, but independent determinant for ISI(M) (p=0.089) and %B (p=0.083). Furthermore, it had no independent effect on dI/dG (p=0.135). CONCLUSIONS: These data suggest that the G-to-A polymorphism in the hepatic GCK promoter is associated with hepatic insulin resistance in Asian Indians

    DEA-based integrated relationship of returns to scale – an application to road maintenance in Taiwan

    An Integrated Relationship of Returns to Scale (IRRS) associated with multiple-stage Data Envelopment Analysis (DEA) is proposed for identifying the returns to scale (RTS) among decision-making units (DMUs) appropriately and accurately. The validity and feasibility of the proposed method is tested by using 31 case studies on highway maintenance and construction offices based on the data provided by the Directorate General of Highways (Taiwan). The results show that the multi-stage DEA method with IRRS is more useful than the traditional single-stage DEA for evaluating the status of RTS for each DMU. Among the 31 units evaluated, 14 units are categorized as having increasing returns to scale, 4 have decreasing returns to scale, and no unit has constant returns to scale; the returns for the remaining 13 units cannot be determined

    SummaryTuberculum sellae meningioma is a common intracranial tumor. However, its coexistence with an intratumoral aneurysm is rare. Here, we present the case of a 65-year-old woman with progressive vision loss caused by a tuberculum sellae meningioma coexisting with an intratumoral anterior communicating artery aneurysm. Treatment modalities for patients with this rare coexisting pathology were reviewed. When an intracranial tumor is closely related to the major intracranial vessel, preoperative magnetic resonance imaging angiography, a safe and noninvasive imaging study, is suggested for the early diagnosis of a possible coexisting aneurysm and for reducing the risk of intraoperative aneurysm rupture

    Resonance in modulation instability from non-instantaneous nonlinearities

    To explore resonance phenomena in the nonlinear region, we show by experimental measurements and theoretical analyses that resonance happens in modulation instability (MI) from non-instantaneous nonlinearities in photorefractive crystals. With a temporally periodic modulation in the external bias voltage, corresponding to a modulation in the nonlinear strength, an enhancement in the visibility of MI at resonant frequency is reported through spontaneous optical pattern formations. Modeled by such temporally periodic nonlinear driving force to the system, theoretical curves obtained from a nonlinear non-instantaneous Schr\"{o}dinger equation give good agreement to experimental data. As MI is a universal signature of symmetry-breaking phenomena, our observation on the resonance in MI may provide a control on chaotic, solitary, and turbulence waves.Comment: 5 pages, 4 figure
