    Subtle Deficits in Instrumental Activities of Daily Living in Subtypes of Mild Cognitive Impairment

Copyright © 2010 S. Karger AG, Basel Background/Aims: Greater cognitive and functional deficits in mild cognitive impairment (MCI) are associated with higher rates of dementia. We explored the relationship between these factors by comparing instrumental activities of daily living (IADLs) among cognitive subtypes of MCI and examining associations between IADL and neuropsychological indices. Methods: We analyzed data from 1,108 MCI and 3,036 normal control subjects included in the National Alzheimer’s Coordinating Center Uniform Data Set who were assessed with the Functional Activities Questionnaire (FAQ). Results: IADL deficits were greater in amnestic than nonamnestic MCI, but within these subgroups, did not differ between those with single or multiple domains of cognitive impairment. FAQ indices correlated significantly with memory and processing speed/executive function. Conclusions: IADL deficits are present in both amnestic MCI and nonamnestic MCI but are not related to the number of impaired cognitiv

The definitive version was published in Journal of Geophysical Research 112 (2007): B07101, doi:10.1029/2006JB004435.Yearlong time series records of exit fluid temperature from the active TAG hydrothermal mound (Mid-Atlantic Ridge, 26°N) reveal a complex space-time pattern of flow variability within the mineral deposit. Exit fluid temperatures were measured every 8–10 min from 17 sites distributed across the upper terrace of the mound from June 2003 to June 2004. High-temperature records were obtained using Deep Sea Power and Light SeaLogger® probes deployed in fractures discharging ∼360°C black smoker fluids, and low-temperature records were obtained using VEMCO Ltd. Minilog probes deployed in cracks discharging ∼20°C diffuse flow fluids. The temperature records are considerably more variable than those acquired from vent fields on the fast spreading East Pacific Rise and exhibit a complex mix of both episodic and periodic variability. The diffuse flow records alternate between periods of discharge and periods of what I infer to be recharge when fluid temperatures are equal to background water column levels (∼2.7°C) as ambient seawater is drawn into the seafloor. The space-time patterns of these episodic variations suggest that they represent reorganizations of the secondary circulation system driving diffuse discharge on the upper terrace of the mound on timescales from a few hours to a few days, most likely in response to permeability perturbations. Harmonic temperature oscillations were observed over a range of periods, with the principal lunar semidiurnal tidal period (M2) being most dominant. During certain times, exit fluid temperatures at diffuse sites pulse at diurnal and semidiurnal tidal periods when they are hovering near background water column levels, which I interpret as flow reversals associated with the vertical displacement of a fluid boundary layer at the seafloor interface when the local net flux is near zero. The pulsing behavior is predicted by poroelastic models of tidal loading but is not consistent with effects from tidal currents, which demonstrates that poroelastic effects from tidal loading modulate shallow subsurface flow at the active TAG mound.This work was supported by the National Science Foundation (OCE-0137329)

    The presentation, clinical features, complications, and treatment of congenital dacryocystocele

    Purpose To determine the incidence and presenting features of congenital dacryocystocele in the United Kingdom. To report on those cases complicated by dacryocystitis, respiratory compromise, and the treatment undertaken. Methods A prospective observational study of cases of congenital dacryocystocele presenting in the United Kingdom between September 2014 and October 2015. Infants <3 months of age presenting with a cystic swelling in the medial canthal area were included. Cases were identified via the British Ophthalmology Surveillance Unit (BOSU) reporting system. Results A total of 49 cases were reported during the study period. This gives an incidence of 1 in 18 597 live births. There was a 71% response rate to the questionnaire. The average age at presentation was 16.94 days. Dacryocystoceles were unilateral in 91% of cases. Dacryocystitis was a complicating factor in 49% of patients and 17% had respiratory distress. Uncomplicated dacryocystocele responded well to conservative measures in 86%. Surgical intervention was required in 23% of patients. Those cases complicated by dacryocystitis (29%) and nasal obstruction (17%) were more likely to require surgical intervention compared to those with dacryocystocele alone (14%). Digital massage appears to reduce the likelihood of requiring surgical intervention. The mean time to resolution was 19 days. Conclusions Congenital dacryocystocele is a rare presentation in the United Kingdom. Dacryocystitis and respiratory compromise commonly complicate a dacryocystocele. The use of digital massage as an early intervention is advocated and conservative measures may be sufficient in cases of uncomplicated dacryocystocele

    The good, the bad and the ugly : Chinese imports, European Union anti-dumping measures and firm performance

    This paper analyses the effects of the European Union's anti-dumping tariffs against Chinese imports on all affected firms: “the good” European import-competing firms, “the bad” Chinese exporters and “the ugly” European importers of dumped products. The results show that temporary import tariffs are beneficial to the least productive “good” EU producers, but harms the most productive “ugly” EU importers. Overall, the net effects of anti-dumping policy on European employment and exports are largely negative. Also tariffs enhance the productivity of surviving “bad” Chinese exporters and widens the productivity gap with European competitors

    Sucrose diffusion in aqueous solution.

    The diffusion of sugar in aqueous solution is important both in nature and in technological applications, yet measurements of diffusion coefficients at low water content are scarce. We report directly measured sucrose diffusion coefficients in aqueous solution. Our technique utilises a Raman isotope tracer method to monitor the diffusion of non-deuterated and deuterated sucrose across a boundary between the two aqueous solutions. At a water activity of 0.4 (equivalent to 90 wt% sucrose) at room temperature, the diffusion coefficient of sucrose was determined to be approximately four orders of magnitude smaller than that of water in the same material. Using literature viscosity data, we show that, although inappropriate for the prediction of water diffusion, the Stokes-Einstein equation works well for predicting sucrose diffusion under the conditions studied. As well as providing information of importance to the fundamental understanding of diffusion in binary solutions, these data have technological, pharmaceutical and medical implications, for example in cryopreservation. Moreover, in the atmosphere, slow organic diffusion may have important implications for aerosol growth, chemistry and evaporation, where processes may be limited by the inability of a molecule to diffuse between the bulk and the surface of a particle

    Patterns and processes of alteration in the lavas and dykes of the Troodos Ophiolite, Cyprus

    Alteration patterns in the lavas and dykes of the Troodos Ophiolite, Cyprus, record a complex history of axial hydrothermal alteration, crustal aging, and subsequent uplift and emplacement of the ophiolite. Field mapping shows that distribution of five alteration zones, each with distinct mineralogical, geochemical, and hydrologie characteristics, is influenced by igneous stratigraphy, structure, and the nature and thickness of the overlying sediments. Paragenetic sequences of secondary minerals indicate that alteration conditions changed progressively as the crust cooled and moved off-axis. Along spreading axes, low temperatures (≤50°C) were maintained by the rapid flow of seawater in and out of the lavas, and only minimal alteration took place. In contrast, lower water/rock ratios and higher temperatures (>200°C) in the dykes promoted extensive seawater-rock interaction. Although the sharp rise in temperature between the two regimes generally coincides with the lava-dyke transition, late-stage intrusions or hydrothermal upwelling zones locally cause high-temperature alteration to extend upward into the lavas. As a segment of crust moved off-axis, temperatures remained low in the lavas and progressively decreased, from >250° to <80°C, in the dykes. High permeability in the uppermost lavas led to the downward migration of an oxidative alteration front whose thickness and spatial distribution was dependent upon the rate and nature of sedimentation and, thus, the original seafloor morphology. Although field relations show that alteration has a consistent vertical pattern in Troodos, the alteration zones are not laterally continuous, and the stratigraphie depth of their boundaries varies considerably

    Screening for multi-drug-resistant Gram-negative bacteria: what is effective and justifiable?

    Effectiveness is a key criterion in assessing the justification of antibiotic resistance interventions. Depending on an intervention's effectiveness, burdens and costs will be more or less justified, which is especially important for large scale population-level interventions with high running costs and pronounced risks to individuals in terms of wellbeing, integrity and autonomy. In this paper, we assess the case of routine hospital screening for multi-drug-resistant Gram-negative bacteria (MDRGN) from this perspective. Utilizing a comparison to screening programs for Methicillin-Resistant Staphylococcus aureus (MRSA) we argue that current screening programmes for MDRGN in low endemic settings should be reconsidered, as its effectiveness is in doubt, while general downsides to screening programs remain. To accomplish justifiable antibiotic stewardship, MDRGN screening should not be viewed as a separate measure, but rather as part of a comprehensive approach. The program should be redesigned to focus on those at risk of developing symptomatic infections with MDRGN rather than merely detecting those colonised

    Genome-Wide Association of Body Fat Distribution in African Ancestry Populations Suggests New Loci

    Central obesity, measured by waist circumference (WC) or waist-hip ratio (WHR), is a marker of body fat distribution. Although obesity disproportionately affects minority populations, few studies have conducted genome-wide association study (GWAS) of fat distribution among those of predominantly African ancestry (AA). We performed GWAS of WC and WHR, adjusted and unadjusted for BMI, in up to 33,591 and 27,350 AA individuals, respectively. We identified loci associated with fat distribution in AA individuals using meta-analyses of GWA results for WC and WHR (stage 1). Overall, 25 SNPs with single genomic control (GC)-corrected p-values<5.0×10−6 were followed-up (stage 2) in AA with WC and with WHR. Additionally, we interrogated genomic regions of previously identified European ancestry (EA) WHR loci among AA. In joint analysis of association results including both Stage 1 and 2 cohorts, 2 SNPs demonstrated association, rs2075064 at LHX2, p = 2.24×10−8 for WC-adjusted-for-BMI, and rs6931262 at RREB1, p = 2.48×10−8 for WHR-adjusted-for-BMI. However, neither signal was genome-wide significant after double GC-correction (LHX2: p = 6.5×10−8; RREB1: p = 5.7×10−8). Six of fourteen previously reported loci for waist in EA populations were significant (p<0.05 divided by the number of independent SNPs within the region) in AA studied here (TBX15-WARS2, GRB14, ADAMTS9, LY86, RSPO3, ITPR2-SSPN). Further, we observed associations with metabolic traits: rs13389219 at GRB14 associated with HDL-cholesterol, triglycerides, and fasting insulin, and rs13060013 at ADAMTS9 with HDL-cholesterol and fasting insulin. Finally, we observed nominal evidence for sexual dimorphism, with stronger results in AA women at the GRB14 locus (p for interaction = 0.02). In conclusion, we identified two suggestive loci associated with fat distribution in AA populations in addition to confirming 6 loci previously identified in populations of EA. These findings reinforce the concept that there are fat distribution loci that are independent of generalized adiposity
