Improvement of Possibilities of Treating Pneumonias in Patients on the Background of Acute Myeloblast Leucosis in the Aspect of Immunoresistance Mechanisms
Aim: to analyze the influence of the immunomodeling preparation glutamyl-cysteinyl-glycine disodium (glutoxim) on indices of cellular and humoral immunity in patients with pneumonias on the background of acute myeloblast leucosis to form new approaches to the improvement of treating pneumonias in patients with immunity disorders.Materials and methods. The research group - 37 patients with pneumonia on the background of acute myeloblast leucosis, who underwent the program treatment on the base of the hematological center “MI city multi-profile clinical hospital №4” Dnipro city, 2014-2015. The age of patients from 23 to 45 years old; 10 women and 27 men. The diagnosis of leucosis and pneumonia forms was verified corresponding to modern conventional clinical and morphological criteria. Patients from the main research group (n = 18) were prescribed with glutamyl-cysteinyl-glycine disodium by the scheme 2 ml of 3 % (60 mg) i/v № 10 in mornings, summary dose - 300 mg. Indications of immunograms were studied: Т-lymphocytes, В-lymphocytes and their subpopulation composition (CD3 +, CD4 +, CD8 +, CD19 +, CD19-, CD16 +, CD56+) using the flowing laser cytofluorimetry. Immunoglobulin levels were determined by the method of immunoturbometry. Indications of immunograms were assessed in the treatment dynamics. The statistical processing – using packages of applied programs «Excel» and «Statistic 10».Results. According to the analysis of indices of the cellular and humoral immunity of patients with pneumonia on the background of acute myeloblast leucosis, the process of glutamyl-cysteinyl-glycine disodium use proved the statistically reliable activation of phagocytosis and anti-infectious defense indices. The dynamics of humoral immunity indices also proved the positive influence on the state of the immune reactivity of the organism with the reliable increase of ІgА and Іg М, responsible for neutralization of infectious agents and bacterial toxins.Conclusions: The use of the ummunomodeling preparation glutamyl-cysteinyl-glycine disodium (glutoxim) in patients with pneumonia on the background of acute myeloblast leucosis results in the improvement of indices of cellular and humoral immunity and phagocytosis activation. The research results prove the possibility of optimization of approaches to treating pneumonias in patients with severe immunity disorders by using immunomedeling therapy by glutamyl-cysteinyl-glycine disodium (glutoxim)
Application of “Polyana Kvasova” Mineral Water in the Complex Therapy of Patients with Gastro-esophageal Reflux Disease and Osteochondrosis of the Spine
Aim: To investigate the effectiveness of complex therapy using “Polyana Kvasova” mineral water in patients with GERD and osteochondrosis
(OH) of the cervical and thoracic spine.
Materials and Methods: The examined patients included 56 Helicobacter pylori (HP)-positive patients with GERD and OH of the cervical and
thoracic spine. The examined patients were divided into 2 groups depending on the scheme of complex treatment. All HP-positive patients
with GERD and OH underwent standard triple anti-helicobacter therapy and itopride hydrochloride. Group I patients (n=26) received only
the above-mentioned regimen. Patients of the II group (n=30) were additionally prescribed 100-150 ml of “Polyana Kvasova” carbonated
bicarbonate-sodium mineral water (MW) as a natural antacid, warm, still 4 times a day (1.5 hours before meals and after 45 minutes after –
both in the morning and evening) within 1 month.
Results: The complex therapy carried out had a positive effect on the endoscopic picture of GERD in patients with cervical and thoracic OH. The
decrease in the severity of RE was accompanied by an increase in the number of GERD patients in whom the erosive form of reflux esophagitis
was not detected during repeated fibroesophagogastroduodenoscopy (FEGDS) (mainly in the II group of examined patients – 10.0% of patients,
p<0.05). The additional prescription of “Polyana Kvasova” MW to the complex treatment increased the frequency of eradication of HP infection
by 9.2% – p<0.05 at the background of more pronounced normalization of clinical and endoscopic signs of GERD.
Conclusions: 1. GERD is often manifested by atypical extraesophageal symptoms (up to 25.0-27.3% of cases) in patients with OH of the
cervical and thoracic spine. 2. The use of standard triple AHT in combination with itopride hydrochloride is an effective method of correcting
esophageal and extraesophageal clinical manifestations of GERD in patients with cervical and thoracic spine OH. 3. The additional prescription
of “Polyana Kvasova” MW as part of the complex therapy of patients with GERD and OH of the cervical and thoracic regions is a safe, effective
method for reducing clinical symptoms, the severity of esophageal lesions with repeated FEGD, and also contributes to increasing the frequency
of HP infection eradication in the data patients

The problem of choice of optimal technological decisions on harvester control
The paper considers the problem of determining optimal technological decisions on a harvester control which includes the choice of the harvester adjustable parameters at presetting and also their updating in the process of operation. To solve this problem, an information system of decision making support based on fuzzy expert knowledge is used. The expedience of this approach is stipulated by the data fuzziness about the factors of the environment in which the harvester is operating and also by complex and system of uncertain interrelations among external factors, adjustable parameters and harvesting quality indices. For the case of harvesting grain crops an expert knowledge base has been made which makes it possible to formalize empirical knowledge about the dependencies of the harvester adjustable parameters on the environment factors such as crop yield, stand of grain humidity, stand of grain dockage rough straw. The most essential adjustable parameters have been considered: speed of the harvester motion, rotational speed of a threshing drum and rotor speed of a separator fan. Fuzzy logic inference has been performed with the help of Fuzzy Logic Toolbox application package (Matlab). The examples of technological decisions of the harvester preliminary adjustment at different environmental conditions have been presented, accurate values of the adjustable parameters were calculated by the «barycentre» method
